目的 探讨乙型肝炎患者的乙型肝炎病毒(HBV)血清学标志(HBV M)与HBV DNA检测结果的相关性与临床意义。方法 对414例乙型肝炎的HBV M和HBV DNA检测结果进行比较。HBV M用ELISA定量分析法检测,HBV DNA用斑点杂交法检测。结果 急性、慢性乙型肝炎患者中HBV DNA的阳性率与乙型肝炎肝硬化患者的HBV DNA阳性率比较,差异有显著性;HBsAg、抗-HBe、抗-HBc阳性和HBsAg、HBeAg、抗-HBc阳性组的HBV DNA阳性率比较,差异无显著性;HBsAg和/或HBeAg的滴度与HBV DNA阳性率呈正相关关系。结论 HBV DNA是评价HBV活动最理想的标志;抗-HBe的出现不能作为HBV复制停止的指标;HBsAg的滴度和HBeAg的滴度变化可作为临床评价病毒复制程度和  相似文献

肝硬变内HBV DNA及其五种抗原的表达及意义   总被引:1,自引:1,他引:1  
取225例人肝硬变活检组织石蜡切片,检测了HBVDNA及其5种抗原。分别用免疫组化ABC法检测HBxAg、pre-S_1和pre-S_2抗原;用PAP法检测HBsAg和HBcAg;用原位杂交方法检测HBVDNA;用免疫组化、原位杂交双标记方法检测HBVDNA和HBsAg、HBxAg或HBcAg。结果显示,阳性检出率HBsAg为70.0%(128/183例),pre-S_1抗原为64.4%(85/132例)、pre-S_2抗原为61.4%(81/132例),HBxAg为75.3%(113/150例),HBcAg为22.4%(39/174例),HBVDNA为62.4%(58/93例)。双标阳性检出率HBVDNA和HBsAg为37.3%(19/51例),HBVDNA和HBx-Ag为86.3%(44/51例),HBVDNA和HBcAg为39.2%(20/51例)。HBVDNA和HBV5种抗原阳性病例中80%以上均伴有肝细胞不典型增生。这一结果表明,在我国肝硬变的发生发展与HBV慢性感染有密切的关系。  相似文献

为探讨Th1与Th2两类细胞因子间相互调节,研究了rhIL-10对PHA体外诱导人PBMC产生IL-2及IFN-γ的影响。结果表明,10ng/mlIL-10明显抑制PBMC产生IL-2及IFN-γ(P<0.02及P<0.01)。在加入兔抗人IL-10抗体的PBMC体外培养体系中,在5μg/ml浓度时,IL-2产生可明显增加(P<0.05),随着抗体浓度增加,IL-2呈剂量依赖性增加(r=0.962,P<0.01),IFN-γ产生亦增加,但差异不明显。本研究表明IL-10抑制活化人PBMC体外产生IL-2及IFN-γ,而抗人IL-10抗体则表现出增强作用。IL-10介导Th1与Th2两类辅助性T细胞间的相互调节,选择机体对抗原的免疫应答类型,从而对临床疾患的防治有一定指导意义。  相似文献

应用半乳糖末端糖蛋白受体(ASGP-R)介导的内吞作用,将外源基因导入真核细胞,与脂质体介导的转染和细胞表面转铁蛋白受体(Tf-R)介导的内吞作用相比,虽然三种方式均能有效介导外源基因的转移,但ASGP-R法具有肝细胞特异性,而脂质体法和Tf-R法不具此特性。将克隆于真核表达载体的针对乙型肝炎病毒(HBV)mRNAPreC/C区的核酶质粒pCMV-Ripc特异性导入肝细胞并发挥作用,通过酶联免疫吸附法(ELISA)检测细胞培养液中的乙型肝炎表面抗原(HBsAg)和e抗原(HBeAg),评价核酶在细胞水平对HBV抗原表达的阻断作用。结果表明当核酶质粒pCMV-Ripc与HBV抗原表达质粒pUC-2HBV共转染HepG2细胞时,核酶对HBsAg和HBeAg表达的抑制率分别为55.29%和68.73%。  相似文献

