BACKGROUND: Medical problems may affect the ability to drive motor vehicles, and programs that control the issuing of driver licenses to individuals with medical conditions exist in most states. The main activity of these programs is the imposition of restrictions upon the driving privileges of individuals with medical conditions that are deemed to pose some risk to public safety. However, little is known about the effectiveness of these licensing programs. OBJECTIVE: The objective of this study was to compare the rates of adverse driving events (crash, at-fault crash and citations) experienced by drivers licensed with medical conditions to those of age-, sex- and location-matched controls. Separate comparisons were made for drivers reporting medical conditions licensed with full driving privileges, and those with restricted driving privileges (e.g. speed, area and time of day). DESIGN: Retrospective case-control. METHODS: The study population was all drivers licensed in the state of Utah who reported a medical condition on their driver license application, over the 5-year period 1992-1996. Drivers enter the program by self-reporting their medical problems. Control drivers were chosen from the entire population of drivers licensed in Utah for the same period. Information on driver license status, participation in the medical conditions program, citations, involvement in crashes, and death certificate data was obtained from the relevant state agencies. Probabilistic linkage methodology was used to link the records in these disparate databases for eventual analysis. Rates of citation, crashes and at-fault crashes, expressed as events per 10000 license days, were calculated separately for program drivers and their corresponding control groups for each medical condition category and restriction status. These data were used to determine an estimate of relative risk (RR) and 95% confidence intervals. Results: As a group, medical conditions drivers had modestly elevated rates of adverse driving events compared with control drivers (RR 1.09-1.74). Rates in the largest medical category, 'cardiovascular conditions', were not higher than controls. Rates were higher than control for some conditions, such as 'alcohol' and 'learning and memory', for some adverse events (RR 2.2 -5.75). Drivers with more than one medical condition appeared comparable to the general group of medical conditions program drivers. CONCLUSIONS: Drivers in Utah medical conditions program had modestly elevated rates of adverse driving events compared to matched controls. Possible underreporting of medical conditions and accurate assessment of exposure rates are potential weaknesses in the program.  相似文献

The currently approved method for the analysis of total cyanide (TCN) in wastewaters has remained virtually unchanged in the 25 years since its initial use; this despite its subjection to a number of interferences, many of which provide a positive bias in cyanide measurements, including the formation of TCN during sample processing and some of which remain undocumented to this day. In particular, many municipal wastewater treatment plant chlorinated effluents throughout North America have often been cited for permit violations on the levels of total cyanide in their effluents measured using this methodology. A recently developed procedure for the analysis of TCN in various matrixes that utilizes segmented flow injection for sample transport and reaction, on-line acidic UV digestion for conversion of complexed cyanide to HCN, and amperometric detection achieved within 4 min of sample injection is demonstrated on chlorinated effluents discharged from municipal wastewater treatment plants. Through a systematic evaluation of the chemistry of the processes involved in this method and an understanding of the complexity of the wastewater matrix, an application was developed that showed consistent reproducibility in measuring TCN in a variety of effluents. The method was employed initially on synthetic solutions containing controlled levels of constituents identified in the literature as interfering with the measurement of TCN through the traditional distillation/colorimetric approach. In its application to the analysis of TCN in chlorinated wastewaters, an approach was evolved that ensured that dechlorination carried out during sample processing did not bias the results of analysis.  相似文献

Using automated reasoning techniques, we tackle the niche activity of proving that a program is free from run-time exceptions. Such a property is particularly valuable in high integrity software, for example, safety- or security-critical applications. The context for our work is the SPARK Approach for the development of high integrity software. The SPARK Approach provides a significant degree of automation in proving exception freedom. Where this automation fails, however, the programmer is burdened with the task of interactively constructing a proof and possibly also having to supply auxiliary program annotations. We minimize this burden by increasing the automation, through an integration of proof planning and a program analysis oracle. We advocate a ‘cooperative’ integration, where proof-failure analysis directly constrains the search for auxiliary program annotations. The approach has been successfully tested on industrial data.  相似文献

