Root and canal morphology of Thai maxillary molars   总被引:3,自引:0,他引:3  
AIM: To investigate the root and canal morphology of 268 maxillary permanent molars collected from an indigenous Thai population. METHODOLOGY: The cleaned teeth were accessed, the pulp dissolved by sodium hypochlorite under ultrasonication, and the pulp system injected with Indian ink. The teeth were rendered clear by demineralization and immersion in methyl salicylate. The following observations were made: (i) number of roots and their morphology; (ii) number of root canals per root; (iii) root-canal configuration in each root using Vertucci's classification with additional modifications; and (iv) presence and location of lateral canals and intercanal communications. RESULTS: All the maxillary first and second molars had three separate roots. Only, half (51%) of the maxillary third molars had three separate roots; the other half had fused or conical roots. The majority of the distobuccal (98.1-100%) and palatal (100%) roots had type I canals. Over half of the mesiobuccal roots of first (65%) and second (55%) molars had two canals. The most common (44.2%) canal configuration in mesiobuccal roots of first molars was type IV (two canals, two foramina). A variety of canal types were found in the mesiobuccal roots of second molars. Maxillary third molars showed the greatest diversity of canal morphology. There was an increase in the prevalence of lateral canals towards the apical part of the roots and intercanal communications were present in 16% of each of first, second and third Thai maxillary molars. CONCLUSIONS: The mesiobuccal roots of Thai maxillary molars possessed a variety of canal system types. Over 50% of the first molars had a second mesiobuccal canal. The palatal and distobuccal canals mainly had type I canals. Only, a small proportion (7.3-13.3%) of the roots exhibited lateral canals which were the most common in the apical third  相似文献

目的 观察上颌第一磨牙的根管系统。方法 制备50个上颌第一磨牙透明牙标本,显露根管系统的空间构型,在体视显微镜下观察并记录根管系统的形态,结合应用加以统计。结果 ①上颌第一磨牙近中颊根单根管率为66%,其中Ⅰ型占38%,双根管率为34%,远中颊根Ⅰ型占74%,腭根Ⅰ型占94%。②根管侧支、根尖分歧、根尖分叉多见于近颊根,在近颊根的发生率为40%, 远颊根的发生率为24%,腭根的发生率为6%,发生部位多见于根尖1/3。结论 上颌第一磨牙近颊根的根管系统最为复杂,根管变异较远颊根多,腭根变异最少。  相似文献

This in vitro study investigated the presence of second (MB2) root canals and the number of apical foramina found in mesiobuccal roots of the permanent maxillary first molars; in addition, the study evaluated the effectiveness of magnification to detect MB2 canals. One hundred extracted maxillary first molars were collected from Jordanian patients. The number of roots (as well as their morphology) was investigated. The number of canal orifices in mesiobuccal root was assessed, with and without magnification, and findings were compared. The number of apical foramina and the distance between mesiobuccal and second canal orifices were investigated under magnification. Of the 97 specimens that were subject to additional investigation, all teeth had three separated roots, except for three specimens that had either mesiobuccal or distobuccal roots fused with the palatal root. The number of MB2 canals that were detected increased from 55 (56.7%) to 61 (62.9%) teeth when magnification was used. Within the limits of this in vitro study, it was concluded that the Jordanian population had a high percentage of MB2 canals in the mesial buccal roots. Moreover, the use of clinically used magnifying devices increased the number of MB2 canals detected.  相似文献

AimTo investigate the root and canal morphology of maxillary and mandibular permanent molar teeth in a Caucasian population by using Cone Beam Computed Tomography (CBCT).MethodsA total of 596 permanent molars were included. The number of roots, the number of canals per root and the root canal configurations according to the method of Vertucci were recorded.ResultsAlmost all of maxillary first molars (95.7%) had three separate roots; however, 40.3% of mesiobuccal roots had two canals (MB2). Of 157 maxillary second molars, 88.5% had three roots. Among the mesiobuccal roots, 15.1% had two canals. The majority of mandibular molars (100% of first molars, 89.4% of second molars) had two separate roots. Most distal roots had a simple type I configuration, whereas mesial roots had more complex canal systems, with more than one canal.ConclusionsCBCT is an efficient method of studying root canal systems.  相似文献

