ObjectivesTo compare new bone formation in mandibular critical-sized bone defects (CSBDs) in healthy, diabetic, osteoporotic, and diabetic-osteoporotic rats filled with bioceramics (BCs) with or without bone marrow mesenchymal stem cells (BMSCs).MethodsA total of 64 adult female Sprague-Dawley rats were randomized into four groups (n = 16 per group): Group 1 healthy, Group 2 diabetic, Group 3 osteoporotic, and Group 4 diabetic-osteoporotic rats. Streptozotocin was used to induce type 1 diabetes in Group 2 and 4, while bilateral ovariectomy was used to induce osteoporosis in Group 3 and 4. The central portion of the rat mandibular symphysis was used as a physiological CSBD. In each group, eight defects were filled with BC (hydroxypatatite 60% and β-tricalcium phosphate 40%) alone and eight with BMSCs cultured on BC. The animals were sacrificed at 4 and 8 weeks, and the mandibles were processed for micro-computed tomography to analyze radiological union and bone mineral density (BMD); histological analysis of the bone union; and immunohistochemical analysis, which included immunoreactivity of vascular endothelial growth factor (VEGF) and bone morphogenetic protein 2 (BMP-2).ResultsIn all groups (healthy, diabetics, osteoporotics, and diabetics-osteoporotics), the CSBDs filled with BC + BMSCs showed greater radiological bone union, BMD, histological bone union, and more VEGF and BMP-2 positivity, in comparison with CSBDs treated with BC alone (at 4 and 8 weeks).ConclusionsApplication of BMSCs cultured on BCs improves bone regeneration in CSBDs compared with application of BCs alone in healthy, diabetic, osteoporotic, and diabetic-osteoporotic rats.  相似文献

目的:研究七氟烷预处理对大鼠骨髓间充质干细胞(MSCs)在缺氧复氧环境下的作用和机制初探。方法:分离培养SD大鼠骨髓间充质干细胞,通过流式细胞仪和诱导分化培养鉴定其免疫表型和分化潜能。细胞随机分为3组:组1空白对照组;组2缺氧复氧组(缺氧24 h,复氧24 h);组3七氟烷预处理组(2%七氟烷预处理2 h后,缺氧24 h,复氧24 h)。处理后收集各组细胞,用流式细胞仪测定其凋亡率,线粒体膜电位变化及细胞周期。Western blot测定细胞外调节蛋白激酶(ERK)的蛋白表达。结果:与组2相比,组3 MSCs凋亡率明显下降,处于S期细胞增多,细胞增殖能力增强。Western blot结果显示:与组2相比,组3磷酸化ERK蛋白表达上升。结论:七氟烷预处理能提高缺氧复氧环境下骨髓间充质干细胞存活率,降低MSCs凋亡数量,并能增强MSCs的增殖能力,这一机制与七氟烷促进MSCs细胞p-ERK蛋白表达相关。  相似文献

目的:探讨扶正解毒抗癌方对晚期非小细胞肺癌(non-small cell lung cancer,NSCLC)的疗效及对患者化疗后骨髓抑制的影响。方法:选取2016年1月至2019年3月滁州市第一人民医院收治的NSCLC患者100例,按照随机数字表法分为对照组和研究组,每组50例。对照组患者采用常规化疗,研究组患者在对照组的基础上加用扶正解毒抗癌方,两组患者连续治疗2个化疗周期。观察两组患者的临床疗效,分别于治疗前后测定白细胞计数、血小板计数及血红蛋白水平,比较两组患者治疗后骨髓抑制发生情况和卡氏(Karnofsky,KPS)评分的差异。结果:研究组患者的总有效率为62.00%(31/50),明显高于对照组的36.00%(18/50),差异有统计学意义(P<0.05)。治疗后,两组患者白细胞计数、血小板计数及血红蛋白水平均明显低于治疗前,但研究组患者上述指标水平明显高于对照组,差异均有统计学意义(P<0.05)。研究组患者骨髓抑制发生率为38.00%(19/50),明显低于对照组的68.00%(34/50),差异有统计学意义(P<0.05)。治疗后,研究组患者KPS评分提高率和稳定率分别为34.00%(17/50)、46.00%(23/50),明显优于对照组的14.00%(7/50)、16.00%(8/50),差异均有统计学意义(P<0.05)。结论:扶正解毒抗癌方联合常规化疗治疗晚期NSCLC的疗效显著,能够减轻患者化疗后骨髓抑制情况,提高患者生活质量。  相似文献

