S1椎弓根螺钉结合髂骨板间螺钉治疗骶髂关节骨折脱位   总被引:4,自引:0,他引:4  
目的 探索S1椎弓根螺钉结合髂骨板问螺钉治疗骶髂关节骨折脱位的临床疗效,评价两者结合对骶髂关节骨折脱位的治疗价值。方法 对11例骶髂关节骨折脱位患者用脊柱内固定系统(TSRH)之S1椎弓根螺钉结合髂骨板间螺钉进行固定,该组患者涉及骶髂关节的垂直移位及旋转的骨盆环变形,归于Tile分型的B类或C类骨盆损伤。11例患者均伴有前环损伤,其中9例予以加压钢板(smith nephew)内固定,余2例患者单纯采用后路手术内固定。结果 7例患者垂直移位完全复位,9例旋转畸形纠正,未发现感染及神经损伤等并发症。结论 S1椎弓根螺钉结合髂骨板问螺钉固定技术治疗骶髂关节骨折脱位,可获得即刻稳定性并良好地维持了复位的效果.这一混合技术对于涉及垂直及旋转损伤的骨盆环损伤有稳定的作用。  相似文献

OBJECTIVE: To measure the failure rate of percutaneous iliosacral screw fixation of vertically unstable pelvic fractures and particularly to test the hypothesis that fixations in which the posterior injury is a vertical fracture of the sacrum are more likely to fail than fixations with dislocations or fracture-dislocations of the sacroiliac joint. DESIGN: Retrospective review. SETTING: Level 1 trauma center. METHODS: All patients with pelvic fractures admitted between January 1, 1993, and December 31, 1998, were identified from the trauma registry. Hospital records were used to identify patients treated with iliosacral screws. Radiologic studies were examined to identify patients who had unequivocally vertically unstable pelvic fractures. Immediate postoperative and follow-up anteroposterior, inlet, and outlet radiographs from a minimum of 12 months postinjury were examined. Position, length, and numbers of iliosacral screws and any evidence of screw failure (eg, bending or breakage) were recorded. Residual postoperative displacement and late displacement of the posterior pelvis were measured. The main outcome measure was failure, defined as at least 1cm of combined vertical displacement of the posterior pelvis compared with immediate postoperative position. The main analysis was for association between fracture pattern and failure. Patient demographic data, iliosacral screw position, and anterior pelvic fixation method also were studied. RESULTS: The study group comprised 62 patients with unequivocally vertically unstable pelvic fractures in whom the posterior injury was treated with closed reduction and percutaneous iliosacral screw fixation. Of patients, 32 had dislocations or fracture-dislocations of the sacroiliac joint, and 30 had vertical fractures of the sacrum. Fixation failed in four patients, all with vertical sacral fractures and all within the first 3 weeks after surgery. These four patients required revision fixation. In two further cases with vertical sacral fractures, there was evidence that the fracture had only barely been held by the fixation, but these fractures healed, and follow-up radiographs did not meet the displacement criteria for failure. A vertical sacral fracture pattern was associated significantly with failure (Fisher exact test, P = 0.04); the excess risk of failure compared with sacroiliac joint injury was 13% (95% confidence interval 1% to 25%). There was no significant association between failure and anterior fixation method, iliosacral screw arrangement or length, or any demographic or injury variable. CONCLUSIONS: Percutaneous iliosacral screw fixation is a useful technique in the management of vertically unstable pelvic fractures, but a vertical sacral fracture should make the surgeon more wary of fixation failure and loss of reduction.  相似文献

Objective  Stable internal screw fixation of posterior pelvic ring disruptions through a posterior approach. Indications  Complete, unstable sacroiliac dislocations with incompetence of anterior and posterior sacroiliac ligaments. Sacroiliac fracture dislocations. Displaced vertical sacral fractures. Contraindications  Damage to posterior soft tissues. Acceptable closed reduction of sacrum or sacroiliac joint. Ipsilateral acetabular fractures treated through an anterior approach. Inadequate intraoperative fluoroscopic visualization of posterior pelvis. Surgical Technique  Vertical paramedian incision overlying the sacroiliac joint. Release of origin of gluteus maximus. Inspection and reduction of sacroiliac joint. Stabilization with iliosacral screws under image intensification. Secure repair of gluteal fascia. Results  107 patients with unstable pelvic ring fractures were treated with open reduction and internal fixation of which 83 had an open reduction of posterior ring injuries. Accuracy of reduction: more than 95% of patients had residual displacement of less than 10 mm. Two patients had a deep wound infection postoperatively. Two-thirds of the patients were able to resume their previous occupation. Pain was either absent or occurred only with strenuous activities. 63% had a normal gait.  相似文献

