应用组织芯片技术检测胃癌组织中PTEN、Survivin和p21ras蛋白的表达

目的 利用组织芯片技术观察胃癌组织Survivin蛋白、PTEN蛋白和p21ras蛋白的表达，探讨三者的相互关系及三者与胃癌病理学指标的相关性。方法 收集江汉大学附属医院1988～2003年行胃癌根治性手术切除的、有完整病理学检查资料的标本140例。构建胃癌组织芯片，采用免疫组织化学法检测PTEN蛋白（17A）、Survivin蛋白和p21ras蛋白（F132）在胃癌组织中的表达情况。结果 构建420点和288点组织芯片2枚，140例胃癌中有118例符合分析要求（组织芯片利用率为84．3％）。PTEN阳性率为76．3％（90／118），Survivin阳性率为52．5％（62／118），p21ras阳性率为78．8％（93／118）。PTEN与患者年龄，肿瘤浸润深度、组织学分型相关（均P〈0.05）。Survivin与UICC淋巴结分期、组织学分型相关（均P〈0．05）。p21ras与年龄相关（P〈0．05）。PTEN与Survivin、PTEN与p21ras、Survivin与p21ras均呈正相关（均P〈0.05）。Cox回归模型多因素分析，UICC淋巴结分期与预后相关（P〈0．05）。结论 PTEN蛋白表达是推测进展期胃癌的分子标志物。Survivin蛋白表达是推测胃癌淋巴结转移及转移程度的分子标志物。PTEN及Survivin蛋白表达与胃癌组织学分型有关。PTEN及p21ras蛋白表达与胃癌患者年龄有关。PTEN蛋白、Survivin蛋白及p21ras蛋白在胃癌中呈正相关关系。UICC胃癌淋巴结分期是重要的预后指标。

Expression of PTEN, Survivin and p21ras in Gastric Cancer by Tissue Microarray

Objective To characterize the relationship of PTEN,Survivin and p21ras,and the possible correlation between them and pathological characters and prognosis in gastric cancer patients.Methods Specimens of curative dissection between 1988 and 2003 were collected from the affiliated hospital of Jianghan University.All 140 patients had complete examination data.Gastric cancer tissue microarray was formed and used to detect the expression of PTEN(17A),Survivin and p21ras(F132) in gastric cancer by immunohistochemical method.Results 420 and 288 dots tissue microarray was formed.Of the 140 cases of gastric cancer,118 cases met the requirements of analysis(utilization rate was 84.3 %).Positive rate of PTEN,Surviving and p21ras was 76.3 %(90/118),52.5 %(62/118) and 78.8 %(93/118),respectively.There was positive correlation between PTEN and Survivin,PTEN and p21ras,Survivin and p21ras(P<0.05).PTEN was related with invasive depth,age and histological type(P<0.05),Survivin with UICC N stage and histological type(P<0.05),and p21ras with age(P<0.05).And Cox multivariate analysis demonstrated a significant correlation between lymph node metastasis and prognosis(P<0.05).Conclusion The expression of PTEN,Survivin and p21ras had a positive correlation with gastric cancer.PTEN expression was a marker to predict the advanced gastric cancer.Survivin expression was a marker to predict lymph node metastasis and the degree of lymph node metastasis in gastric cancer.The expression of PTEN and Survivin had a correlation with histological type in gastric cancer.The expression of PTEN and p21ras had a correlation with age in gastric cancer.UICC N stage is an important prognostic factor in gastric cancer.
