Hydrogel microspheres of chitosan (CS) and Pluronic F127 (PF-127) were prepared by the emulsion-crosslinking method employing glutaraldehyde (GA) as a crosslinker. 5-Fluorouracil (5-FU), an anticancer drug with good water solubility, was encapsulated into hydrogel microspheres. Various formulations were prepared by varying the ratio of CS and PF-127, % drug loading and amount of GA. Microspheres were characterized by Fourier transform infrared (FTIR) spectroscopy to confirm the absence of chemical interactions between drug, polymer and the crosslinking agent. Scanning electron microscopy (SEM) was performed to study the surface morphology of the microspheres. SEM showed that microspheres have smooth shiny surfaces. Particle size, as measured by laser light scattering technique, gave an average size ranging from 110 to 382 microm. Differential scanning calorimetry (DSC) and X-ray diffraction (X-RD) studies were performed to understand the crystalline nature of the drug after encapsulation into hydrogel microspheres. Encapsulation of the drug up to 86% achieved was measured by UV spectroscopy. Equilibrium swelling experiments were performed in distilled water. Diffusion coefficients (D) of water through microspheres were estimated by an empirical equation. In vitro release studies indicated the dependence of release rate on the extent of crosslinking, drug loading and the amount of PF-127 used to produce the microspheres; slow release was extended up to 24 h. The release data were also fitted to an empirical equation to compute the diffusional exponent (n), which indicated that the release mechanism followed the non-Fickian trend.  相似文献

Self-inflicted eye injuries are rare but a devastating consequence of a serious mental disorder. Bilateral self-enucleation also known as oedipism has been documented in ancient texts and myths. Various biologic, psychologic, and social theories have been put forward to explain this rare phenomenon. Background

Hypertensive disorders are the second highest direct obstetric cause of maternal death after haemorrhage, accounting for 14% of maternal deaths globally. Pregnancy hypertension contributes to maternal deaths, particularly in low- and middle-income countries, due to a scarcity of doctors providing evidence-based emergency obstetric care. Task-sharing some obstetric responsibilities may help to reduce the mortality rates. This study was conducted to assess acceptability by the community and other healthcare providers, for task-sharing by community health workers (CHW) in the identification and initial care in hypertensive disorders in pregnancy.

Methods

This study was conducted in two districts of Karnataka state in south India. A total of 14 focus group discussions were convened with various community representatives: women of reproductive age (N = 6), male decision-makers (N = 2), female decision-makers (N = 3), and community leaders (N = 3). One-to-one interviews were held with medical officers (N = 2), private healthcare OBGYN specialists (N = 2), senior health administrators (N = 2), Taluka (county) health officers (N = 2), and obstetricians (N = 4). All data collection was facilitated by local researchers familiar with the setting and language. Data were subsequently transcribed, translated and analysed thematically using NVivo 10 software.

Results

There was strong community support for home visits by CHW to measure the blood pressure of pregnant women; however, respondents were concerned about their knowledge, training and effectiveness. The treatment with oral antihypertensive agents and magnesium sulphate in emergencies was accepted by community representatives but medical practitioners and health administrators had reservations, and insisted on emergency transport to a higher facility. The most important barriers for task-sharing were concerns regarding insufficient training, limited availability of medications, the questionable validity of blood pressure devices, and the ability of CHW to correctly diagnose and intervene in cases of hypertensive disorders of pregnancy.

Conclusion

Task-sharing to community-based health workers has potential to facilitate early diagnosis of the hypertensive disorders of pregnancy and assist in the provision of emergency care. We identified some facilitators and barriers for successful task-sharing of emergency obstetric care aimed at reducing mortality and morbidity due to hypertensive disorders of pregnancy.

Amphiphilic block copolymers composed of prepoly(sebacic anhydride) and Pluronic-F68/F127 have been synthesized in different molar compositions via melt-polycondensation reaction. Poly(sebacic anhydride-co-PLF68/PLF127) thus formed was characterized by Fourier transform infrared spectroscopy (FTIR), gel permeation chromatography (GPC) and nuclear magnetic resonance spectroscopic (NMR) techniques. The amphiphilic block copolymers were used to prepare microspheres and to encapsulate nifedipine (NFD) by the solvent evaporation technique. Differential scanning calorimetry (DSC) was used to confirm the incorporation of Pluronic into polyanhydrides, while X-ray diffraction (XRD) was performed on the drug-loaded microspheres to investigate the crystalline nature of the drug after encapsulation. Scanning electron micrograph (SEM) pictures indicated spherical nature of the microspheres. Microspheres obtained were in the size range of 10-50microm as measured by the laser particle size analyzer. In vitro release studies of NFD from poly(sebacic anhydride-co-Pluronic-F68/F127) microspheres performed in pH 7.4 phosphate buffer indicated sustained release rates of NFD at higher amounts of Pluronic in polyanhydride copolymers; there was no significant difference obtained between the release patterns of NFD from Pluronic-F68 and Pluronic-F127 copolyanhydride microspheres when same amount of Pluronics were used.  相似文献

Facial artery plays a key role in blood supply of the face. Facial artery one of the anterior branches of the external carotid artery originates within the carotid triangle. Variation in the origin of the facial artery was observed on two sides out of 60 sides in 30 cadavers. Facial artery was observed originating at the level just below the maxillary artery within the parotid gland. Facial artery may originate as linguo-facial trunk or thyro-linguo-facial trunk. Other developmental variations have also been described in the literature like, agenesis of facial artery, enlarged facial artery, hypoplastic facial artery. Variability in situation may complicate the surgery. Therefore maxillofacial surgeon, plastic surgeons and head & neck surgeon should be aware of such variation.  相似文献

Abstract

Objective: Immunomodulation by helminth proteins has potential therapeutic implications in inflammatory bowel disease. In the present study, we have explored the therapeutic effect of a RAL family protein of filarial parasite Wuchereria bancrofti i.e., rWbL2 protein against DSS induced colitis in a mouse model.

Materials and methods: Anti-inflammatory activity of rWbL2 on mice peritoneal exudate cells was analyzed under in vitro condition. The colitis mice were treated intraperitoneally (i.p.) with rWbL2 in increasing doses (10?µg, 25?µg, and 50?µg) on days 4, 5, and 6. Disease severity was assessed by disease activity index (DAI), macroscopic and histopathological scores, and enzyme myeloperoxidase activity (MPO) in the colon. The response of the cultured splenocytes from treated mice to Con-A stimulation, in terms of ELISA-based assessment of the protein followed by the assessment of mRNA expression of cytokines, was measured by real-time PCR analysis.

Result: rWbL2 protein showed anti-inflammatory activity in vitro. Treatment with rWbL2 (at 25?µg/dose) effectively attenuated disease severity by reducing weight loss, DAI, mucosal edema, colon damage, and MPO activity. This therapeutic effect was found to be associated with increased release of anti-inflammatory cytokine IL-10 and decreased release of pro-inflammatory cytokine IFN-γ and TNF-α by the splenocytes of treated mice followed by stimulation with Con-A.

Conclusions: These results provide evidence of the strong immunomodulatory potential of rWbL2 protein implicating its therapeutic application against ulcerative colitis.  相似文献

Extracted teeth can be bonded directly to adjacent teeth utilizing a prefabricated composite resin framework reinforced with polyethylene fiber as a noninvasive long-term provisional tooth replacement. This immediate provisional restoration allow for exact repositioning of the coronal part of the extracted tooth in its original intraoral three-dimensional position and thus relieves the apprehension of the patient caused by the sudden loss of an anterior tooth.  相似文献