AIM: To investigate in vitro the incidence and position of the root canal isthmus in extracted mesiobuccal roots of maxillary and mesial roots of mandibular first molars. METHODOLOGY: Fifty maxillary and 50 mandibular molars were included in the study. The mesiobuccal roots of maxillary molars and the mesial roots of mandibular molars were sectioned from their crowns in the furcation region and embedded in clear resin. Transverse serial 1-mm-thick sections from the apical 6 mm were prepared. The apical side of each section was stained with India ink and observed through a light microscope. The sample images were saved to disk using a digital camera and the root canals in terms of the number present and the incidence and classification of isthmuses. RESULTS: In the mesiobuccal root of the maxillary first molars, 70% had one canal, whereas 29.5% had two canals. In the mesial root of mandibular molars, 41% had one canal, whereas 59% had two canals. In some sections, more than two canals were found close to the apical foramen. The isthmus incidence was greatest 3-5 mm from the apex. In teeth having two canals, a complete or partial isthmus was frequently observed in the sections between 3 and 4 mm from the apex. Of the isthmuses present, 22% were complete and 37% partial in mandibular molars and 17.3% were complete and 11.7% partial in maxillary molars. CONCLUSIONS: The incidence of isthmus in the mesiobuccal root of the maxillary first molars and in the mesial root of the mandibular first molars was high, particularly in sections 3-5 mm from the apex. Cleaning the isthmus is a major challenge during root canal treatment.  相似文献

AIM: To investigate the root and canal morphology of Burmese maxillary molars using a canal staining and tooth clearing technique. METHODOLOGY: Maxillary molars (239) were collected from indigenous Burmese patients and designated; first (90), second (77), third (72) molars. Following pulp tissue removal and canal system staining with Indian ink, the teeth were decalcified with 10% nitric acid, dehydrated and cleared with methyl salicylate. The following features were evaluated: (i) number of roots and morphology; (ii) number of canals per root: (iii) root canal configuration (Vertucci's classification); (iv) number of apical foramina per root; (v) number and location of lateral canals; and (vi) the presence of intercanal communications. RESULTS: All first and second molars had three separate roots, whilst third molars had one of five different root forms, of which three separate (25%) or fused (31%) roots were most prevalent. The majority of palatal (100%) and disto-buccal (96%) roots possessed one canal (type I). The prevalence of mesio-buccal roots with two canals decreased from first to third molars (68%, 49%, 39%, respectively); type II (two orifices, one apical foramen) and IV (two orifices, two apical foramina) configurations were the most common. Single/fused rooted third molars had a range of number and type of canals. Most roots in all molars had one apical foramen, those with four apical foramina were confined to third molars. Intercanal communications were most prevalent in mesio-buccal roots and two/three fused rooted third molars. Lateral canals were most prevalent in the apical third of the roots of all molars. CONCLUSIONS: The mesio-buccal roots of Burmese maxillary molars possessed a variety of canal system types. Over 50% of the first and second molars had a second mesio-buccal canal, of which over 20% had intercanal communications. The palatal and disto-buccal canals mainly had type I canals. Lateral canals were equally prevalent in all tooth types but were most common in the apical third.  相似文献

Aim To identify the number of roots and canal configurations in permanent first molars of the indigenous Kuwaiti sub‐population and compare them against a similar group of non‐Kuwaiti population in different age groups and gender, and to determine the frequency of a second canal in the mesiobuccal root of maxillary first molars and distal root of mandibular first molars that could be located and treated in routine practice without using magnification or special lighting conditions. Methodology A total of 220 permanent first molar teeth of patients scheduled for root canal treatment over a period of 1 year were included. Patients were identified and grouped according to their nationality as Kuwaiti and non‐Kuwaiti (which included Filipinos, Indonesian Indians, Srilankans, Egyptians, Saudi Arabians and Syrians). In both nationality groups, patients were successively distributed into four groups based on their age. The first age group included patients below 20 years, the second 21–30 years, the third 31–40 years and the fourth were over 40 years. The first three groups comprised of 15 male and 15 female patients whilst the fourth group had 10 patients of each gender. Teeth with open apices, resorptions and calcification were excluded from the study. The teeth included were both clinically and radiographically examined for number of roots, the canal configuration and the presence of the additional mesiobuccal and distal canals and recorded. The simplified classification of canal configurations proposed by Weine was utilized. Results The incidence of a second canal in the mesiobuccal (MB) root of maxillary first molars and the distal root of mandibular first molars was not influenced significantly by nationality and gender. All the 110 maxillary first molars treated had three roots; 58% of MB root had one canal and 42% had two canals. The majority of the mesial roots had Weine type II canal configuration when the second mesiobuccal canal was present. All distobuccal and palatal roots had a single or type I canal configuration. Of the 110 mandibular first molars that were treated, 96% had two roots and 4% had three roots. When present, the third root was located either buccal or lingual to the main distal root. Overall 51% of the single distal roots had one canal whilst 49% had two canals. The mesial root frequently had a Weine type II canal configuration as did the distal root when a second canal was present. Pearson’s correlation analysis of both molars revealed a significant (P < 0.05) negative correlation (r = ?0.274, r = ?0.144) between age and number of canals as well as the type of canal. Conclusions The incidence of a second canal in distal roots of permanent mandibular first molars was 49% in the Kuwaiti population and this was similar to other Asian ethnic populations. Adopting modified access and troughing procedures revealed a 42% frequency of MB2 canals in maxillary first molars. The incidence of a second canal in both mesiobuccal roots of maxillary molars and distal roots of mandibular molars decreased significantly with age; no differences were noticed amongst the nationalities and gender studied. The possibility of extra roots should be anticipated in mandibular molars.  相似文献