In chronic hepatitis B virus (HBV) infection, immune responses to hepatitis B core antigen (HBcAg) are weak. Interleukin (IL)-10 is a potent immunosuppressive cytokine which we reported recently to be secreted in response to HBcAg by peripheral blood mononuclear cells (PBMCs) from patients with chronic HBV infection or healthy controls. Using an enzyme-linked immunospot assay, we compared the ability of HBcAg to stimulate IL-10 production by PBMC with that of lipopolysaccharide (LPS), phytohaemagglutinin-P and hepatitis C virus-derived antigens in 16 patients with chronic HBV infection and six healthy controls. Frequencies of IL-10 spot-forming cells (SFC) in response to HBcAg were comparable to those obtained with LPS in patients with chronic HBV infection. Frequencies of IL-10 SFC in response to HBcAg or to LPS were significantly higher in patients with chronic HBV infection than in healthy controls. IL-10 SFC in response to HBcAg consisted of 26-35% T cells, 62-70% monocytes and less than 1% B cells in patients with chronic HBV infection. Only monocytes contributed to IL-10 production in controls. Frequencies of HBcAg stimulated IL-10 SFC representing T cells and monocytes were significantly higher in patients with elevated serum alanine aminotransferase (ALT) and detectable HBV DNA than in patients with normal ALT and undetectable HBV DNA. The potent ability of HBcAg to stimulate IL-10 production by PBMC may contribute importantly to immune tolerance toward HBV.  相似文献

探讨慢性乙肝患者树突状细胞(dendritic cells,DC)对CD4+Th细胞亚群分化的影响。分离慢性乙肝患者外周血单个核细胞(PBMC),以rhIL-4(50 ng/ml)、rhGM-CSF(10 ng/ml)和rhTNF-α(100 u/ml)诱导培养DC。以流式细胞仪检测DC表面CD1a、CD83、CD80、CD86、HLA-DR分子表达情况。MTT法检测DC刺激同种异体淋巴细胞增殖能力。免疫磁珠分离外周血CD4+T细胞亚群,PMA+Ionomycin刺激后胞内荧光染色,流式细胞仪检测辅助性T细胞(helper T cell,Th)内特征性细胞因子IFN-γ/IL-4以判断Th1/Th2分化。ELISA法检测DC或Th细胞培养上清中IL-6、IL-12、IFN-γ和IL-4的含量。结果:慢性乙肝患者的DC表达CD1a、CD83、CD80、CD86、HLA-DR分子水平明显低于正常人(P<0.01);培养至第7天,慢性乙肝患者DC分泌的IL-12水平低于正常人(P<0.01),而分泌的IL-6水平增高(P<0.05)。与正常人相比,慢性乙肝患者外周血中Th1细胞占CD4+T细胞的百分比较低(P<0.01),其Th细胞培养上清中IFN-γ的量也较低(P<0.01)。患者DC与同种异体的健康人Th细胞共培养,刺激Th1型细胞因子IFN-γ产生的能力低于正常人(P<0.01)。慢性乙肝患者体内DC功能的异常可能导致了外周血Th1细胞分化不足。  相似文献

目的 研究合并HBV感染的晚期血吸虫病患者外周血Th1/Th2类细胞因子的表达及临床意义.方法 选择合并HBV感染的晚期血吸虫病患者、乙型肝炎后肝硬化患者、单纯晚期血吸虫病患者各30例,分别设为A、B和C组,分别采用日立7600全自动生化分析仪检测肝功能,lightcycle基因荧光定量分析仪检测HBV DNA,流式细胞检测技术检测外周血Th1和Th2类细胞因子,并进行比较.结果 A组丙氨酸转氨酶水平、天冬氨酸氨基转移酶水平、总胆红素水平高于B组、C组,差异均有统计学意义,(A组与B组比较t=4.725、3.292、3.012;A组与C组比较t=4.568、3.126、3.217,P均<0.05);A组HBV DNA水平低于B组,差异有统计学意义(t =3.916,P<0.05).A组外周血IFN-γ、TFN-α表达水平较B组、C组增加,A组与C组之间差异有统计学意义(t =3.652、3.413,P均<0.05).A组外周血IL-4、IL-13表达水平高于B组,低于C组,但与B组和C组之间结果比较差异均无统计学意义(A组与B组比较t=0.721、0.452、A组与C组比较t=0.625、0.632,P均>0.05).结论 合并HBV感染的晚期血吸虫病患者,在疾病的发生发展中Th1类细胞因子可能起了重要作用.  相似文献

