STUDY OBJECTIVE: To determine the effect of dietary supplementation with n-3 polyunsaturated fatty acids (n-3 PUFA) on disease variables in patients with rheumatoid arthritis. DESIGN: Multicenter, randomized, placebo controlled, double blind. SETTING: Three Danish hospital Departments of Rheumatology. PATIENTS: Fifty-one patients with active rheumatoid arthritis. INTERVENTION: Random allocation to 12 weeks of treatment with either six n-3 PUFA capsules (3.6 g) or six capsules with fat composition as the average Danish diet. MAIN RESULTS: Significant improvement of morning stiffness and joint tenderness. No significant effect on the four other assessed clinical parameters. No serious side effects. CONCLUSIONS: Dietary supplementation with n-3 PUFA in patients with rheumatoid arthritis improved two out of six patient reported disease parameters. Further studies are needed to clarify the more precise role of n-3 PUFA in the treatment of rheumatoid arthritis. 相似文献

Heart rate variability was determined from three consecutive Holter recordings performed on days 1, 7, and 28 in 17 normal subjects, in 13 patients with angiographically normal coronary arteries, and in 9 patients with remote myocardial infarctions. Group data of several time and frequency domain measures of heart rate variability were highly reproducible (correlation coefficients 0.629–0.894). However, some individuals exhibited considerably larger day-to-day variations in heart rate variability. Single heart rate indices differed by up to 50% between two Holter recordings. Such potential differences must be considered when repeated heart rate variability determinations are used to assess changes in neurocardiac reflex regulation or effects of therapeutic interventions. 相似文献

Mycobacterial infections occurred in 11 of 633 (1.7 per cent)recipients of successful renal transplants. Cancer stem cells (CSCs) possess the capacity to self-renew and to generate heterogeneous lineages of cancer cells that comprise tumors. A substantial body of evidence supports a model in which CSCs play a major role in the initiation, maintenance, and clinical outcome of cancers. In contrast, analysis of the role of CSCs in metastasis has been mainly conceptual and speculative. Cancer cells show a broad spectrum of bioenergetic states, with some cells using aerobic glycolysis while others rely on oxidative
phosphorylation as their main source of energy. In addition, there is mounting evidence that metabolic coupling occurs in
aggressive tumors, between epithelial cancer cells and the stromal compartment, and between well-oxygenated and hypoxic compartments.
We recently showed that oxidative stress in the tumor stroma, due to aerobic glycolysis and mitochondrial dysfunction, is
important for cancer cell mutagenesis and tumor progression. More specifically , increased autophagy/mitophagy in the tumor
stroma drives a form of parasitic epithelial-stromal metabolic coupling. These findings explain why it is effective to treat
tumors with either inducers or inhibitors of autophagy, as both would disrupt this energetic coupling. We also discuss evidence
that glutamine addiction in cancer cells produces ammonia via oxidative mitochondrial metabolism. Ammonia production in cancer
cells, in turn, could then help maintain autophagy in the tumor stromal compartment. In this vicious cycle, the initial glutamine
provided to cancer cells would be produced by autophagy in the tumor stroma. Thus, we believe that parasitic epithelial-stromal
metabolic coupling has important implications for cancer diagnosis and therapy, for example, in designing novel metabolic
imaging techniques and establishing new targeted therapies. In direct support of this notion, we identified a loss of stromal
caveolin-1 as a marker of oxidative stress, hypoxia, and autophagy in the tumor microenvironment, explaining its powerful
predictive value. Loss of stromal caveolin-1 in breast cancers is associated with early tumor recurrence, metastasis, and

One hallmark of neoplasia is the deregulation of cell cycle control mechanisms, which is secondary to altered protein phosphorylation. Dual specificity protein phosphatases uniquely dephosphorylate both phosphoserines/threonines and phosphotyrosines on the same protein substrate. As a class they regulate intracellular signaling through the mitogen activated and stress activated kinases and govern cellular movement through G1/S and G2/M cell cycle checkpoints by affecting the activity of cyclin-dependent kinases. In particular, the Cdc25 phosphatases, which dephosphorylate cyclin-dependent kinases, are overexpressed in many human tumors and this increased expression is associated with a poor prognosis. In addition to expression levels, the intracellular activity of Cdc25 phosphatases is determined by their subcellular distribution and physical proximity to substrates. Small molecules that either inhibit the catalytic activity or alter the subcellular distribution of these dual specificity protein phosphatases could provide effective tools to interrogate the role of phosphorylation pathways and may afford new approaches to the management of cancer. 相似文献

The main causes for mid-trimester loss are known. There is likely to be
overlap with those of first trimester loss, but the proportions may be
different. We wished to perform an aetiological survey in a large
population of patients with a history of recurrent miscarriage, for
possible explanations for their second trimester miscarriages. Database
analysis of 636 patients attending a UK University Teaching Hospital
dedicated miscarriage clinic between 1991 and 1996 revealed a 25%
prevalence (n = 158) for second trimester miscarriage. Results from an
investigative screening protocol were positive in 50% of cases: 33% (n =
52) tested positive for antiphospholipid syndrome (APS); 8% (n = 13)
fulfilled strict criteria for cervical incompetence; there was a 4%
prevalence of uterine anomaly; 3% for infection (n = 5) and 2% of patients
(n = 3) proved to be hypothyroid. Importantly, dual pathology was found in
5% of patients with a history of second trimester miscarriage. As
idiopathic mid-trimester loss is a diagnosis by exclusion, a high index of
suspicion is required, as are modern diagnostic techniques.
