Therapeutic management in trochanteric fractures

The authors present the management of trochanteric fractures based on 343 fractures treated in Department of Orthopaedics and Trauma in Clinical Hospital of Emergencies of Ia?i for 5 years. The non-operative treatment was used in 17.2% of cases and the conservative methods were plaster immobilization, continuous traction and early mobilization. The surgical treatment was used in 82.8% of cases. The reduction of the fractures was usually possible using closed methods. For fixation, the authors used four types of implants: the 135 degrees blade-plate in 9.2% of cases, the condylar blade plate in 44.7%, Ender nails in 45.4% and DHS in 0.6% of cases. The choice of one of this methods depends on the type of fracture, age of the patient and on his biological status.     

The behavior of the fibrinolytic system during long-term orthopedic immobilization]

Our previous experimental studies on rats motionless for 7 to 60 days in special devices limiting their movements revealed a significantly increased activity of the fibrinolytic system (Groza, Artino) due to the "detention stress" rather then to the immobilization. Starting from these studies we have tried to observe the behaviour of the fibrinolytic system during long-term orthopedic immobilization (7-28 days) on patients having different injuries of the lower limb and submitted to orthopedic therapy (with or without osteosynthesis) to which an anticoagulant preventive treatment was added (heparin or low-molecular-weight substitutes such as Clivarine, Fraxiparine). We studied on 23 patients (11 male and 12 female) motionless for 14, 21, 28 days the plasma fibrinolytic activity (PFA) through euglobulin lysis time (ELT). Clinical investigation revealed that PFA did not change significantly during long-term orthopedic immobilization regardless of the duration of immobilization (14,21,28 days). Rosenfeld et al. (1994) described in healthy volunteers on bedrest for 36 hours an increase of PFA beginning at 24 hours of immobilization, this variation being capable of preventing stasis effects. Our results suggest that preventive anticoagulant therapy properly given during immobilization prevents thromboembolic events.     

Intuitionistic fuzzy system for fingerprints authentication

Using a fuzzy logic control system, we propose in this paper an optimized method to reduce the points number to be used in order to identify a person using fuzzy fingerprints. Two fingerprints are similar if n out of N points from the skin are identical. We discuss a criteria to choose these points. Our method compares two matching sets and selects the optimal set from these, using a fuzzy reasoning system based on intuitionistic fuzzy numbers. The advantage of our method with respect to the classical existing methods consists in a smaller number of calculations.     

Residual Stress State of Brazed Ceramic/Metal Compounds, Determined by Analytical Methods and X-ray Residual Stress Measurements

The residual stress state of brazed ceramic/metal compounds is described by means of X-ray residual stress determinations and analytical calculations using a model of three elastic infinite plates. It is shown that the residual stress state of the soldered compound depends on the materials combination and on the geometrical conditions. The combination of X-ray residual stress measurements and analytical calculations allows decisions on whether the assumption of a linear elastic model, based on elementary bending theory, is valid for the particular compounds.     

Generalized modus Ponens using various implications and T-norm product with threshold

Using Generalized Modus Ponens reasoning, we examine the values of the inferred conclusion depending on the correspondence between the premise of the rule and the observed fact. The conclusion is obtained using a set of implications in order to represent a fuzzy if-then rule with a single input single output and the t-norm with threshold generated by t-norm T(x,y)?=?xy, as a compositional operator. Some comments and an example are presented in order to show how the obtained results can be used.     

A software-defined communications baseband design

Software-defined radios offer a programmable and dynamically reconfigurable method of reusing hardware to implement the physical layer processing of multiple communications systems. An SDR can dynamically change protocols and update communications systems over the air as a service provider allows. In this article we discuss a baseband solution for an SDR system and describe a 2 Mb/s WCDMA design with GSM/GPRS and 802.11b capability that executes all physical layer processing completely in software. We describe the WCDMA communications protocols with a focus on latency reduction and unique implementation techniques. We also describe the underlying technology that enables software execution. Our solution is programmed in C and executed on a multithreaded processor in real time.     

Obsessive-compulsive disorder limited to pregnancy

Two patients with fulminant amebic colitis with colon perforation and concomitant liver abscess were collected over the last 5 years. One patient underwent emergency laparotomy to treat amebic cecal perforation. Diverted ileostomy saved his life. The ileostomy was successfully reversed 6 months later. The other patient underwent 4 laparotomies with more invasive procedures in less than 1 month due to sequential complications of amebiasis. Colon resection with enterostomy miraculously allowed him to survive. In comparison with the latter, who underwent more aggressive surgery and experienced more catastrophic complications, the former with conservative surgery had a smoother clinical result. Thus, conservative operation for colon perforation due to amebiasis is recommended. Besides, thanks to the alertness of doctors, the favorable age of the patients, the advent of new antiamebic and antimicrobial agents, excellent hyperalimentation, the great improvement in medical facilities and postoperative care, the two critical patients eventually survived after several operations, and had a better outcome as compared with the high mortality rate of 87.5% in our hospital 2 decades earlier.

Manipulating Kondo temperature via single molecule switching

Two conformations of isolated single TBrPP-Co molecules on a Cu(111) surface are switched by applying +2.2 V voltage pulses from a scanning tunneling microscope tip at 4.6 K. The TBrPP-Co has a spin-active cobalt atom caged at its center, and the interaction between the spin of this cobalt atom and free electrons from the Cu(111) substrate can cause a Kondo resonance. Tunneling spectroscopy data reveal that switching from the saddle to a planar molecular conformation enhances spin-electron coupling, which increases the associated Kondo temperature from 130 to 170 K. This result demonstrates that the Kondo temperature can be manipulated just by changing molecular conformation without altering chemical composition of the molecule.     

Impact strength elastomer composites based on polystyrene components separated from waste electrical and electronic equipment

The reuse of plastic components of waste electrical and electronic equipment (WEEE) is an important concern both for environmental issues and to preserve the material resources, with minimum energy consumption. Considering that polystyrene fraction was reported as approximate 80% of the total amount of WEEE plastic, this article aims to evaluate the recycling of this fraction, without separation by components, by melt compounding with styrene-butadiene block-copolymer (SBS) and hydrogenated and maleinized SBS, the blend of the two elastomers acting both as an impact modifier and compatibilizer. The composites are characterized by mechanical analysis, impact tests, dynamic mechanical analysis, differential scanning calorimetry, thermogravimetric analysis, scanning electron microscopy, energy dispersive X-ray analysis, and X-ray diffraction. The recycling conditions of the polystyrene fraction as composites without eliminating the WEEE additives for improved UV and flame resistance, with physical mechanical properties comparable to those of high-impact polystyrene resulted from the study. © 2019 Wiley Periodicals, Inc.