OBJECTIVE: Closed reduction and retention of translatory unstable pelvic injuries (type C injuries), in order to restore the form and function of the posterior pelvis by percutaneous iliosacral screw osteosynthesis, using conventional fluoroscopy. INDICATIONS: Definitive treatment of the posterior pelvis in type C injuries (AO classification) with complete sacral fracture, sacroiliac joint (SI joint) dislocation, transiliac or transsacral dislocation fracture of the SI joint with insignificant small fragment and sacroiliac avulsion injuries which can be reduced almost anatomically in closed technique. CONTRAINDICATIONS: Poor general health, local soft-tissue damage, rotationally unstable type B pelvic injuries as well as type C injuries which cannot be reduced satisfactorily in closed technique. SURGICAL TECHNIQUE: Closed reduction, stab incision and percutaneous stabilization of the posterior pelvis by transiliosacral screw osteosynthesis, guided by fluoroscopy. POSTOPERATIVE MANAGEMENT: Partial loading of the injured side with 15 kg for 8-12 weeks with two underarm crutches. Implant removal 6-12 months after injury. RESULTS: 20 patients with a transforaminal sacral fracture consistent with a type C pelvic injury underwent screw fixation with fluoroscopy with 7.3-mm cannulated screws, placed in a transiliosacral position in the vertebral body of S1. The average preoperative displacement of 3.8 mm was decreased by closed reduction to 1.6 mm postoperatively. The average operating time was 55 min, the average screening time 2.22 min. Incorrect screw position with no consequences was observed in three patients; iatrogenic nerve damage was not found. All fractures healed within 3 months.  相似文献

BACKGROUND: The purpose of this comparative cadaveric study was to investigate whether the stability of partially unstable pelvic fractures can be improved by combining plate fixation of the symphysis with a posterior sacroiliac screw. METHODS: In six specimens, a Tile B1 (open-book) pelvic fracture was created. We compared the intact situation with isolated anterior plate fixation and plate with sacroiliac screw fixation. Using a three-dimensional video system, we measured the translation and rotation stiffness of the fixations and the load to failure. RESULTS: Neither absolute displacements at the os pubis or at the sacroiliac joint nor stiffness of the ilium with respect to the sacrum were significantly different for the techniques with or without sacroiliac screw or the intact situation. Load to failure was reached in only one of the six cases. In all other cases, the fixation of the pelvis to the frame failed before failure of the fixation itself. In these cases, a load of approximately 1,000 N or more could be applied. CONCLUSION: The addition of a sacroiliac screw in a Tile B1 fracture does not provide significant additional stability. Although cyclic loading was not tested, in these experiments forces could be applied that were similar to full body weight. Clinical experiments into direct postoperative weight bearing are recommended to examine the clinical situation.  相似文献

