Two distinct clinical patterns, encephalitic (furious) and paralytic (dumb), have been recognized in human rabies. It has been postulated that different rabies virus variants associated with particular vectors may be responsible for these different clinical manifestations. Analysis of the glycoprotein (G), nucleoprotein (N), and phosphoprotein (P) genes of rabies viruses from 2 human cases of encephalitic rabies and from 2 human cases of paralytic rabies demonstrated only minor nucleotide differences. Deduced amino-acid patterns of the N protein were identical in both human and canine samples that came from the same geographic location, regardless of the clinical form. All differences in amino-acid patterns of the G protein were found outside the ectodomain, in either the signal peptide or the transmembrane and endodomains. None of the amino-acid differences of the P protein was within the interactive site with dynein. These findings support the concept that clinical manifestations of rabies are not explained solely by the associated rabies virus variant. 相似文献

Surveillance for Nipah virus (NV) was conducted in Thailand's bat population. Immunoglobulin G antibodies to NV were detected with enzyme immunoassay in 82 of 1,304 bats. NV RNA was found in bat saliva and urine. These data suggest the persistence of NV infection in Thai bats. 相似文献

We have the knowledge and tools to eliminate the threat of canine rabies but this disease, nevertheless, remains a public health threat in many parts of the world. Lack of motivation by governments, cultural issues and inadequate funding remain barriers. This is amazing since the number of human rabies deaths worldwide is greater than that from polio, meningococcal meningitis, Japanese encephalitis, yellow fever, SARS, bird flue and other scourges that attract more attention. Safe and effective vaccines are now widely available. Reduced dose effective and less expensive post-exposure vaccination regimens have helped eliminate nerve tissue vaccines in Thailand, Philippines and Sri Lanka. India and Pakistan, the major users of dangerous nerve tissue derived Semple type vaccine, are now considering following suite. Immediate wound care and prompt use of a potent vaccine will save a majority of infected persons. Rabies immunoglobulin, injected into and around bite wounds, provides added safety for the severely exposed. The high cost of rabies immunoglobulin and tissue culture vaccines are remaining barriers, but new manufacturers and the use of intradermal vaccination schedules can reduce costs. Ultimately, it is the need to control rabies in dogs that must occupy most of our attention. The tools are available, but attitudes must change before they can be applied. There have been many new developments since publication of the last WHO rabies expert committee report in 1992 (new version in print)] and we will address those that have practical applicability. 相似文献

Background
In Thailand, rabies remains a neglected disease with authorities continuing to rely on human death statistics while ignoring the financial burden resulting from an enormous increase in post-exposure prophylaxis. Past attempts to conduct a mass dog vaccination and sterilization program have been limited to Bangkok city and have not been successful. Dog vaccination and population management have been suggested as priorities in attempts at disease control in canine rabies-endemic countries. Budget limitations and the complexity of social, cultural and religious variables have complicated progress in the developing world. In Bangkok, Thailand, an intensive canine vaccination and sterilization programme has been in place since November 2002. Our objective was to determine if the rabies virus could be mapped according to its genetic variations and geographical location on the small localized scale of Bangkok and its surrounding provinces. Phylogenetic characterization of 69 samples from Bangkok and five neighbouring and two remote provinces, by limited sequence analysis of the rabies virus nucleoprotein gene, distinguished six different clades. Rabies viruses of four clades were intermixed in Bangkok and in the surrounding highly populated regions whereas the other two clades were confined to rural and less populated provinces. Such a complex pattern of gene flow, particularly in Bangkok, may affect the outcome of canine control programmes. 相似文献

The aim of this study was to investigate the effect of acidic agents on surface roughness and characteristics of four restorative materials. Fifty-two discs were created from each restorative material: metal-reinforced glass ionomer cement (Ketac-S), resin-modified glass ionomer cement (Fuji II LC), resin composite (Filtek Z250), and amalgam (Valiant-PhD); each disc was 12 mm in diameter and 2.5 mm thick. The specimens were divided into four subgroups (n=13) and immersed for 168 hours in four storage media: deionized water (control); citrate buffer solution; green mango juice; and pineapple juice. Surface roughness measurements were performed with a profilometer, both before and after storage media immersion. Surface characteristics were examined using scanning electron microscopy (SEM). Statistical significance among each group was analyzed using two-way repeated ANOVA and Tukey's tests. Ketac-S demonstrated the highest roughness changes after immersion in acidic agents (p<0.05), followed by Fuji II LC. Valiant-PhD and Filtek Z250 illustrated some minor changes over 168 hours. The mango juice produced the greatest degradation effect of all materials tested (p<0.05). SEM photographs demonstrated gradual surface changes of all materials tested after immersions. Of the materials evaluated, amalgam and resin composite may be the most suitable for restorations for patients with tooth surface loss. 相似文献

STATEMENT OF PROBLEM: A 10-minute treatment with acidulated phosphate fluoride (APF) gel has been used as an alternative in ceramic surface etching before repairing with composite resin. However, the optimal etching time for APF gel is still unknown. PURPOSE: The purpose of this study was to determine the in vitro shear bond strengths of composite resin on high-leucite ceramics after APF gel treatment over different time periods. MATERIAL AND METHODS: One hundred and twenty high-leucite ceramic (Empress 1) specimens (12 mm in diameter and 1.5 mm thick) were prepared and divided into 12 groups (n=10). Ten experimental groups were surface treated with 1.23% APF gel, each group receiving 1 to 10 minutes of etching time in 1 minute increments. One group was treated with 9.6% hydrofluoric acid for 4 minutes and the final group received no treatment and served as a control. The surface condition of the treated specimens was analyzed under a scanning electron microscope (SEM). All specimens received a silane application and were bonded to a composite resin Filtek (Z250) cylinder with an adhesive system (Scotchbond Multi-Purpose Plus adhesive) and then stored in 100% humidity at 37 degrees C for 24 hours before shear bond strength testing in a universal testing machine. Mean bond strengths (MPa) were analyzed with 1-way ANOVA and the Tukey HSD test (alpha=.05). RESULTS: Hydrofluoric acid etching produced the highest mean shear bond strength (SD) between composite resin and the ceramic (17.64 (1.48) MPa). Overall, APF gel etching produced lower bond strengths. No significant difference in mean bond strength (SD) was observed between etching with hydrofluoric acid and etching with APF gel for 7 to 10 minutes (15.21 (1.93) to 17.33 (1.43)). The lowest mean shear bond strengths (SD) were recorded in the untreated group (7.61 (1.03) MPa) (P<.05). CONCLUSIONS: Within the limitations of this study, shear bond strength values between composite resin and high-leucite ceramics after etching with 1.23% APF gel for 7 to 10 minutes were not significantly different than that after etching with 9.6% hydrofluoric acid for 4 minutes. 相似文献