The purpose of this study was to examine extracted root canal treated maxillary molars cleared for: (i) the presence of a mesiopalatal (MP) canal in both first and second molars, (ii) the extension of MP canal from the pulp to the apical area, and (iii) the incidence of two foramina in the MB root. Seventy-two extracted maxillary molars (42 first and 30 second molars) were root canal treated by graduate students. The mesiobuccal (MB) roots of these samples were rendered transparent using a clearing technique. The results demonstrated that 52.3% of first and 40% of second molars had two canals obturated in the MB root. After clearing the same roots, the presence of MP canals rose to 80.9% and 66.6%, respectively. The MP canals were root canal treated as far as the foramen in 35.2% of first and 35% of second molars. However, after making them transparent, 91.1% and 90% showed the presence of this canal to the anatomical apex. The MB roots of the root canal treated first molars showed the presence of two foramina in 47% of cases but in 88.2% after clearing. The second molars showed 50% and 70%, respectively. The differences between root canal treated teeth before and after clearing were significantly different in almost all comparisons in both first and second molars. The only exception was when the presence of two foramina was compared before and after the clearing process in second molars (McNemar test P < 0.05).  相似文献

AIM: To analyse the occurrence of canal isthmuses in molars following root-end resection. METHODOLOGY: The material consisted of 56 mandibular and 32 maxillary first molars subjected to periradicular surgery. Based on radiographic, clinical, as well as intraoperative status, only roots with associated pathological lesions were treated. In total, 124 roots were resected (80 mandibular and 44 maxillary molar roots). The cut root faces were inspected with a rigid endoscope following apical root-end resection. The number of canals as well as the presence and type of canal isthmuses were recorded. RESULTS: In maxillary first molars, 76% of resected mesio-buccal roots had two canals and an isthmus, 10% had two canals but no isthmus, and 14% had a single canal. All disto-buccal and palatal roots had one canal. In mandibular first molars, 83% of mesial roots had two canals with an isthmus. In 11%, two canals but no isthmus were present, and 6% demonstrated a single canal. Sixty-four per cent of distal roots had a single canal and 36% had two canals with an isthmus. CONCLUSIONS: This clinical study during periradicular surgery and intraoperative endoscopic examination of first permanent molars found a high frequency of canal isthmuses at the resection level. Endoscopic inspection also demonstrated that none of the isthmuses were filled, emphasizing the difficulty of orthograde instrumentation and root filling of canal isthmuses.  相似文献

The aim of this study was to investigate whether the use of operating microscope in combination with ultrasonics increased the rate of second mesiobuccal (MB2) canal detection in permanent maxillary first molar teeth. A hundred extracted human maxillary first molars were assessed. After location of the main canals, the MB2 canal was sought in all teeth first without microscopy, then with the aid of the operating microscope and finally with the combined use of the operating microscope and ultrasonics. With these techniques, the MB2 canal was detected in 62%, 67% and 74% of the teeth, respectively. The combination of the operating microscope and ultrasonics detected significantly more MB2 canals than when no microscopy was utilized (P < 0.05). Sectioning of the roots disclosed the presence of the MB2 canal in 82% of the teeth. Twenty‐nine per cent of the teeth had a separate MB2 canal orifice and separate apical foramina. The results of this study suggested that the combined use of the operating microscope and ultrasonics increased the detection of MB2 canals in maxillary first permanent molars.  相似文献