Biomechanical comparison of posterior pelvic ring fixation   总被引:35,自引:0,他引:35  
OBJECTIVE: To determine relative stiffness of various methods of posterior pelvic ring internal fixation. DESIGN: Simulated single leg stance loading of OTA 61-Cl.2, a2 fracture model (unilateral sacroiliac joint disruption and pubic symphysis diastasis). SETTING: Orthopaedic biomechanic laboratory. OUTCOME VARIABLES: Pubic symphysis gapping, sacroiliac joint gapping, hemipelvis coronal plane rotation. METHODS: Nine different posterior pelvic ring fixation methods were tested on each of six hard plastic pelvic models. Pubic symphysis was plated. The pelvic ring was loaded to 1000N. RESULTS: All data were normalized to values obtained with posterior fixation with a single iliosacral screw. The types of fixation could be grouped into three categories based on relative stiffness of fixation: For sacroiliac joint gapping, group 1-fixation stiffness 0.8 and above (least stiff) includes a single iliosacral screw (conditions A and J), an isolated tension band plate (condition F), and two sacral bars (condition H); group 2-fixation stiffness 0.6 to 0.8 (intermediate stiffness) includes a tension band plate and an iliosacral screw (condition E), one or two sacral bars in combination with an iliosacral screw (conditions G and I); group 3-fixation stiffness 0.6 and below (greatest stiffness) includes two anterior sacroiliac plates (condition D), two iliosacral screws (condition B), and two anterior sacroiliac plates and an iliosacral screw (condition C). For sacroiliac joint rotation, group 1-fixation stiffness 0.8 and above includes a single iliosacral screw (conditions A and J), two anterior sacroiliac plates (condition D), a tension band plate in isolation or in combination with an iliosacral screw (conditions E and F), and two sacral bars (condition H); group 2-fixation stiffness 0.6 to 0.8 (intermediate level of instability) includes either one or two sacral bars in combination with an iliosacral screw (conditions G and I); group 3-fixation stiffness 0.6 and below (stiffest fixation) consists of two iliosacral screws (condition B) and two anterior sacroiliac plates and an iliosacral screw (condition C). DISCUSSION: Under conditions of maximal instability with similar material properties between specimens, differences in stiffness of posterior pelvic ring fixation can be demonstrated. BACKGROUND: In recent years, the closed reduction and percutaneous fixation of posterior pelvic ring fractures by sacroiliac screws has become a well established treatment option for stabilization of posterior pelvic ring disruptions. Stable percutaneous pelvic ring fixation also implies a very low complication rate, e.g., in operative blood loss, wound healing, and operative time. To avoid malpositioning of the screws, sufficient reduction and radiologic visualization are essential. The surgical technique has been described in several studies; however, great importance is attached to the personal experience of the surgeon. Therefore, this study was conducted to establish a standard procedure that allows different surgeons a safe positioning of sacroiliac screws. RESULTS: A total of 41 injuries of the posterior pelvic ring were stabilized with 73 sacroiliac lag screws inserted by 7 different surgeons using a standardized technique. In all cases adequate reduction of the fracture and radiologic visualization were achieved. No wound infections, no relevant bleedings, and no spiral fractures of screws were observed. In two cases malpositioning led to revision of the screws. Of interest, one case of S1 paresthesia resulting from a malpositioned screw could be revised. In contrast, two cases of screw loosening and one case of screw bending did not require further intervention. CONCLUSION: We conclude that safe positioning of the sacroiliac screws was accomplished by all surgeons given a standardized technique. For safe insertion preparation of the patients, accurate visualization of the fracture zone, and potential closed reduction is always required.  相似文献

The primary goal in the treatment of pelvic fractures is the restoration of haemodynamic stability. The secondary goal is the reconstruction of stability and symmetry of the pelvic ring. Percutaneous reconstruction can only be accepted if these goals are met. The type of definitive surgery is dependent of the degree of instability of the anterior and posterior pelvic ring. Retrograde transpubic screw fixation of pubic rami fractures is a good alternative to external fixation or plate and screw osteosynthesis. The technique of screw placement and image intensifier control is explained. Internal fixation of pure sacroiliac dislocations, fracture-dislocations of the sacroiliac joint and sacral fractures can be fixed with sacroiliac screws, placed percutaneously. Reduction of the fracture or dislocation is performed closed, or open if anatomy cannot be restored in a closed manner. The primary goal in the treatment of acetabular fractures is to restore anatomy. Reduction comes before fixation. The goal of minimising approaches cannot be more important. In most cases open reduction will be necessary to achieve anatomical reconstruction. Only the experienced acetabular surgeon will be able to decide when and how he can restore anatomy through a less invasive approach or with a percutaneous procedure. The anterior column screw can be inserted through a separate incision in addition to a Kocher-Langenbeck approach. It is the same screw as the retrograde transpubic screw but placed in the opposite direction. The posterior column screw is placed percutaneously from the lateral cortex of the ilium in the direction of the posterior column. Techniques of placement of both screws are demonstrated. Open reduction and internal fixation remains the standard of care in stabilisation of pelvic and acetabular fractures. Only the experienced surgeon will be able to judge if percutaneous procedures can be an alternative or a useful additive to conventional techniques.  相似文献

