PAROXYSMAL HEMOGLOBINURIA WITH A REPORT OF A CASE IN A CHINESE

Paroxysmal hemoglobinuria is an uncommon disease: Up to date there have been hardly 300 cases reported in the literature. The disease has appeared to have been relatively more common in Japan than elsewhere in this part of the world. So far, there is no record of its occurrence among the.Chinese. S. Inamori(11 and N. Ito (2) report- ed two cases from Mukden Medical College Hospital in Manchuria, but the nationality of these two patients was not mentioned.     

CHOLECYSTECTOMY THROUGH A MINI-LAPAROTOMY A PRELIMINARY REPORT

Cholecystectomy through a small subcostal incision (mini-lap cholecystectomy), has recently been introduced as an alternative to conventional cholecystectomy in an effort to reduce its attendant morbidity. A trial was conducted to assess the morbidity of cholecystectomy performed through a small subcostal incision. Eighteen consecutive patients posted for elective cholecystectomy were operated through such an incision. In 2 [11%], the incision had to be extended. The records of these patients were retrospectively compared with an equal number of consecutive cholecystectomies previously performed by the same surgeon through a conventional incision. There was no significant difference in the average operating time, incidence of wound infection or the number of post-operative complications between the conventional and the mini-laparotomy group. However, the number of doses of post operative analgesic required, the duration of post-operative ileus, hospitalisation and convalescence needed was nearly halved. Thus mini-lap cholecystectomy has much lesser morbidity and is considered to be a safe and viable alternative to conventional cholecystectomy.KEY WORDS: Cholecystectomy, Mini-laparotomy, Post-operative morbidity

维生素A中毒1例

患儿 ,女 ,3岁 ,因“发热、头痛、呕吐伴咳嗽 2ｄ”入院 ,近 1个月来患儿全身皮肤反复出现皮疹 ,瘙痒明显 ,纳差。查体 :生命体征平稳 ,精神较差 ,全身皮肤粗糙 ,可见散在红色丘疹及新旧抓痕和血迹 ,口唇红 ,略干 ,咽部充血 ,双侧扁桃体ＩＩ度肿大 ,未见脓性分泌物 ,心、肺、腹部及神经系统检查无异常 ,拟诊“上呼吸道感染”予抗感染及对症治疗 ,患儿上呼吸道症状消失 ,而头痛、呕吐、皮肤瘙痒未见好转 ,并出现全身非凹陷性肿胀 ,以颜面、四肢为主 ,触痛明显 ,且口唇干燥、皲裂和鼻粘膜出血 ,怀疑维生素Ａ中毒。经详细追问病史 ,发现患儿自 …     

A Recurrent Ectopic Pregnancy within A Previous Caesarean Scar: A Case Report

Objective To investigate the clinical diagnosis and treatment of caesarean scar pregnancy (CSP). Methods We reported here a case of recurrent ectopic pregnancy within a previous cesarean scar and reviewed the literature. Results Surgical evacuation of a CSP was a small side effect and effective treatment of CSP. Conclusion Early and accurate diagnosis, timeliness and effective treatment were extremely important in saving patients?life and retaining their fertility.     

某幼儿园一起手足口病暴发的现场流行病学调查报告

目的了解某幼儿园手足口病暴发的流行病学特征及造成传染的主要原因,为进一步做好该园手足口病防控工作提供依据。方法按照病例定义,电话开展病例搜索,结合现场流行病学调查情况,形成假设,采用1:2匹配的病例对照研究,对照选取同班同年龄组同性别的儿童,使用统一的调查表询问儿童的日常卫生习惯。结果该幼儿园从2010年3月16日至3月23日共发生9例临床诊断病例,3例实验室确诊病例,罹患病率为7.95%。以小班病例为主,3～4岁组比例高(8/12)。3份粪便检测出病原体CoxA16。有吸吮手指、吸吮玩具、饭前不洗手、便后不洗手、挖鼻孔、揉眼睛等任一不良卫生习惯是手足口病发病的危险因素(OR=12,95%CI=1.3～114)。结论幼儿不良卫生习惯是该幼儿园手足口病传播的主要原因。建议该园加强儿童不良卫生习惯的纠正,培养良好卫生习惯,以达到控制传染的目的。     

ConA诱导抑制细胞检测法的改进

ConA诱导抑制细胞检测应用于检测抑制性T细胞的功能状态。原法要采血二次,并经五天才能判断结果。本文采用同时培育刺激细胞和应答细胞的方法,仅需采血一次,且在三天后即可判断结果。     

A Psychiatric Emergency Service: A Preliminary Report

An emergency service for psychiatric patients was organized in a general hospital owing to the increasing need for such a service. Its main functions have been to screen cases, to offer short-term treatment, and to provide immediate consultation facilities to general practitioners and clinical experience to students, interns and residents.

Of 133 patients, 40% were diagnosed as psychotic and 35% as neurotic. The commonest presenting symptoms were anxiety and depression. Approximately 40% were treated by the staff of the emergency service, 40% were sent back to the general practitioner for treatment, and 20% were transferred to a mental hospital.

It is concluded that this type of service can provide immediate and effective consultation and treatment for psychiatric emergencies, and is a very useful teaching facility, especially for students and physicians not intending to specialize in psychiatry.

Family history is.essentially irrelevant. Mother never had abortion Patient bas never been seriously ill before. Her menstruation-began at the age of fifteen, and has been always regular (3/28). She has never experienced dysmenorrhea or other acute 8ymptoms pointing to some illness of pelvic lesions. She has neVer. had any trouble with micturition nor abortion since her marriage 3 years ago: She denies venereal disease* or drug addiction.     

VOLKMANN'S CONTRACTURE WITH A REPORT OF A CASE

Richard Volkman n, in 1809, first referred to a severe contracture of the hand which he ascribed to tigbt bandaging in the treatment of fractures about the elbow; and, in bis classical article (1) in 1881, reaf- firmed his belief in the ischemic nature of the affection which he thought due to deprivation of arterial blood and to venous obstruction. Thomas (2), in 1909, reviewed the literature and found record of 103 cases from the time of Volkmann''s original report to that of Quinby''s report in 1908. To these he added four cases of his own. In 1908, Jones (3) stated he had seen 24 cases. In 1914, Murphy (4) presented six cases, but stated tliat he had seen ''a large number'' of others. From 1910 t0 1930 nurr.erous reports of this condition have entered the literature (5, 6, 8, 9).     

单个动物多项免疫功能检测研究

用SRBC经不同途径(sc+ip、sc+iv、ip、iv)免疫单只动物即可诱导产生迟发型变态反应(DTH),又能同时诱导产生抗体,皮下(sc)免疫只能诱导产生DTH。这个动物模型的灵敏性用免疫抑制药物坏磷酰胺和免疫增强药物厌氧棒状杆菌菌苗进行检测。给予小鼠环磷酰胺可明显抑制DTH和抗体(IgM PFC)的反应,而厌氧棒状杆菌菌苗可明显增强小鼠的抗体反应。多项免疫功能检测与单项免疫功能检测方法所得结果非常相似,单个动物多项免疫功能检测是一个经济、省时、灵敏的评价化合物和药物的免疫毒性和药理毒性的方法。
