目的:观察烤瓷冠修复前行高频电刀龈成形术对基牙牙周组织的影响.方法:收集需行高频电刀龈成形术后再作烤瓷冠修复的患者91例135颗牙,修复后1年复查牙周情况,分析患者术后1年牙龈成形效果、牙龈指数、牙菌斑指数、牙周探诊深度、牙齿动度.结果:龈成形术后烤瓷冠修复1年5.9%患者出现牙龈增生或退缩;牙龈指数轻度上升,有显著性差异;菌斑指数、牙周探诊深度、牙齿动度无明显差异.结论:高频电刀龈成形术对其后烤瓷冠修复基牙牙龈成形效果良好,对基牙牙周健康无明显不良影响. 相似文献

Objective: Severe atherosclerosis of the ascending aorta and arch frequently causes difficulties during heart operations, hindering surgical manoeuvres and potentially leading to systemic embolism. The aim of our study was to assess the safety and effectiveness of replacing the atherosclerotic ascending aorta in this setting. Methods: Aortic atherosclerosis was characterized by epiaortic ultrasonographic scanning in 90.1% of 1927 consecutive adult patients undergoing cardiac operations, and by computed tomographic chest scanning in selected cases. Thirty-six of the 152 patients requiring major derangements from our standard practice due to aortic atherosclerosis underwent replacement of the ascending aorta and constitute the study group. Replacement of the aorta was extended to the arch in 13 cases (36.1%). It was associated with single or multiple valve surgery in 34 patients (94.4%) and with coronary revascularization in 30 (83.3%). Two patients (5.6%) underwent coronary bypass grafting without valve surgery. A cryoablation procedure was associated in three patients with permanent atrial fibrillation. Deep hypothermic circulatory arrest was employed in 34 patients (94.4%), while proximal aortic disease allowed conventional distal crossclamping in 2 cases. The risk of operative mortality was estimated by the logistic EuroSCORE both with and withholding the variable ‘surgery of the thoracic aorta’. All survivors were followed-up for 1–41 months (16 ± 12). Results: Two patients died in the hospital (5.6%) and two during follow-up, for a cumulative survival of 91.3% and 85.6% at 1 and 3 years, respectively (hospital deaths included). The hospital death rate compared favourably with the expected estimates of 25.5% (p < 0.05) and 10.3% (p = 0.67) obtained by the EuroSCORE full model and without ‘aortic surgery’, respectively. In-hospital adverse neurologic events occurred in six patients (16.7%), including stroke in one patient (2.8%) and neurocognitive disturbances in five (13.9%), although they were all transient and cleared before discharge. Excess bleeding required re-exploration in four patients (11.1%), and one more patient underwent emergency grafting for acute postoperative coronary occlusion. Ten patients (38.5%) were intubated for longer than 24 h. Conclusion: Despite significant perioperative morbidity, replacement of the severely atherosclerotic aorta is worth consideration to avert expectedly higher death and stroke rates. 相似文献

ObjectiveThis study evaluated the effect of dwell time (conventional or extended) and cooling protocol (fast or slow) of self-glaze firings on the mechanical (flexural strength and crack propagation) and optical (color and translucency) properties of a porcelain-veneered zirconia system.MethodsBilayer disc-shaped samples were prepared (Vita VM9 + In-Ceram YZ) and divided according to the final thermal treatment: glaze firing followed by slow cooling (furnace opening at 200 °C) (G-S) or fast cooling (furnace opening at 600 °C) (G-F, manufacturer-recommended protocol), extended glaze firing (15 min of dwell time) followed by slow cooling (EG-S) or fast cooling (EG-F), or no thermal treatment (CTRL). Porcelain roughness (Ra and Rz) was measured before and after glaze firings. Color (ΔE00) and translucency (TP00) alteration were also evaluated. Flexural strength was measured with the piston-on-three-ball test and crack propagation analysis was performed after Vickers indentations. Complementary analyzes of crystalline phase and scanning electron microscopy were carried out.ResultsSignificant effect of dwell time was observed, with extended glaze leading to higher flexural strength and shorter crack lengths. Cracks of EG groups were observed to end in clusters of crystals. Color and translucency changed below perceptibility thresholds. All treatments led to a smoother surface and EG groups reached the lowest Rz values. An extra SiO2 peak was revealed in control and EG groups. No effect of cooling protocol was found.SignificanceExtended glaze firing was able to improve the resistance to crack initiation and propagation of porcelain-veneered zirconia without clinically perceptible changes in optical properties. 相似文献