目的:乙肝病毒蛋白对不同感染状态慢性乙型肝炎患者的外周血单核细胞(PBMCs)功能的影响。方法:收集乙肝表面抗原(HBeAg)阳性丙氨酸氨基转移酶(ALT)异常者40例(A组), 乙肝e抗原(HBeAg)阳性ALT持续正常者20例(B组), HBeAg及 乙肝病毒脱氧核糖核酸(HBV-DNA)阴性ALT正常者20例(C组),另外选择15例各型肝炎病原学标志检测均为阴性的健康体检者为正常对照组,分离培养外周血单核细胞,加入植物血凝素(PHA)、HBeAg、HBcAg, 培养48 h后,1 500 r/min 离心10 min,收集上清液-20 ℃保存,统一检测IFN-γ、IL-10含量。采用流式细胞仪检测外周血CD8+CD28+T细胞亚群。结果:IFN-γ在HBeAg阳性ALT正常组,HBeAg阳性ALT异常组中的含量明显低于正常对照组和HBeAg及 HBV-DNA阴性ALT正常组;IL-10在HBeAg阳性ALT正常组,HBeAg阳性ALT异常组中的含量明显高于正常对照组和HBeAg及 HBV-DNA阴性ALT正常组。CD8+CD28+T细胞在HBeAg阳性ALT正常组中明显低于其它各组。结论:HBeAg阳性的患者,Th1型细胞免疫反应明显低下,Th2型细胞免疫反应明显增高,CTL的数量减少。  相似文献

Recent studies about autoimmune diseases in animal models and in humans focused their attention on lymphocyte activation and in vitro cytokine production. The respective contribution of the Th1 and Th2 cytokines to the pathogenesis of autoimmune diseases is still a matter of debate. In this study the role of IL-2, IL-4, IFN- &#110, IL-10 and IL-12 cytokines were investigated by examining their spontaneous and mitogen-induced (OKT3 and PHA or LPS) synthesis and T-cells proliferative response by peripheral blood mononuclear cells to determine their role in the pathogenesis of AIHA. Thirteen patients affected by AIHA, idiopathic or associated with other diseases, and 13 healthy subjects, randomly selected from a group of blood donors, were investigated. This study indicated that AIHA is characterised by increased basal synthesis of IL-4 and decreased levels of IFN- &#110 compared with healthy controls ( p <0,01). These results suggest that there is a basal decrease of Th1 cytokine and an increase of the Th2 ones. Enhanced IL-2 levels in AIHA patients are likely due to the necessity of a T-cell proliferation stimulus rather than produced as Th1 prevalent stimulation. Furthermore, it has been observed a significant increase in IL-12 production in LPS stimulated cultures from healthy controls, but not in AIHA patients, that shows IL-10 increased levels, which could cause a secondary decrease in IFN- &#110 production and a stimulation of Th2 differentiation. These observations indicate that decreased production of Th1-type cytokines and prevalent Th2 ones leading to autoantibodies production in AIHA may be secondary to the imbalance between IL-10 and IL-12. These results strongly suggest that manipulation of the cytokine network, i.e. IL-10/IL-12 balance, maintained by cells of the innate immune system, can have a strong effect on the incidence of AIHA and their modulation might be useful for a therapeutic control of the disorder.  相似文献

A proportion of healthy neonates fail to produce protective levels of anti-HBs antibody following vaccination with recombinant hepatitis B vaccine. This study was undertaken to investigate contribution of Th1 and Th2 responses to anti-HBs antibody production and to explore the mechanism(s) of unresponsiveness to HBsAg in human neonates. Peripheral blood manonuclear cells (PBMCs) were isolated form 28 nonresponder (anti-HBs antibody <10 IU/l) and 25 responder neonates. The cells were stimulated in vitro with recombinant HBsAg and PHA mitogen and concentrations of IL-4, IL-10 and IFN-gamma were quantified in culture supernatants by sandwich ELISA. Our results demonstrated significantly increased production of all cytokines, including IL-4 (P < 0.001), IL-10 (P < 0.002) and IFN-gamma (P < 0.01) in responder compared to nonresponder vaccinees. No significant differences, however, were observed between the two groups of neonates in the levels of cytokines induced by PHA or secreted in absence of antigen and mitogen. Our findings suggest that unresponsiveness to recombinant HBsAg in healthy neonates is linked to inadequate secretion of both Th1 and Th2 cytokines.  相似文献

