以“七麦数据”网站收录的中医移动医疗App作为研究对象,采用网络调查法和文献分析法,根据“七麦数据”对移动医疗App的分类,结合中医移动医疗App的信息服务内容和特点,将筛选出的中医移动医疗App划分为医疗健康类、中医养生类、知识传播类、全面综合类,并根据“七麦数据”网站中对各类中医移动医疗App的打分及累计下载量筛选出最具代表性的12款中医移动医疗App,从全面性、人性化、安全性、实用性4个一级指标和40个二级指标对其信息服务现状进行评价,指出当前中医移动医疗App信息服务存在的问题并提出建议。  相似文献

目的探讨"317护"健教平台在住院的慢性肝病患者健康教育应用中的临床效果。方法选择2018年1~6月我院170例15~65周岁的住院患者为研究对象,采用随机数字表法等分为观察组和对照组,对照组由责任护士进行常规面对面的健康教育指导,观察组采用"317护"微信平台开展健康教育,比较两组患者对入院须知、疾病知识、治疗方法、检查须知、药物知识、饮食指导、出院指导等知识的掌握程度,两组患者对所获健康知识方式的满意度。结果观察组患者对入院须知、疾病知识、药物知识、饮食指导等的知晓率达93.95%,知识掌握程度明显高于对照组的81.18%,差异有统计学意义(P0.05),观察组患者对健康教育的满意度高于对照组,差异有统计学意义(P0.05)。结论结合"317护"健教平台的管理模式,能提高患者对健康教育知识的知晓率,促进患者对疾病的自我管理,提高其对护理工作的满意度。  相似文献

以浙江省首届“云馆配”图书展示会为例,介绍了“云馆配”采访模式的具体实践,对“云馆配”与现场采访、征订目录采访、读者荐购、读者决策采购等不同采访模式进行了比较,指出了各种模式的优势与不足,提出了“云馆配”采访模式的优化发展策略。认为“云馆配”立足用户需求,不仅使图书馆(用户)、馆配商、出版社三方得到了及时、有效的沟通,还成为读者决策采购的一种新的积极尝试。“云馆配”采访模式已在疫情期间得到了较好应用,未来可辅助日常图书采访。  相似文献

Background

Solid organ transplantation is associated with a higher risk of Epstein-Barr virus (EBV)–related lymphoproliferative disease due to immunosuppressive regimen. Little evidence is currently available on post-transplant lymphoproliferative disorders (PTLDs) in the lung transplant (LuTx) setting, particularly in cystic fibrosis (CF) recipients.

Methods

We retrospectively analyzed all the cases of PTLDs that occurred in our LuTx center between January 2015 and December 2017. We reviewed clinical and radiologic data, donor and recipient EBV serostatus, immunosuppressive therapy, histologic data, and follow-up of these patients.

Results

A total of 77 LuTxs were performed at our center in the study period; 39 (50.6%) patients had CF; 4 developed EBV-related PTLDs. They were all young (17–26 years) CF patients with high serum EBV DNA load. Disease onset was within the first 3 months after LuTx. In 3 cases presentation was associated with fever and infection-like symptoms, whereas in 1 case radiologic suspicion arose unexpectedly from a CT scan performed for different clinical reasons. Diagnosis was reached through lung biopsy in all cases. All patients received rituximab,?cyclophosphamide, doxorubicin hydrochloride (hydroxydaunomycin), vincristine sulfate (Oncovin), and prednisone with variable response and complications.

Conclusion

In our experience, the early development of EBV-related PTLD was a highly aggressive, life-threatening condition, which exclusively affected young CF patients in the early post-transplant period. The rate of this complication was relatively high in our population.Diagnosis with lung biopsy is crucial in all suspected cases and regular monitoring of EBV DNA levels is of utmost importance given the high correlation with PTLDs in patients at increased risk.  相似文献