Myelofibrosis (MF) is a BCR-ABL1 myeloproliferative neoplasm that arises from hematopoietic stem and progenitor cells frequently harboring a somatic driver mutation in 1 of 3 genes: JAK2, CALR, or MPL. The pathologic features of this hematologic malignancy include myeloproliferation, diffuse bone marrow fibrosis, and overactivation of the JAK-STAT pathway, resulting in enhanced inflammatory cytokine release. The common clinical manifestations of MF include systemic symptoms, abnormal peripheral blood count levels, and splenomegaly. However, it has become increasingly appreciated that significant clinical heterogeneity exists among patients with MF. Two distinct MF clinical phenotypes include the myeloproliferative and myelodepletive phenotype, with peripheral blood counts being the main discerning feature. Patients with the myeloproliferative phenotype will present with elevated peripheral blood counts and often experience significant constitutional symptoms and progressive splenomegaly. In contrast, patients with the myelodepletive phenotype will have low peripheral blood counts and will frequently require transfusion support. Current frontline therapies for MF, include ruxolitinib and fedratinib, which can exacerbate cytopenias and thereby pose an impediment to effective treatment of the myelodepletive patient. The present review discusses the clinical and prognostic implications of the myelodepletive phenotype and the therapeutic options and limitations for this subset of patients, representing an unmet clinical need.  相似文献

IntroductionThe presence of FMS-like tyrosine kinase 3 (FLT3) internal tandem duplication (ITD) mutation in pediatric acute myeloid leukemia (AML) is associated with high rates of induction failure and worse survival. Its presence places the patient into a high-risk group. We aimed to describe the outcome of pediatric AML with FLT3-ITD mutation.Patients and MethodsWe performed a retrospective analysis of cases of AML from July 2007 till July 2017 at Children’s Cancer Hospital Egypt.ResultsSeventy-one patients had FLT3 gene mutation out of 687 patients with AML. Sixty-five patients had FLT3 gene mutation with allelic ratio > 0.4; 43 (66.1%) of 65 patients experienced complete remission (CR). Of the 43 patients, 16 patients maintained CR, 18 patients relapsed after first CR, 8 patients died, and 1 patient was lost to follow-up. Patients with relapsing disease died after salvage chemotherapy, except for one patient, who was alive after second CR. Allogeneic bone marrow transplantation (allo-BMT) was performed for 9 (13.8%) of 65 patients in first CR, of whom 8 were alive and in CR, and 1 patient experienced disease relapse and died. Seven patients (10.7%) were alive without allo-BMT. Three years’ overall and event-free survival for patients with FLT3-ITD mutation with high allelic ratio was 26.9% and 22.8%, respectively. Three years’ overall and event-free survival for patients treated with allo-BMT was 77.8% and 78.8%, respectively, versus patients treated without allo-BMT, 16.3% and 12.8%, respectively.ConclusionFLT3-ITD mutation in pediatric AML was associated with poor treatment outcomes, and the survival of relapsing patients was extremely poor. Allo-BMT in first remission was the best treatment option. Alternative donor transplants and FLT3 inhibitors are needed to improve outcome in developing countries.  相似文献