The importance of cytokine production in some disease processes is now widely recognized. To investigate temporal relationships between cytokines, we stimulated peripheral blood mononuclear cells (PBMC) in vitro using the T cell mitogen phytohemagglutinin (PHA) and various antigens chosen to induce predominantly Th1 (streptokinase: streptodornase or purified protein derivative) or Th2 (Dermatophagoides pteronyssinus, bee or wasp venom: allergens in sensitive subjects) responses. Cytokine production was measured by sensitive bioassays or enzyme-linked immunosorbent assays. Of the 30 subjects studied, 10 were normal and 20 individuals were allergic to either D. pteronyssinus (n = 10) or bee venom (n = 10) (examined before specific allergen immunotherapy). We examined the temporal profiles of a panel of cytokines produced in prmary culture. In PHA-driven cultures, cytokines were found to be sequentially produced in the order interleukin (IL)-2, IL-4, IL-5, IL-3, interferon (IFN)-γ, IL-10, IL-6, IL-12 and tumor necrosis factor (TNF)-α. The response to allergen in allergic patients was predominantly Th2 in nature, with the production of IL-4, IL-5, IL-6 and IL-10, but little or no IFN-γ. IL-2, IL-3, TNF-α and IL-12 were also produced in low amounts. The response of both atopic and normal subjects to recall bacterial antigens was predominantly Th1, with high levels of IFN-γ, IL-2 and TNF-α. The relevance of the order, amount and speed of production, characteristic kinetics (production, consumption, homeostatic regulation) and the cell source of the cytokines are discussed.  相似文献

目的: 探讨八肽胆囊收缩素(CCK-8)对Th1/Th2平衡的调节作用。方法: 给予BALB/c小鼠钥孔戚血蓝蛋白(KLH)免疫同时体内给予不同剂量的CCK-8,酶联免疫吸附试验(ELISA)检测其脾细胞培养上清中Th1型细胞因子γ-干扰素(IFN-γ)、白细胞介素-2(IL-2)和Th2型细胞因子白细胞介素-4(IL-4)、白细胞介素-5(IL-5)水平,逆转录聚合酶链式反应(RT-PCR)法检测脾细胞中IFN-γ、IL-2、IL-4、IL-5 mRNA表达;ELISA法检测血清中Th1型抗KLH抗体IgG2a和Th2型抗KLH抗体IgG1水平。结果: ①KLH免疫使小鼠脾细胞分泌Th1/Th2型细胞因子水平明显增高,mRNA表达增高,KLH免疫同时给予CCK-8可使脾细胞培养上清中IFN-γ、IL-2含量进一步增加和IFN-γ、IL-2mRNA表达增高,而使IL-4、IL-5含量降低,IL-4、IL-5 mRNA表达减低和降低IL-4/IFN-γ比值。②KLH免疫小鼠血清中IgG2a、IgG1发生不同程度增高,CCK-8可使其血清中IgG1水平减低而使IgG2a水平增高。结论: CCK-8可促进KLH免疫小鼠体内Th1反应,使Th2优势反应向Th1方向转变。  相似文献

目的:分析7种乳杆菌对原代淋巴细胞增殖和细胞因子(CK)分泌的作用,进而探讨其对Th1/Th2细胞平衡的影响。方法:用不同种属、不同浓度的活的/热致死的乳杆菌体外作用于小鼠脾淋巴细胞培养60 h后,采用MTT比色法检测淋巴细胞的增殖效果。用ELISA法检测Th1型细胞因子(IL-12、IFN-γ)、Th2型细胞因子(IL-4、IL-10)和调节型细胞因子(TGF-β)的分泌量。结果:活的/热致死的乳杆菌单独作用,就能促进淋巴细胞体外增殖并表现出剂量依赖关系(P<0.05)。当菌的浓度为107集落形成单位(CFU)/mL(即细菌与细胞的比例为10∶1)时,热致死的发酵乳杆菌和嗜酸乳杆菌的免疫活性近似于活菌。而且,这两株热致死菌还可适当提高淋巴细胞分泌IL-12和IFN-γ,抑制IL-4、IL-10和TGF-β的分泌,使其IFN-γ/IL-4的比值(代表Th1/Th2细胞平衡)均显著高于刀豆蛋白A(ConA)对照组(P<0.05)。结论:乳杆菌可通过提高淋巴细胞的IFN-γ/IL-4分泌率来促进Th1优势状态的Th1/Th2细胞平衡,并具有菌株特异性。  相似文献