IntroductionPercutaneous screw fixation is considered the best option in unstable pelvic fracture with severe soft tissue injury. However, fixation technique at the level of S3 has not been well established. This paper showed the feasible surgical technique of S3 screw insertion in unstable pelvic fracture with severe soft tissue injury.MethodsWe reported 2 cases of unstable pelvic injury of an 11 years old boy with Marvin-Tile (MT) C1 pelvic fracture with sacroiliac (SI) joint disruption, skin avulsion and Morel-Lavallée lesion. Second case was 30 years old male with open pelvic fracture MTB2 and vertical sacral fracture Denis zone I with Morel-Lavallée lesion, intraperitoneal bladder rupture, infected laparotomy wound dehiscence. We performed percutaneous screws insertion on both pubic rami and IS screw on S1 and S3 to both cases. Functional outcome was evaluated using Majeed and Hannover pelvic score.ResultsAll patients survived and had good reduction with no residual displacement on SI joint. The former case at 21-month follow up presented with excellent outcome (100/100) by Majeed score and very good outcome (4/4) by Hannover score; while the latter case, at 18-month, present with good outcome (85/100) Majeed score and fair outcome (2/4) Hannover score.ConclusionsPercutaneous screw fixation at the level of S3 is feasible and can be inserted in S3 level by sacroiliac type and sacral type with minimal soft tissue intervention and good functional outcome.  相似文献

We present a case of a pseudoaneurysm of the superior gluteal artery following placement of an iliosacral screw in a patient with an unstable pelvic ring fracture. Percutaneous fixation of posterior pelvic ring injuries is becoming a popular method for pelvic stabilization. Several techniques are available t achieve stability and allow early mobilization in patients with displaced pelvic fractures. Percutaneous screw fixation of sacral fractures or sacroiliac joint disruptions should reduce operative and anaesthesia times as well as blood loss, while lowering the risk of surgical wound problems are decreased. The risks with this technique are generally iatrogenic, related with surgeon's lack of experience or imaging difficulties at the time of screw placement. The present case report describes an injury to the superior gluteal arterial system during percutaneous iliosacral screw insertion.  相似文献

目的 探讨选择骨盆后环损伤内固定的重建方法。方法 2000年5月~2005年6月,对40例骨盆后环骨折脱佗患者采用内同定重建手术,其中男28例,女12例。年龄21~58岁。致伤原因:车祸伤23例,压碰伤11例,坠落伤6例。骨盆后环损伤情况:骶髂关节脱位22例,骶骨骨折伴脱位12侧,髂翼侧骨折伴脱位6例。按Denis等(1988)分类标准,骶骨Ⅰ区骨折6例,Ⅱ区骨折3例,Ⅲ区骨折3例。其中合并骨盆前环骨折:耻骨联合分离14例,单侧耻骨上、下支骨折10例,双侧耻骨上、下支骨折8例,同侧髋臼骨折4例,同侧髋臼骨折合并对侧耻骨卜下支骨折3例,对侧髋臼骨折1例。丁伤后24h~15d手术。分别采用骶髂关节前路钢板内固定28例,骶髂关节后路螺钉内固定2例,采用Galveston技术联合ISOLA系统内同定10例。结果40例均获随访6个月~3年。术前合并骶丛神经损伤3例,术后1例遗留会阴部麻木和尿失禁,1例遗留跛行;后尿道断裂3例,膀胱破裂2例,经手术修补后功能完全恢复;其余患者均恢复满意。结论 对于骨盆后环骨折脱位,应根据骨盆后环骨折的类型,掌握内固定技术的适用范围,选择合适的内固定重建方法。  相似文献

Purpose

This study aimed at comparing the risk of breakage of lengthened sacroiliac screw and ordinary sacroiliac screw for the treatment of bilateral vertical sacral fractures to provide reference for clinical application.

Methods

A finite element model of type C pelvic ring injury (bilateral type Denis II fracture of sacrum) was produced. The bilateral sacral fractures were fixed with lengthened sacroiliac screw and ordinary sacroiliac screw in seven types of models, respectively. The maximal Von Mises stresses and stress distribution of the two kinds of screws in the case of standing on both feet were measured and compared.