PURPOSE
To compare the shear bond strength of various veneering materials to grade II commercially pure titanium (CP-Ti).MATERIALS AND METHODS
Thirty specimens of CP-Ti disc with 9 mm diameter and 10 mm height were divided into three experimental groups. Each group was bonded to heat-polymerized acrylic resin (Lucitone 199), porcelain (Triceram), and indirect composite (Sinfony) with 7 mm diameter and 2 mm height. For the control group (n=10), Lucitone 199 were applied on type IV gold alloy castings. All samples were thermocycled for 5000 cycles in 5-55℃ water. The maximum shear bond strength (MPa) was measured with a Universal Testing Machine. After the shear bond strength test, the failure mode was assessed with an optic microscope and a scanning electron microscope. Statistical analysis was carried out with a Kruskal-Wallis Test and Mann-Whitney Test.RESULTS
The mean shear bond strength and standard deviations for experimental groups were as follows: Ti-Lucitone 199 (12.11 ± 4.44 MPa); Ti-Triceram (11.09 ± 1.66 MPa); Ti-Sinfony (4.32 ± 0.64 MPa). All of these experimental groups showed lower shear bond strength than the control group (16.14 ± 1.89 MPa). However, there was no statistically significant difference between the Ti-Lucitone 199 group and the control group, and the Ti-Lucitone 199 group and the Ti-Triceram group. Most of the failure patterns in all experimental groups were adhesive failures.CONCLUSION
The shear bond strength of veneering materials such as heat-polymerized acrylic resin, porcelain, and indirect composite to CP-Ti was compatible to that of heatpolymerized acrylic resin to cast gold alloy. 相似文献7.

Objectives
To assess transient and residual stresses within the porcelain of veneered restorations (zirconia and metal) as a result of cooling rate and porcelain thickness.Methods
Porcelain-on-zirconia (PZ) and porcelain-fused-to-metal (PFM) crowns were fabricated with 1 or 2 mm of porcelain. Thermocouples were attached both internally and externally to the crowns to record transient temperatures. For fast cooling, the furnace was opened after the holding time and switched off. Slow cooling was accomplished by opening the furnace at 50 °C below the glass transition temperature (Tg) of the material. An axially symmetric FEA model simulated thermal stresses. Time-dependent temperature equations from thermocouple readings were set as boundary conditions. Framework materials and the porcelain below Tg were considered to behave elastically. Visco-elastic behavior was assumed for porcelain above the Tg modeling properties as dependent on cooling rate.Results
Differences in residual stress were found for fast and slow cooled PZ and PFM crowns. Significant transient stress waves were observed within the porcelain when fast cooling through Tg. They are believed to be related to non-uniform volumetric changes originated from thermal gradients. Results were confirmed by modeling and physical testing of crowns containing a defect.Significance
Residual stresses do not distinguish PZ from PFM. High magnitude transient stresses observed within the porcelain during fast cooling may explain clinical fractures involving internal defects. Stress waves may also originate internal micro-cracking which could grow under function. Therefore, slow cooling, especially for all-ceramic crowns with thick porcelain, is important to prevent thermal gradients and high-magnitude transient stresses. 相似文献10.

ObjectiveTo investigate the association of the failure of porcelain laminate veneers with factors related to the patient, material, and operator.MethodsThis clinical survey involved 29 patients (19 women and 10 men) and their dentists, including undergraduate and postgraduate dental students and dental interns. Two questionnaires were distributed to collect information from participants. All patients were clinically examined. Criteria for failure of the porcelain laminate veneers included color change, cracking, fracture, and/or debonding.ResultsA total of 205 porcelain laminate veneers were evaluated. All of the restorations were fabricated from IPS e.max Press and cemented with Variolink Veneer (Ivoclar Vivadent, Schaan, Principality of Liechtenstein) or RelyX veneer cement (3M ESPE, St. Paul, MN, USA). The preparations were generally located in enamel (58.6%), and most veneers had an overlapped design (89.7%). Ten patients (34.48%) showed veneer failure, most often in terms of color change (60%). Overall, 82.8% of patients were satisfied with their restorations.ConclusionInsufficient clinical skills or operator experience resulted in restoration failure in one-third of patients. 相似文献
