肾鹿角状结石开放手术中气压弹道碎石术的应用

目的：探讨和评价肾完全鹿角状或部分鹿角状结石开放手术中，气压弹道碎石术的应用效果。方法：回顾性分析23例患者采用肾盂及肾实质切开取石，15例患者采用肾盂或肾实质切开加气弹道碎石术取石资料。结果：单纯开放组出血平均（223．00±6．24）ml，手术时间（142．00±7．38）min，残余结石6例；气压弹道碎石组出血平均（116．00±2．27）ml，手术时间（108．00＋8．14）min，残余结石2例，手术顺利，无并发症。结论：目前，肾完全鹿角状或部分鹿角状结石取石治疗．仍是一个比较困难的问题，应用开放手术联合气压弹道碎石术取石，有利于缩短手术时间、减少手术中出血量、降低残余结石率。在基层医院是一种治疗鹿角状结石的较好方法。

Combination with open operative methods and lithotripsy for the removal of complex staghorn calculus

Objective： To evaluate the effect of combination with open operative methods and lithotripsy for complex staghorn calculus. Methods： The data of 23 cases treated with pyelolithotomy and nephrolithotomy,15 cases treated with pyelolithotomy or nephrolithotomy and lithotripsy were reviewed. Results： Patients who underwent simple open surgery,the average intraoperative blood loss was （223.00＋6.24） ml,the mean operative time was （142.00±7.38） rain,small stones were left in 6 cases; patients treated with open surgery and lithotripsy,the average intraoperative blood loss was （116.00±5.27） ml, the mean operative time was （108.00±8.14） min,small stones were left in 2 cases. Conclusion： Combination with open operative methods and lithotripsy for complex staghorn calculus,this procedure has the advantages of less bleeding,less operative time and less stones left, It is a good choice for the treatment of staghorn calculus in the primary hospitals.