Results

(1) Whether in lengthened sacroiliac screw or ordinary sacroiliac screw, the maximal Von Mises stress of screw fixation only in S1 segment is the largest, and the maximal Von Mises stress of screw fixation only in S2 segment is minor, and the maximal Von Mises stress of screw fixation in S1 and S2 segments, respectively, is the least. (2) When S1 and S2 were both fixed with ordinary screws, the maximal Von Mises stress of screw in S1 segment is larger than that of S2. When S1 and S2 were both fixed with lengthened screws, the maximal Von Mises stress of screw in S1 segment is similar to that of S2. (3) The maximal Von Mises stresses of bilateral symmetrical screws are similar. (4) When only S1 was fixed, the maximal Von Mises stress of lengthened screw is less than that of ordinary screw. When only S2 was fixed, the maximal Von Mises stress of lengthened screw is larger than that of ordinary screw. When S1 and S2 were both fixed, the maximal Von Mises stress of lengthened screw is slightly less than that of ordinary screw. (5) Whether in lengthened screw or ordinary screw, the stress concentrations all exhibited at the regions of screws corresponding to the sacral fracture regions and the part between sacral bilateral fracture lines. Compared with ordinary screw, the stress distribution in lengthened screw is more homogeneous. Whether in lengthened screw or ordinary screw, the stress distribution of only one sacral segment fixation is more concentrated than that of two sacral segments’ fixation. When S1 and S2 were both fixed, the stress distribution of upper screw is more concentrated and that of lower screw is more homogeneous.

Conclusion

In a finite elements simulated type C pelvic ring disruption (bilateral type Denis II sacral fracture), the breakage risk of screws fixed in double-segment bilaterally symmetrically is low, and the breakage risk of screws fixed in S2 segment is lower than that of S1 segment. The bilaterally symmetrical screw fixation in double-segment is strongly recommended to reduce the breakage risk of screws. In addition, the breakage risk of lengthened screws is lower than that of ordinary screws except when screws are fixed in only S2 segment, which merits attention.  相似文献

We propose a new technique for sacroiliac fixation for the treatment of pelvic fracture with vertical and horizontal instability (Tile class C). This fixation technique allows control of vertical displacement while allowing, if needed, a certain degree of movement in the horizontal plane to facilitate reduction of anterior lesions. The technique involves insertion of two sacral screws, one in S1 and one in S2, and two iliac screws. The iliac screws are inserted in the posterior iliac crest through two sacroiliac connectors placed on a rod linking the two sacral screws. Vertical displacement is controlled by blocking the screw heads on the connecting rod. If needed, a certain degree of horizontal mobility of the half pelvis can be allowed by loosening the connectors on the rods. This technique was used for 4 patients. Anatomic reduction was achieved and no secondary movement of the osteosynthesis material nor secondary displacement were observed. The quality of the fixation allowed rapid weight bearing in the standing position and early walking without crutches. This type of fixation can only be used for type C12 fractures in the Tile classification.  相似文献

目的 探讨仰卧位经皮骶髂置钉固定术治疗骨盆后环损伤的可行性、手术方法及疗效.方法 在10具尸体操作的基础上,2004年10月至2007年10月对14例骨盆后环损伤行仰卧位经皮骶髂置钉固定术.男7例,女7例;年龄28~75岁,平均41.6岁.Tile B型损伤4例,C型损伤10例.患者仰卧位,于"C"型臂X线机透视下以髂前上棘上2 cm与腋后线交点及髂前上棘与髂后上棘连线中、后1/3交点为进钉点,若两点距离较大,则在透视下确定最佳进钉点.进钉角度为向前20°~30°角,向尾端倾斜5°~15°.经皮骶髂置入一枚直径7.2 mm空心钛螺钉固定.术后摄骨盆正位、骶骨侧位X线片,并行骶髂关节CT扫描,观察螺钉在S1椎体的位置.结果 14例均获随访,平均随访16个月.术后3个月骨盆骨折均临床愈合.14例共置入15枚空心钛螺钉,未发生与置钉有关的并发症.随访期间无神经损伤、螺钉松动及断裂现象,无骨盆畸形及骶髂部疼痛.Majeed疗效评定标准优良率为92.9%.结论 采用体表双定位法,可提高仰卧位骶髂置钉固定术的安全性;仰卧位经皮骶髂置钉固定术治疗骨盆后环损伤方便、可行.  相似文献