Diabetes is on the rise in India and recently shown to be increasing in the urban underprivileged. Lack of awareness of the disease, its complications, combined with lack of financial resources among the underprivileged, often results in late detection and more complications in them. To combat this, healthcare delivered at the doorstep through the use of a customised mobile medical van is a potentially attractive option.We used a customized mobile van (included trained personnel, glucose meters, fundus evaluation camera, apparatus for detection of neuropathy and foot circulation and net enabled Skype calling for remote consultation) for educating general population regarding healthy lifestyle and screening, management and intervention in patients with diabetes.The project covered 10 underprivileged areas (n, 2,31,000 people) in Delhi. Total of 24,072 individuals (10.9% of total population) attended 352 awareness sessions. A total 3,12,347 visits (included repeat visits) were carried out for screening, education and management for obesity and diabetes. During screening (n, 16,834), 2933 subjects (18.7%) had high random blood glucose levels (>200 mg/dL) and had a blood pressure averaging 127.1 ± 23.6/81.3 ± 16.6 mm of mercury (n, 16,339). A pre-post intensive lifestyle counselling for 6 months in a subset of 352 diabetic patients (of which 77.8% i.e. n, 274 were overweight/obese) showed a significant lowering in weight (p < 0.001). In addition, 292 frontline workers and 256 paramedical workers were given training regarding lifestyle and diabetes, over 20 sessions.Based on achievements of this project of spreading awareness, screening, and management of diabetes and obesity in the large number of individuals in urban underprivileged colony, we believe this project could be extended to other cities and rural areas of India, and to other developing countries as well.  相似文献

ObjectiveIndividuals with a diagnosis of multiple sclerosis (MS) often present with cognitive and motor deficits, and thus the ability to perform tasks that rely on both domains may be particularly impaired. Yet, dual-task walking studies yield mixed results. Individual variance in the ability to cope with brain insult and mobilize additional brain resources may contribute to mixed findings.MethodsTo test this hypothesis, we acquired event-related potentials (ERP) in individuals with MS and healthy controls (HCs) performing a Go/NoGo task while sitting (i.e., single task) or walking (i.e., dual-task) and looked at the relationship between task related modulation of the brain response and performance.ResultsOn the Go/NoGo task the MS group showed dual-task costs when walking, whereas HCs showed a dual-task benefit. Further, whereas the HC group showed modulation of the brain response as a function of task load, this was not the case in the MS group. Analysis for the pooled sample revealed a positive correlation between load-related ERP effects and dual-task performance.ConclusionsThese data suggest a neurophysiological marker of cognitive-motor dysfunction in MS.SignificanceUnderstanding neural processes underlying dual-task walking will help identify objective brain measurements of real-world issues and may improve assessment of MS.  相似文献

Photographs are necessary in the clinical practice of dermatology, but there are ethical implications to consider. Moreover, dermatologists must be aware of and comply with certain legal requirements affecting the use of photographs. The main ethical principles are respect for patient autonomy and the physician's obligation to do no harm. The law differentiates between 2 bases for protection: one concerns the photographed person's rights over the image and the other protects personal data. Recent legislation places restrictions on taking photographs and exhibiting them. Photographs taken to be stored with a medical history have not been called into question, but the physician is recommended to inform the patient that they exist. When a photograph is exhibited for the purpose of teaching or illustrating concepts, it is necessary to determine whether or not the patient can be identified. If the answer is yes, the patient must give explicit permission. Caution should be exercised when publishing medical photographs on social media.  相似文献

Study DesignThis is a two-group randomized controlled trial.IntroductionFinger stiffness after treatment for metacarpal fractures often occurs due to poor compliance to the conventional rehabilitation programs. Gamification has shown success in improving adherence to and effectiveness of various therapies.Purpose of the StudyThe purpose of this study was to evaluate whether gamification, using cost-effective devices was comparable with conventional physiotherapy in improving hand functions and adherence to rehabilitation in metacarpal fractures.MethodsA 2-group randomized controlled trial involving 19 patients was conducted. Participants were randomized to a control (conventional physiotherapy, n = 10) or interventional group (gamification, n = 9). The grips strength and composite finger range of motion were measured at the baseline and each follow-up together with Patient-Rated Wrist and Hand Evaluation scores and compliance.ResultsThere were no significant differences on improvements of grip strength (means difference 24.38 vs 20.44, P = .289) and composite finger range of motion (means difference 50.50 vs 51.11, P = .886). However, the gamification group showed better results in Patient-Rated Wrist and Hand Evaluation (mean 0.44 vs 8.45, P = .038) and compliance (P < .05). No adverse events were reported.DiscussionOur results suggest that gamification using a cost-effective device demonstrated similar effectiveness as conventional physiotherapy in post–metacarpal fracture rehabilitation.ConclusionsGamification using a mobile device is an inexpensive and safe alternative to conventional physiotherapy for hand rehabilitation after metacarpal fractures. It effectively serves as a guide for future development of cost-effective technology–enhanced therapy.  相似文献