AIM: To compare a clearing technique with conventional radiography in studying certain features of the root-canal system of maxillary right first and second molars. A secondary aim was to assess interexaminer agreement for these features using radiographs. METHODOLOGY: Eighty-three recently extracted permanent maxillary right first molars and 40 recently extracted maxillary right second molars from an Irish population were included. Standard periapical radiographs were taken from a buccolingual and mesiodistal direction. The specimens were then decoronated, demineralized in 10% hydrochloric acid for 8 days and then cleared using methyl salicylate. The cleared teeth were examined using a dissecting microscope (x20), and data relating to number of roots, canal type following Vertucci's classification, presence of lateral canals, presence of transverse anastomoses and position/number of apical foramina were collected. The radiographs were examined by two independent trained endodontists using an X-ray viewer and a magnifying lens (x2) in a dark room for the same features studied using the clearing technique. RESULTS: The Kappa values for the agreement between the radiographic examiners A and B and the clearing technique and between the two examiners for the number of roots were 0.60, 0.64 and 0.53; for the root-canal type, 0.37, 0.41 and 0.42; for the number of roots with lateral canals, 0.21, 0.18 and 0.14; and for the transverse anastomoses, 0.29 for radiographic Examiner A. Radiographic Examiner B did not feel capable of accurately recognizing transverse anastomoses from the radiographs. For the position/number of apical foramina, the Kappa values were 0.33 and 0.24, respectively. In general, the Kappa values were low to modest for all comparisons. CONCLUSIONS: It is concluded that the agreement between the two radiographic examiners and the agreement between either radiographic examiner and the clearing technique were poor to moderate, indicating the limited value of radiographs alone when studying certain aspects of the root-canal system.  相似文献

Introduction

The aim of this study was to analyze and characterize root canal morphology of maxillary molars of the Brazilian population using cone-beam computed tomographic (CBCT) imaging.

Methods

Patients referred for a CBCT radiographic examination for accurate diagnosis and treatment planning were enrolled in the study. A total of 620 healthy, untreated, fully developed maxillary first and second molars were included (314 first molars and 306 second molars). The following observations were recorded: (1) number of roots and their morphology, (2) number of canals per root, (3) fused roots, and (4) primary variations in the morphology of the root canal systems.

Results

First and second molars showed a higher prevalence of 3 separate roots, mesiobuccal, distobuccal, and palatal, with 1 canal in each root (52.87% and 45.09%, respectively). Two canals in the mesiobuccal roots represented 42.63% of teeth, whereas mesiobuccal roots of second molars presented 2 canals in 34.32%. The most common anatomic variation in the maxillary first molar was related to the root canal configuration of the mesiobuccal root, whereas the root canal system of the maxillary second molar teeth showed more anatomic variables.

Conclusions

Mesiobuccal roots of maxillary molar teeth had more variation in their canal system than the distobuccal or palatal roots. The root canal configuration of the maxillary second molars was more variable than the first molars in a Brazilian population. CBCT imaging is a clinically useful tool for endodontic diagnosis and treatment planning.  相似文献

Abstract The purpose of this investigation was to study the prevalence of fused roots, C-shaped roots, C-shaped root canal orifices and C-shaped root canals in the maxillary molars of a Chinese population. A total of 305 first molars and 309 second molars were collected in Taiwan. The teeth were demineralized and placed in methyl salicylate to make them transparent. Root fusion was examined and the pulp chamber floor was checked for C-shaped orifices. Chinese ink was then injected into the root canal system to demonstrate possible C-shaped canals. The maxillary first molars had the palatal root fused with the mesiobuccal root in 0.3% of the cases, and with the distobuccal root in 2.0%. Teeth with C-shaped roots existed in only 0.3% of maxillary first molars, while 6.2% of the maxillary first molars and 40.1% of the maxillary second molars had fused roots. The maxillary second molars had the palatal root fused with the mesiobuccal root in 18.1% and with the distobuccal root in 2.6%; the palatal root was fused with the mesiobuccal and distobuccal root in 8.1% of the material. The maxillary second molars had a C-shaped root in 4.5% and C-shaped root canal orifices with C-shaped root canals in 4.9% of the cases. Fused roots or incompletely separated roots are common in the maxillary second molars amongst Chinese people, while C-shaped roots and root canals in maxillary molars are not frequently seen.  相似文献