目的 :探讨微创经皮骨盆前环耻骨支螺钉固定在Tile B型骨折中的应用及疗效。方法 :回顾性分析自2010年5月至2015年8月收治的骨盆后环损伤伴前环耻骨支、坐骨支骨折患者56例,其中男31例,女25例;年龄35~65岁,平均36.8岁。按Tile分型:B1型13例,B2型28例,B3型15例。26例采用骶髂螺钉联合外固定架治疗(外固定架组),30例采用骶髂螺钉联合前柱螺钉治疗(耻骨支螺钉组),从术中出血量、术后并发症、术后下地时间、骨折愈合情况、Majeed骨盆功能评分和视觉模拟评分(visual aualogue scale,VAS)等方面进行对比分析。结果:54例获得随访,时间3~24个月,平均12个月,两组术中出血量比较差异无统计学意义(P0.05),耻骨支螺钉组术后下床时间、骨折愈合时间明显短于外固定架组,差异有统计学意义(P0.05)。耻骨支螺钉组术后平均Majeed及VAS评分明显高于外固定架组,差异有统计学意义(P0.05)。耻骨支螺钉组并发症发生率明显低于外固定架组,差异有统计学意义(P0.05)。结论 :骶髂螺钉联合经皮耻骨支螺钉在治疗骨盆后环损伤伴前环耻骨支、坐骨支骨折的Tile B型骨盆骨折中具有术后下地时间早、疼痛减轻、并发症少等优点,是一种有效安全的治疗方法,能提高术后肢体功能,且有效减少术后并发症的发生。  相似文献

目的 探讨手术内固定治疗骨盆后环损伤的临床疗效.方法 对22例骨盆后环损伤患者分别采用微创技术椎弓根螺钉固定、骶髂关节空心螺钉固定及骨盆重建带后方髂髂固定3种方式治疗.结果 22例均随访,时间4~22个月.无切口感染、血管神经损伤及内固定松动或断裂,无骨折不愈合.结论 微创椎弓根螺钉固定、骶髂关节空心螺钉固定及骨盆重建带后方髂髂固定3种方式均为治疗骨盆后环损伤的有效方法,根据骨折类型及患者的情况选择不同的内固定方式,可获满意疗效.  相似文献

目的 :探讨骨盆前、后环微创内固定在骨盆C型骨折治疗中的可行性、技术要点以及临床效果。方法:自2010年12月至2015年12月,选择经髂腹股沟微创小切口重建接骨板内固定治疗骨盆前环损伤;经皮骶髂关节螺钉内固定治疗骨盆后环损伤患者18例,男11例,女7例;年龄29~68岁,平均43.6岁。骨折按Tile分型:C1型14例,C2型3例,C3型1例。耻骨骨折合并同侧骶骨骨折12例,耻骨骨折合并同侧骶髂关节脱位2例,双侧耻骨骨折合并单侧骶骨骨折伴耻骨联合分离3例,双侧耻骨骨折合并双侧骶髂关节骨折脱位1例。观察结果包括手术时间、手术出血量、腰骶神经及髂血管损伤情况、骨折复位情况等。结果:所有患者伤口Ⅰ期愈合,无感染、深静脉血栓、腰骶神经及髂血管损伤、异位骨化等并发症发生。根据Matta复位标准,优14例,良3例,可1例。16例患者获得随访,时间6~33个月,平均16.7个月。根据Majeed疗效评价标准,优15例,良1例,Majeed评分92.13±5.44。结论:骨盆前环损伤选择经髂腹股沟微创小切口重建接骨板内固定,骨盆后环损伤选择经皮骶髂关节螺钉内固定治疗骨盆C型骨折具有手术时间短、创伤小、出血少等优点,临床操作安全可行,疗效满意。  相似文献

