加强医患沟通是防范医疗纠纷的重要手段,对减少医疗纠纷的发生具有十分重要的意义。目前医疗纠纷的成因主要是医务人员的服务态度、道德素养、技术水平欠佳,医疗机构的管理不到位以及患者及家属的主观意识等因素造成。加强医患沟通可取得医患之间的相互理解、相互信任、相互谅解,并可取得医患之间的密切合作,也是尊重患者的重要表现。要想使医患沟通更加顺利并想获得比较全面的信息,应主要做好以下几个方面的工作:(1)提高自身素质,增加沟通的知识性和通俗性;(2)因人而异,增加沟通的灵活性和亲切感;(3)认真倾听,鼓励病人积极暴露信息;(4)实事求是,确保沟通信息的真实性。 相似文献

Based on Street’s (2003) ecological framework of communication in medical encounters, this study examined personal, interpersonal, and media factors that could influence patients’ use of the Internet to communicate with doctors. Results from data analysis of responses from the 2014 Health Information National Trends Survey showed that patient activation and ease of Internet access were two positive predictors of online doctor-patient communication. In addition, patients’ trust in doctors positively moderated the relationships between patient activation and online doctor-patient communication, and between perceived health status and online doctor-patient communication. Finally, the quality of patients’ past experiences communicating with doctors had a positive moderation effect on the association between health information seeking behavior and online doctor-patient communication. In this article I analyze doctor-patient use of e-mail within the context of the evolving doctor-patient relationship and ongoing changes in the U.S. health care system. Evidence from the published literature on doctor-patient relationships and doctor-patient communication, empirical studies of uses of the Internet by doctors and patients, and commentaries about the Internet's role in health care are examined and discussed to reveal common, unsupported assumptions about the likely impact of e-mail on the doctor-patient relationship and also the value structure associated with current e-mail practices between doctors and patients. I argue that existing research does not adequately account for the technical, professional, and economic forces that are shaping doctor-patient use of e-mail. I conclude that physicians' preferences for technical, instrumental exchanges with patients will likely mitigate the positive influence that e-mail could have on the doctor-patient relationship, and doctor-patient communication is unlikely to improve as a result of their use of e-mail. 相似文献

Social determinants of health in Canada: Are healthy living initiatives there yet? A policy analysis
Objective
In recent years, the importance of social differences in the physician-patient relationship has frequently been the subject of research. A 2002 review synthesised the evidence on this topic. Considering the increasing importance of social inequalities in health care, an actualization of this review seemed appropriate.Methods
A systematic search of literature published between 1965 and 2011 on the social gradient in doctor-patient communication. In this review social class was determined by patient's income, education or occupation.Results
Twenty original research papers and meta-analyses were included. Social differences in doctor-patient communication were described according to the following classification: verbal behaviour including instrumental and affective behaviour, non-verbal behaviour and patient-centred behaviour.Conclusion
This review indicates that the literature on the social gradient in doctor-patient communication that was published in the last decade, addresses new issues and themes. Firstly, most of the found studies emphasize the importance of the reciprocity of communication. Secondly, there seems to be a growing interest in patient's perception of doctor-patient communication.Practice implications
By increasing the doctors' awareness of the communicative differences and by empowering patients to express concerns and preferences, a more effective communication could be established. 相似文献17.

Research design We examined the use of the Internet by women with breast cancer and men with prostate cancer with an online questionnaire.

Results While trust in physicans and their medical authority is not being fundamentally questioned, patients are increasingly confronting
their doctors with high expectations concerning transparent information, openness, and willingness to shared decision-making;
they want to know the doctors’ opinions on various possibilities of treatment, and they expect serious comment on their findings

The Internet has become a major health information source for many patients, and they might discuss the information they get from the Internet with their doctors. I explored how the Internet as an information source influences cancer patients' communication with their doctors in Taiwan, where the doctor-patient relationship is traditionally doctor dominated. Forty-six cancer patients or families participated in seven focus group discussions. I conducted inductive analysis to examine themes emerging from discussions. Participants searched for information on the Internet to probe and verify their doctors' competence. Participants took responsibility for understanding the doctors' jargon, and the Internet helped them to do that. The Internet also helped participants spur doctors to think further about their condition, but these patients did so cautiously, with an effort not to offend doctors. The Internet as an information source did help participants talk to doctors, but the effect on changing the doctor-dominant nature of the relationship was limited. 相似文献
