Envenomation by the Irukandji jellyfish (Carukia barnesi) can result in an array of systemic symptoms known as Irukandji syndrome. In 1996, 62 people presented to Cairns emergency departments with Irukandji envenomation: 57 developed systemic symptoms, and 38 required parenteral narcotics. All patients were discharged home within 24.5 hours, except for two who required high-dependence care for pulmonary oedema. Patients were more likely to be stung on hotter days, with lower-than-average rainfall in the past seven days, and with winds from the north, but less-than-average wind speed. We offer a protocol for treating patients with Irukandji envenomation.  相似文献

This study assessed the effects of changes in skin temperature on multifrequency bioimpedance analysis (MF-BIA) and on the prediction of body water compartments. Skin temperature (baseline 29.3 +/- 2.1 degrees C) of six healthy adults was raised over 50 min to 35.8 +/- 0.6 degrees C, followed by cooling for 20 min to 26.9 +/- 1.3 degrees C, by using an external heating and cooling blanket. MF-BIA was measured at both distal (conventional) and proximal electrode placements. Both distal and proximal impedance varied inversely with a change in skin temperature across all frequencies (5-500 kHz). The change in proximal impedance per degree centigrade change in skin surface temperature was approximately 60% of distal impedance. The change in measured impedance at 50 kHz erroneously increased predicted total body water (TBW) by 2.6 +/- 0.9 liters (P < 0.001) and underpredicted fat mass by 3.3 +/- 1.3 kg (P < 0.0001). Computer modeling of the MF-BIA data indicated changes in predicted water compartments with temperature modifications; however, the ratio of extracellular water (ECW) to TBW did not significantly change (P < 0.4). This change in impedance was not due to a change in the movement of water of the ECW compartment and thus probably represents a change in cutaneous impedance of the skin. Controlled ambient and skin temperatures should be included in the standardization of BIA measurements. The error in predicted TBW is < 1% within an ambient temperature range of 22.3 to 27.7 degrees C (72.1-81.9 degrees F).  相似文献

The quasi- or pseudo-floating gate (QFG) technique addresses a key issue with the floating-gate MOS transistor technique, by using ultra-high resistances to provide dc paths to otherwise floating nodes. Several ways have been suggested to implement the quasi-infinite resistors (QIRs). In this paper, basic QIR structures are analyzed and compared, and three sources of error, dc offset, signal distortion, and signal-dependent offset, are defined. Then, through simulations and experiments, the suitability of several QIR implementations for use in various applications is compared. A particular QIR implementation is found to minimize dc offset, but requires voltage swings to be limited to less than a diode turn-on voltage. Some application circuits using quasi-floating gate are presented, including a QFG translinear geometric-mean circuit and QFG low-voltage fully-differential amplifiers with QFG common-mode feedback using several QIR structures. Measurements on current-mode QFG circuits exhibit large offsets and very long turn-on transients, which could limit practical application of this technique. Inchang Seo received B.S. and M.S. degrees in physics from Hanyang University, Seoul, Korea, in 1988 and 1990. He also received M.S. and Ph.D degrees in electrical and computer engineering from the University of Florida, Gainesville, Florida, in 2000 and 2004, respectively. During 1990–1997, he worked as a researcher at the Agency for Defence Development (ADD), Chinhae, Korea. His responsibility was design, development, and evaluation of underwater acoustic transducers and sonar systems which were based on piezoelectric, magnetostrictive, and fiber-optic transducers. His main research interests involve low-voltage, low-power, high-precision analog and mixed-signal integrated circuit design including low-voltage wide-band DS data converters, precise bias circuit blocks, low-power charge transfer amplifiers and filters, floating-gate CMOS analog circuits, and quasi-floating gate analog applications. Robert M. Fox received the B. S. degree in Physics from the University of Notre Dame in 1972, and M. S. and Ph. D. degrees in Electrical Engineering from Auburn University in 1981 and 1986, respectively. Since 1986 he has been on the Electrical and Computer Engineering faculty at the University of Florida, where he is an Associate Professor. Dr. Fox's research emphasizes circuit design and modeling for advanced IC technologies. He has worked on a variety of topics, including analog circuit design, cryogenic electronics, circuit design with SOI, radiation response of semiconductors, noise modeling, and modeling of transistor self-heating. Currently his research interests center on design-oriented analysis of analog integrated circuits, including low-voltage circuit techniques, design of log-domain circuits, analog test strategies and transistor modeling. Dr. Fox is a member of the Analog Signal Processing Technical Committee of the Circuits and System Society, having served as Committee Chairman and ISCAS Track Chair. He is a member of the Analog/Mixed-Signal Technical Committee for the IEEE Bipolar/BiCMOS Circuits and Technology Meeting (BCTM).  相似文献

The relationship between adolescent egocentrism and post-formal thinking was examined in 163 college undergraduates. Participants were administered the Imaginary Audience Scale and the Social Paradigm Belief Inventory. Results showed that females had higher levels of adolescent egocentrism than did males. A weak negative correlation between egocentrism and cognitive reasoning was found for females during late adolescence. The findings are consistent with Peterson and Roscoe's (1991) study of egocentrism in older adolescent females.  相似文献

In a clinical trial of paclitaxel (Taxol) and carboplatin in combination, the severity of thrombocytopenia was less than would be expected with an equivalent dose of carboplatin alone. To determine whether a pharmacokinetic interaction was responsible for this observation, the effect of pretreatment with Taxol on the pharmacokinetics of carboplatin was examined in 11 patients. Each patient was randomized to one of two treatment groups that determined the order of drug treatments. The treatments were carboplatin as a 30-min infusion alone or immediately following 175 mg/m2 Taxol administered as a 3-h i.v. infusion. The treatments were separated by 1 week. The carboplatin dose was chosen to produce a target area under the concentration-time curve (AUC) of 3.75 mg-min/ml according to a previously published formula (A. H. Calvert et al., J. Clin Oncol., 7: 1748-1756, 1989). The mean administered dose of carboplatin was 338 mg. Serial blood samples were collected over 24 h and analyzed for total and free platinum, and, in some patients, Taxol. The pharmacokinetics of carboplatin (i.e., total clearance and volume of distribution at steady state), was not significantly affected by pretreatment with Taxol. Total clearances of carboplatin were 67.2 +/- 28.8 ml/min and 64.6 +/- 27.9 ml/min in the absence and presence of Taxol, respectively (P = 0.56). The AUC of free carboplatin (3.45 mg-min/ml) obtained in the absence of Taxol was not significantly different from that measured in the presence of Taxol (3.27 mg-min/ml). The AUC of carboplatin in both the absence and presence of Taxol agreed with the projected target AUC of 3.75 mg-min/ml. In conclusion, the application of an individualized dosing strategy is valid for the calculation of the carboplatin dose in this combination. The pharmacokinetics of carboplatin is not altered by pretreatment with Taxol at a standard dose, and a pharmacokinetic interaction is not responsible for the altered toxicity of the combination.  相似文献