目的从基因分子水平探讨酒精性骨质疏松(alcoholic osteoporosis,AOP)大鼠的发病机制,观察生髓健骨胶囊对AOP大鼠骨密度(bone mineral density,BMD)、骨矿含量(bone mineral content,BMC)表达的影响,探讨生髓健骨胶囊对AOP大鼠模型的中药防治作用机理。方法选取成年雄性(清洁级)SD大鼠120只,称体重,随机分为4组,每组各30只,用白酒灌胃法造模,同时分别给予生理盐水、碳酸钙阿法D3、生髓健骨胶囊灌胃给药。于造模8、12、16周末取材,检测股骨上端BMD、BMC指标。结果检测造模干预8、12、16周后BMD、BMC指标变化,模型组BMD、BMC与正常组比较明显降低,且差异有统计学意义(P0.01),结果表明饮酒大鼠确实存在骨量减少,BMD、BMC降低;中药干预组BMD、BMC与模型组相比显著升高(P0.01);中药干预组BMD、BMC与西药对照组相比,明显升高(P0.05)。结论通过观察生髓健骨胶囊对AOP大鼠的实验指标,证明了生髓健骨胶囊能够提高AOP大鼠骨密度,增加骨矿含量,抑制矿物质丢失,改善大鼠的骨代谢。  相似文献

Recent experimental strategies to reduce graft-versus-host disease (GVHD) have focused largely on modifying innate immunity. Toll-like receptor (TLR)-driven myeloid differentiation primary response 88 (MyD88)-dependent signalling pathways that initiate adaptive immune function are also critical for the pathogenesis of GVHD. This study aimed to delineate the role of host MyD88 in the development of acute GVHD following fully major histocompatibility complex-mismatched allogeneic bone marrow transplantation (BMT). When myeloablated BALB/c MyD88 knock-out recipients were transplanted with C57BL/6 (B6) donor cells, they developed significantly more severe GVHD than wild-type (WT) BALB/c hosts. The increased morbidity and mortality in MyD88–/– mice correlated with increased serum levels of lipopolysaccharide and elevated inflammatory cytokines in GVHD target organs. Additionally, MyD88 deficiency in BMT recipients led to increased donor T cell expansion and more donor CD11c+ cell intestinal infiltration with apoptotic cells but reduced proliferation of intestinal epithelial cells compared with that in WT BMT recipients. Decreased expression of tight junction mRNA in epithelial cells of MyD88–/– mice suggested that MyD88 contributes to intestinal integrity. Cox-2 expression in the GVHD-targeted organs of WT mice is increased upon GVHD induction, but this enhanced expression was obviously inhibited by MyD88 deficiency. The present findings demonstrate an unexpected role for host MyD88 in preventing GVHD after allogeneic BMT.  相似文献

BackgroundPatients undergoing bone marrow transplantation or chemotherapy for cancer are profoundly immunosuppressed. They are at risk for both endogenous and exogenous infections and require enhanced protection from infection while in hospital.ObjectivesThe aim of this narrative review was to determine the optimal design features of bone marrow transplant (BMT) units for reducing infection risk in these vulnerable patients.SourcesA literature search was performed on PubMed and other databases for documents published between January 2000 and October 2021. Keywords were: bone marrow transplant unit OR hematopoietic stem cell transplant unit OR haematology unit OR haemato-oncology unit AND design OR design guidelines OR design criteria OR ventilation specification OR HEPA filtration OR water outbreaks OR water system design.ContentGuidelines and other papers pertaining to BMT unit design are discussed. Key design features identified from the literature to reduce infection risks include high efficiency particulate air filtration, positive-pressure ventilation, sufficient air changes and sealed rooms. The evidence for each of these parameters and other findings are discussed. We found no guidelines specific to water quality and control in BMT units.ImplicationsGuidelines on the various components of design were found, but no comprehensive guidance documents addressing all relevant aspects, such as ventilation, water, and other design features, were found. Literary publications and policy documents were combined and summarised to highlight key design features aimed at reducing infection risk in this vulnerable patient group. We propose the development of international guidance for the design of BMT units encompassing all components.  相似文献
