胸痹心痛中医病性证候要素分布特点

目的探讨胸痹心痛中医病性证素的分布、主要组合规律及冠心病不同类型证素分布。方法对223例胸痹心痛患者进行中医病性证素分布的临床调查，分析病性证素的分布及组合情况，并分析冠心病不同分型证素分布规律。结果胸痹心痛常见病性证素分布频率由高至低依次为血瘀、气虚、阴虚、痰浊、气滞、阳虚、寒凝；其组合情况：双因素组合中，气虚＋血瘀最多，其他依次为气虚＋阴虚、血瘀＋气滞、血瘀＋痰浊、气虚＋痰浊、阳虚＋血瘀、气虚＋阳虚、血瘀＋阴虚；三因素组合中，气虚＋阴虚＋血瘀最多，其次为气虚＋痰浊＋血瘀、气虚＋阴虚＋痰浊、气滞＋血瘀＋痰浊、阳虚＋血瘀＋气滞；冠心病心绞痛与心肌梗死中医病性证素分布存在差异。结论胸痹心痛在治疗上应标本兼顾，以益气活血、滋阴化痰的方药为主。

Distribution Characteristics of TCM Syndrome Factors in Chest Stuffiness and Pain

Objective： To discuss the distribution laws of TCM syndrome factors,their major combination and different types of coronary heart disease （CHD）. Methods： Though the clinical investigation of TCM syndrome factors in chest stuffiness and pain on 223 cases of patients,the distributions,the combinations and the distribution laws were analyzed of different types of coronary heart disease. Results： The descending order of frequency distribution of TCM syndrome factors in chest stuffiness and pain,as follows,the sta- sis,the qi deficiency,the yin deficiency,the phlegm,the qi stagnation,the yang deficiency and the chilliness. The descending order of frequency distribution of combinations situation with two factors combinations as follows qi deficiency ＋stasis,qi deficiency ＋yin deficiency,stasis ＋qi stagnation, stasis ＋phlegm,qi deficiency ＋ phlegm,yang deficiency＋stasis,qi deficiency＋yang deficiency,stasis ＋yin deficiency. Three-factor combina- tions indcluded qi deficiency＋yin deficiency＋stasis,qi deficiency＋phlegm＋stasis,qi deficiency＋yin deficiency＋ phlegm,qi stagnation＋stasis＋phlegm,yang deficiency＋stasis＋qi stagnation. The distribution of TCM syndrome factors in angina pectoris and myocardial infarction were different. Conclusion： The treatment of chest stuffiness and pain should aim directly at the symptoms and causes.h is also proved that some compound traditional Chinese herbal of medicines for suppfementing qi and activating blood circulation nourishing yin and resolving phlegm should be used in treatment of dhest stuffiness and pain.