This paper presents technical aspects of a robot manipulator developed to facilitate learning by young children who are generally unable to grasp objects or speak. The severity of these physical disabilities also limits assessment of their cognitive and language skills and abilities. The CRS robot manipulator was adapted for use by children who have disabilities. Our emphasis is on the technical control aspects of the development of an interface and communication environment between the child and the robot arm. The system is designed so that each child has user control and control procedures that are individually adapted. Control interfaces include large push buttons, keyboards, laser pointer, and head-controlled switches. Preliminary results have shown that young children who have severe disabilities can use the robotic arm system to complete functional play-related tasks. Developed software allows the child to accomplish a series of multistep tasks by activating one or more single switches. Through a single switch press the child can replay a series of preprogrammed movements that have a development sequence. Children using this system engaged in three-step sequential activities and were highly responsive to the robotic tasks. This was in marked contrast to other interventions using toys and computer games.  相似文献

The authors conducted 2 experiments to assess how phonologically related lures are rejected in a false memory paradigm. Some phonological lures were emotional (i.e., taboo) words, and others were not. The authors manipulated the presence of taboo items on the study list and reduced the ability to use controlled rejection strategies by dividing attention and forcing a short response deadline. The results converge on the idea that participants reduce false alarms to emotional lures by setting more stringent recognition criteria for these items based on their expected memorability. Additionally, emotional lures are less familiar than nonemotional lures because emotional lures have affective and semantic features that mismatch studied nonemotional items. Retinal ganglion cell (RGC) axons in lizards (reptiles) were found to regenerate after optic nerve injury. To determine whether regeneration occurs because the visual pathway has growth-supporting glia cells or whether RGC axons regrow despite the presence of neurite growth-inhibitory components, the substrate properties of lizard optic nerve myelin and of oligodendrocytes were analyzed in vitro, using rat dorsal root ganglion (DRG) neurons. In addition, the response of lizard RGC axons upon contact with rat and reptilian oligodendrocytes or with myelin proteins from the mammalian central nervous system (CNS) was monitored. Lizard optic nerve myelin inhibited extension of rat DRG neurites, and lizard oligodendrocytes elicited DRG growth cone collapse. Both effects were partially reversed by antibody IN-1 against mammalian 35/250 kD neurite growth inhibitors, and IN-1 stained myelinated fiber tracts in the lizard CNS. However, lizard RGC growth cones grew freely across oligodendrocytes from the rat and the reptilian CNS. Mammalian CNS myelin proteins reconstituted into liposomes and added to elongating lizard RGC axons caused at most a transient collapse reaction. Growth cones always recovered within an hour and regrew. Thus, lizard CNS myelin and oligodendrocytes possess nonpermissive substrate properties for DRG neurons--like corresponding structures and cells in the mammalian CNS, including mammalian-like neurite growth inhibitors. Lizard RGC axons, however, appear to be far less sensitive to these inhibitory substrate components and therefore may be able to regenerate through the visual pathway despite the presence of myelin and oligodendrocytes that block growth of DRG neurites.  相似文献

The objective of this study was to determine if progesterone administered near the onset of estrus in dairy cows would block the preovulatory surge of LH and result in subsequent persistence of ovarian follicular structures. Following synchronization of estrus with prostaglandin F2 alpha, 20 multiparous, non-lactating Holstein cows were randomly assigned into three groups: 1 ml ethanol administered i.m. at 12-h intervals for 24 h (n = 6; group 1); 1 mg progesterone administered i.m. at 12-h intervals for 24 h (n = 7; group 2); 2.5 mg progesterone administered i.v. at the onset of standing estrus (n = 7; group 3). Ovarian structures were palpated per rectum on the day of estrus and twice weekly for 14 d. Blood was collected every 2 h from onset of standing estrus for 30 h, and concentrations of LH and progesterone were determined. Numbers of cows diagnosed with persistent follicles 10 d after estrus were 1 of 6 (group 1), 2 of 7 (group 2), and 5 of 7 (group 3). The preovulatory surge of LH did not occur during the sampling period (30 h) for 1 of 6, 7 of 7 and 5 of 7 cows, for groups 1, 2, and 3, respectively, and mean serum concentrations of LH were higher in group 1 than in groups 2 and 3. Serum concentration of progesterone (ng/ml) was higher in group 1 (1.9 +/- .4) than in groups 2 (.9 +/- .4) and 3 (.9 +/- .4) 10 d following estrus. Blocking the preovulatory surge of LH with exogenous progesterone resulted in persistence of ovarian follicles.  相似文献
