华支睾吸虫感染兔模型治疗前后抗体消长规律的研究

摘    要:目的通过对华支睾吸虫感染兔模型的研究,了解有效治疗前后血清抗体的消长规律,为探讨特异性抗体检测用于疗效考核的价值提供参考。方法自感染的鱼体分离华支睾吸虫囊蚴,经口感染7只家兔,14周后给予吡喹酮治疗,使用SPA-ELISA法检测感染及治疗前后血清IgG抗体,观察针对华支睾吸虫粗抗原的抗体消长规律。结果7只实验兔均感染成功,抗体水平在感染后2-4周内显示不同程度的上升,至第4周均达较高水平.8~12周继续上升至峰值(0.143—0.307)。经过有效驱虫治疗,特异性抗体水平总体上呈现下降趋势,大部分兔(71.4%)在治疗5周后下降显著。到死亡或饲养结束,71.4%的实验兔A值降至0.1以下,为感染前的1.28-1.80倍。此外,解剖观察显示。完全驱虫治疗7或10个月后的3只兔肝仍可见病理性结节。结论华支睾吸虫感染后IgG抗体水平的上升较早,部分兔感染后约2周就有明显上升,使用高敏感度的抗体检测方法有助于对部分感染者进行早期诊断、早期治疗。有效驱虫治疗后,大部分实验动物显示抗体水平先快速后缓慢下降的规律,血清抗体滴度的动态观察具有一定的疗效考核价值。此外,本研究显示单纯驱虫治疗并不能使华支睾吸虫感染动物的肝脏病理改变很快恢复,提醒我们在防治工作中须重视该寄生虫寄生所引起的长期肝脏损害,将预防感染放在第一位的同时.对感染者的治疗应联合使用肝脏康复药物。

Abstract:Objective To study the kinetics of serum antibody production in rabbit after infection with Clonorchis sinensis and to evaluate the feasibility of using specific antibody for the diagnosis and treatment Clonorchis sinensis infection. Methods Clonorchis sinensis metacercaria were isolated from the naturally infected fish. Seven rabbits were orally infected and then treated with praziquantel 14 weeks after infection. Serum IgG antibody was determined before and after treatment using SPA-ELISA method. Results Seven experimental rabbits were all infected. The level of their serum antibody was elevated starting at 2 to 4 weeks post-infection. The level of antibody production was peaked at 8 to 12 weeks. After effective therapy, the level of specific antibody was decreased. By the end of the experiment, 5 rabbits showed an obviously decreased at the 5^th week after treatment. To death or the end of breeding, the absorption values of 71.4% experimental rabbits decreased to 0.1 or below, 1.28-1.80 times of the pre-infection values. In addition, the dissection observation showed that the pathological nodules still could be seen in the 3 rabbit livers with ideal deworming effect 7 or 10 months after therapy. Conclusion Compared with the detection of parasite eggs in stool, specific IgG antibody could be detected early after Clonorchis sinensis infection. Treatment with anti-parasitic drug can not provide an immediate improvement in liver pathology. Combination therapy should be given for both parasite eradication and liver protection.