背景:机器人导航定位系统GD-2000是国内自主研发的基于术中X线透视影像的双平面骨科机器人系统,可用于辅助完成经皮股骨颈空心螺钉和骶髂螺钉置入的内固定术。目的:探讨机器人导航定位系统辅助完成经皮骶髂螺钉内固定术的安全性及有效性。方法:回顾性分析2016年6月至2018年8月收治的21例骨盆骨折后环不稳定型损伤患者,均采用机器人导航定位系统辅助经皮骶髂螺钉内固定术,男16例,女5例;年龄27~52岁,平均(38.2±6.9)岁。按Tile分型,B1型7例,B2型6例,B3型5例,C1型3例。记录每枚螺钉的置入时间、X线透视时间、钻孔次数,术后CT检查评价螺钉置入位置,统计术后并发症发生情况,术后6个月随访进行Majeed功能评分。结果:21例患者共置入27枚螺钉,全部患者均一次钻孔完成螺钉置入,每枚螺钉置入时间为18~56 min,平均(38.6±9.5)min;X线透视时间为12~21 s,平均(16.5±3.5)s。所有螺钉位置满意,无螺钉切出骨皮质、误入骶孔或骶管,未出现医源性血管神经损伤及切口感染。术后6个月随访的Majeed评分,优14例,良6例,可1例,优良率为95.2%。结论:应用机器人导航定位系统辅助经皮骶髂螺钉内固定术具有定位准确、稳定性好、手术时间短、创伤小等优点,降低了手术风险,减少医护人员和患者的放射性损害,是一种治疗骨盆后环损伤安全而有效的技术选择。  相似文献

目的:比较5种不同固定方法治疗不稳定骨盆骨折中前环损伤的生物力学稳定性,为临床治疗提供参考。方法:使用三维有限元方法,建立一侧骶髂关节脱位合并耻骨支骨折的不稳定骨盆骨折模型(Tile C型),模拟前方采用5种不同的固定方法,后方统一采用骶髂螺钉进行固定,并在模拟站立状态下比较分析不同组合固定方法治疗后的骨盆环的von Mises应力及应变分布情况。结果:竖直方向500 N载荷加载后,前方骨折处最大应力3.56 MPa(前方外固定架组),骶髂关节和骨折处总位移和Y轴上垂直位移在应力下均未超过1.5 mm。其中前方经皮入路组和前方外固定架组在内固定、骨折前方、骶髂关节处的最大应力明显大于改良Stoppa入路组、传统的髂腹股沟入路组、空心螺钉组,且在骶髂关节和骨折处的总位移和Y轴上垂直位移也大于其他3组。结论:不稳定性骨盆骨折中的前环损伤在5种组合方法植入物的固定后均能得到明显的改善,但采用改良Stoppa入路、髂腹股沟入路、前方空心钉固定方法治疗前环损伤在生物力学总体性能要优于前方经皮入路和前方外固定架治疗的方法。  相似文献

Pelvic ring fracture presents with a wide spectrum of clinical status and fracture type that requires multimodal treatment strategy. We report our experience in the treatment of 224 pelvic ring fractures in terms of clinical and radiological findings, mode of treatment, surgical data, and functional outcome at final follow-up. The study subjects were 140 men and 84 women (mean age 58 years, range 8–94). Surgery was conducted in 63 patients, while 161 were treated conservatively. The average follow-up period was 7.1 years (range 1–15). AO-Orthopaedic Trauma Association classification of fracture type, Injury Severity score rating, Rommens and Hessmann’s ambulatory and pain assessment, and radiographic studies were conducted. Conservative treatment was provided in 161 (73%) patients, including simple bed rest with definitive external fixation, skeletal traction, and/or pelvic sling. Of these, 148 patients achieved excellent/good results, but the remaining 13 cases complained of severe pain at the sacroiliac joints caused by malunion or fibrous union of the joint. All 63 (27%) patients who were treated surgically, using most frequently a combination of anterior extraperitoneal and Pfannenstiel approaches, showed excellent/good clinical and radiographic results. Treatment of unstable pelvic ring fracture should be urgent and based on biomechanical and anatomic reconstructive strategy, paying utmost care to associated injuries. A combination of open stabilization of posterior sacroiliac area and anterior fixation of pubic rami and symphysis pubis is recommended for unstable anteroposterior compression, lateral compression, vertical instability, and unilateral or bilateral posterior injuries to the pelvic ring.  相似文献
