奥氮平对精神分裂症患者血脂联素和瘦素水平的影响

目的 观测奥氮平治疗对精神分裂症患者体质量、血糖、血脂及血清脂联素和瘦素水平的影响.方法 在中山大学附属第三医院治疗的精神分裂症患者20例,给予奥氮平单药治疗达6周.测量治疗前后身高和体质量,并检测空腹血糖、血清甘油三酯、总胆固醇、高密度脂蛋白、血清脂联素和瘦素水平的变化.结果 奥氮平治疗6周后体质量、体质量指数、甘油三酯、胆固醇分别为(65.06±9.91)kg,(23.91±3.01)ks/m2,(2.15±0.15)mmoL/L,(5.91±1.47)mmol/L]比治疗前分别为(60.48±9.41)kg,(22.02±2.39)kg/m2,(1.26±0.51)mmol/L,(4.63±1.10)mmol/L]显著升高(P<0.05),治疗后脂联素、瘦素水平分别为(45.14±16.72)peCmi,(9.55±3.36)ns/m1]比治疗前分别为(41.22±16.36)μg/ml,(4.16±5.11)ng/ml]显著升高(P<0.05).spearman相关分析表明治疗后体质量与瘦素水平成明显正相关(r=0.560,P=0.046).结论 奥氮平治疗早期即可出现体质量增加和脂代谢异常,同时血脂联素和瘦素水平均升高.

Early effects of olanzapine on serum levels of adiponectin and leptin in schizophrenic patients

Objective To explore the early effects of olarmapine on body weight,serum levels of glucose,lipid ,adiponeetin and leptin in schizophrenic patients.Methods Twenty patients with schizophrenia who were treated in the department of psychiatry of the Third affiliated Hospital of Sun Yat-sen University were recruited into our study.All the patients have been initiated with a stable dose of olanzapine for six weeks.The height,weight,fasting blood glucose,lipid,adiponectin and leptin levels were investigated at the baseline and at six weeks.Results The weight(65.06±9.91)kg,BMI (23.91±3.01)kg/m2,blood triglyeeride (2.15±0.15)mmool/L,cholesterol(5.91±1.47)mmol/L increased significantly after six weeks" olanzpine treatment (P<0.05),adiponectin(45.14±16.72)μg/ml and leptin levels (9.55±3.36) ng/ml increased significantly after treatment (P<0.05).Spearman correlation analysis indicated that leptin is positively correlated with weight after six weeks (r=0.560,P=0.046).Conclusion Weight gain and lipid metablism disorder were considered to be an early change of the treatment of olanzapine.Meanwhile,adiponeetin and leptin levels increased after initiation of olanzapine therapy,suggesting that they played some role against APs associated-weight gain and lipid metabliam disorder.