Identifying the variations of root canal morphology is crucial prior to commencing any endodontic treatment. The advancement of current endodontic instrumentation and technology has greatly enhanced treatment outcomes, which are now more predictable. This clinical case report presents a case of a maxillary first molar showing six root canals and apical foramina, i.e. three mesiobuccal canals, two palatal canals and one distobuccal canal. AIM: To investigate the root and canal morphology of permanent maxillary molar teeth from a Ugandan population. METHODOLOGY: Maxillary first (n = 221) and second molar (n = 221) teeth were collected from patients attending dental clinics in Kampala. Teeth were prepared using a clearing technique: the pulp chambers were accessed and the teeth placed consecutively into 5% sodium hypochlorite, 10% nitric acid, then methyl salicylate. Indian ink was injected into the pulp chambers to demonstrate the canal system. RESULTS: In the first molars, 95.9% of the teeth had separate roots. The mesiobuccal root was fused with the palatal root in 3% of specimens and with the distobuccal root in 0.5% of teeth. In the second molars, 86% of the teeth had separate roots. The mesiobuccal root was fused with the palatal root in 6.3% of specimens and with the distobuccal root in 6.8% of teeth. Apical deltas were more frequent in the mesiobuccal root when compared with distobuccal and palatal roots of both the first and second molars. A type I canal configuration (>75%) was the most frequent in all the roots of both the first and second molars. Canal intercommunications and lateral canals were more frequent in the mesiobuccal root when compared with other roots. CONCLUSIONS: The mesiobuccal root tended to have more variations in the canal system followed by the distobuccal root, whereas the palatal root had the least. The findings in root and canal morphology of this Ugandan population were different from previous studies, which may partly be attributed to racial differences.  相似文献

The root canal anatomy of 149 mandibular second molars was studied using a technique in which the pulp was removed, the canal space filled with black ink and the roots demineralized and made transparent. Of the 149 teeth, 22 per cent had single roots, 76 per cent had two roots and 2 per cent had three roots. In the single-rooted teeth, three canals were most common, while in the mesial root of the two-rooted teeth, two canals that joined near the apex and one canal in the distal root were most frequent. Round canals were most common in two-rooted teeth and C-shaped canals in single-rooted teeth. Transverse anastomoses were found in 33 per cent of roots, most commonly in the middle third of the root. Lateral canals were found in 72 per cent of roots, most commonly in the apical third of the root. The apical foramen was positioned at the apex in only 33 per cent of roots, and apical deltas were found in 35 per cent. The patient's age and race affected canal shape, with more round canals present in patients over 35 years of age, and more C-shaped canals in Asians. The sex of the patient and the side of the mouth affected the presence of apical deltas, with more being found in males and on the left side. Single-rooted teeth had more complex root canal systems than two-rooted teeth, with more lateral canals, transverse anastomoses, apical deltas and C-shaped canals.  相似文献

Aim  To investigate the root form and canal morphology of mandibular first molar teeth in a Taiwan Chinese population.
Methodology  A total of 183 mandibular first molars were collected and examined visually through a dissecting microscope after rendering the teeth transparent. The number of roots, number of canals and canal configuration were recorded. The root canal morphology was categorized and compared according to Vertucci (1984).
Results  Overall 46% of the mandibular first molars had four canals and 20% had extra-distal roots (distolingual root). In addition, 97% of the specimens had two mesial canals and 46% had two distal canals. In total, 68% of teeth with two mesial canals had two separated apical foramina, and 33% of teeth with two distal canals had two apical foramina. The most common canal configurations (Vertucci, 1984) of mesial roots were type 4 (46%) and type 2 (23%). Type 1 was encountered in 54% of the distal roots.
Conclusions  The frequency of the extra-distal root on the mandibular first molar was 20%, and the incidence of three canals (Vertucci's type 8 classification) with separate apical foramina at the apex of the mesial root was 6%. The three-rooted variation of the mandibular first molar appears to be a genetic characteristic of an Asiatic racial background.  相似文献

The main purpose of this study was to investigate the root and canal morphology of Sri Lankan mandibular molars and to determine the affinities of these morphological variations to those of people of European and Asian origin. Two hundred mandibular first and second molars were examined. The number of roots and the prevalence of C-shaped (gutter-shaped) roots in mandibular second molars were recorded. Root canal morphology was studied using a clearing technique. The examination of root canal systems of the teeth was based on Vertucci's classification. Among mandibular first molars, only 3% had three roots. Mesial roots of the first molar typically presented with two canals and two apical foramina with type IV, V, or VI canal configuration. Most distal roots of the first molar presented with a type I canal configuration. The remainder were distributed mainly between types III and V. Among mandibular second molars, 6% had single C-shaped roots, while C-shaped canals were found in only 2%. The majority of mesial roots of the second molar showed one or two canals, with one apical foramen with type I, II, or III canal morphology. In addition, the distal root commonly showed type I canal configuration. Intercanal communications and lateral canals were more frequently seen in the first molar than in the second. The root canal morphology in mandibular molars varies among population groups. Mandibular root and canal morphology of Sri Lankan people has closer affinities with that of people of European than of East Asian origin.  相似文献
