Pragmatism, Adaptation, and Total Quality Management: Philosophy and Science in the Service of Managing Continuous Improvement

Today’s engineering managers who use total quality management (TQM) face the challenge of moving it from adoption to seamless incorporation into their organizations. Both the application and the theory of TQM can be advanced through an examination of the philosophy of pragmatism and the sciences of adaptation. Pragmatism provides a philosophy that establishes a theory for incorporating TQM into engineers’ management practice; it derives its epistemology from actual experience. In essence, pragmatism suggests that practical experience, such as that gained from the continuous improvement of and experimentation with TQM, provides the basis for successful organizational performance. The sciences of adaptation identify the characteristics of organizations that promote successful adaptation. They suggest that incremental adjustments to ongoing change are the keys to effective and successful growth. This paper explores both philosophy and science to identify actions and attitudes that can enable the success of TQM in an organization.

Accidents,often the result of an ‘uncontrolled business process’—a study in the (Dutch) chemical industry

Often companies in the (petro‐) chemical industry claim that all possible countermeasures against potential accidents have been taken and therefore accidents are unforeseeable. In this paper we question this statement by analysing the pre‐warning signals (precursors) preceding a number of industrial accidents. 17 accidents that occurred in the (petro‐) chemical industry have been investigated by exploring FACTS, an accident database containing information about industrial accidents worldwide. This paper will demonstrate that the existence of precursor information could have been used to foresee and even prevent these accidents if a proper control action had been initiated. The accidents are analysed further, according to a control model, which was adapted from that of C. Argyris. It demonstrates the ineffectiveness of several elements of business process control loops and that the so‐called ‘double‐loop learning’ cycle is more important than the ‘single‐loop learning’ cycle if one considers safety improvement. Two-degree-of-freedom output feedback controllers for discrete-time nonlinear systems

A discrete-time, model-based output feedback control structure for nonlinear processes is developed in the present work. The structure makes use of a closed-loop observer, while at the same time it guarantees that the overall feedback controller possesses integral action. An algebraic transformation is applied on the observer states to insure that the input/output gain of the observer matches the model upon which the static state feedback control law is based. The resulting control algorithm is a two-degree-of-freedom control law, in the sense that the output and the set point are processed in different ways. The control structure is shown not only to have the same properties as the standard model-state feedback structure, but also that it emerges from a model algorithmic control framework. Finally, a simulation example using an exothermic CSTR operating at an open-loop unstable steady state is used to evaluate the closed-loop performance of the proposed method.
