The aim of this project was to evaluate and compare the radiographic quality and sealability of root fillings in extracted human teeth using lateral condensation of gutta-percha or the Trifecta obturating system. One hundred freshly extracted human mature, single-rooted teeth were divided into four identical groups of 25 teeth on the basis of root canal shape. The root canals of two groups were prepared in such a way as to produce a relatively parallel canal shape with little or no flare towards the coronal orifice. The root canals of the other two groups were prepared in such a way as to produce a canal shape that was deliberately more flared to ensure that they were wider at the orifice than at the endpoint of the preparation. All root canals were flushed with 17% EDTA solution and 2.5% NaOCl to remove the dentinal smear layer. The canals of one flared and one parallel group were obturated using cold lateral condensation and the canals of the other two groups were obturated using the Trifecta system. The sealability of each technique was assessed by a dye penetration method. The radiographic quality of obturation was determined for each canal using a 4-point scale. Canals filled with thermoplasticised gutta-percha had significantly less apical dye penetration than those obturated by lateral condensation (P < 0.001). Lateral condensation achieved significantly better scores for radiographic quality than thermoplasticised gutta-percha from both the bucco-lingual (P < 0.001) and the mesio-distal views (P < 0.001). Root canals obturated using the Trifecta technique showed significantly more apical extrusion of sealer than those obturated by lateral condensation (P < 0.001). Under laboratory conditions the Trifecta technique had better sealability but poorer radiographic quality than lateral condensation.  相似文献

METHODOLOGY: A total of 108 freshly extracted human, mature single-rooted teeth were divided into two identical groups of 54 teeth on the basis of root canal shape. The canals were prepared to a minimum 0.055 taper and enlarged to size 35 at the apex. All root canals were flushed with 17% EDTA solution and 2.5% NaOCl to remove the dentinal smear layer. The canals of one group were obturated using cold lateral condensation of gutta-percha and the canals of the other group were filled using a warm multiphase gutta-percha obturation technique. The extrusion of sealer and/or gutta-percha through the apex of the teeth was recorded using a simple yes/no scheme. The sealability of each technique was assessed by a dye penetration method. The radiographic quality of obturation was determined for each canal using a four-point scale. RESULTS: Root canals filled by multiphase obturation had significantly more extrusion of sealer (P < 0.001) and gutta-percha (P < 0.001) than canals filled by lateral condensation. Canals filled by multiphase gutta-percha obturation had significantly less apical dye leakage than those obturated by lateral condensation (P < 0.05). Lateral condensation achieved significantly better scores for radiographic quality than multiphase obturation from the bucco-lingual view (P < 0.01). CONCLUSIONS: Under laboratory conditions multiphase gutta-percha had better sealability but poorer radiographic quality than lateral condensation.  相似文献

OBJECTIVE: To evaluate and compare the sealability and radiographic quality of root fillings in extracted teeth by using lateral condensation of gutta-percha or alpha-phase gutta-percha in conjunction with a single gutta-percha cone. STUDY DESIGN: A total of 108 single-rooted teeth were divided into 2 groups of 54 teeth based on canal shape. Canals were prepared to a minimum 0.055 taper and enlarged to a size 35 at the working distance. Irrigation was done with 17% EDTA and 2.5% sodium hypochlorite (NaOCl) to remove smear layer. One group was obturated by using cold lateral condensation of gutta-percha; the other group was obturated with thermomechanically compacted alpha-phase gutta-percha and a single cone of gutta-percha. Apical extrusion of sealer or gutta-percha was recorded. Sealability of each technique was assessed by dye penetration. The radiographic quality of obturation was also determined. RESULTS: Root canals filled with alpha-phase gutta-percha and a single cone had significantly more extrusion of sealer than canals filled by lateral condensation. Lateral condensation achieved significantly better scores for radiographic quality. There was no significant difference between the 2 filling techniques in terms of apical or coronal leakage. CONCLUSIONS: Thermomechanically condensed alpha-phase gutta-percha used in conjunction with a single gutta-percha cone had poorer radiographic quality than laterally condensed gutta-percha.  相似文献

The aim of this in vitro study was to determine the effect of removal of the smear layer on canal obturation as measured by penetration of bacteria from a coronal direction. One hundred and twenty extracted human teeth with straight, single root canals were decoronated. The canals were prepared using the modified double-flared technique with balanced force under copious irrigation. The apical matrix was prepared to size 40 and apical patency subsequently confirmed with a size 15 file. The teeth were divided randomly into experimental groups (80 teeth) and control groups (40 teeth). The root canals of 40 experimental and 20 control teeth were rinsed with 40% citric acid and 2% NaOCl to remove the smear layer before obturation. In experimental groups, 20 teeth with smear layer intact and 20 teeth with smear layer removed were obturated with lateral condensation of cold gutta-percha and Apexit sealer. A further 20 teeth with smear layer intact and 20 teeth with smear layer removed were obturated with the Trifecta technique with the same sealer. In control groups, 10 teeth with smear layer intact and 10 teeth with smear layer removed were obturated with lateral condensation of cold gutta-percha and Apexit sealer. These teeth were completely sealed both coronally and apically to serve as negative controls. The remaining 20 teeth with either smear layer intact or smear layer removed were not obturated and served as the positive controls. The root surface of each tooth was sealed with nail varnish. The cut end of a polypropylene tube was sealed around the coronal part of each root canal so that bacteria placed therein could move only through the obturated canal space. Each root was placed in a glass bottle containing sterile Todd-Hewitt Broth (THB) and aliquots of 0.5 ml of THB were injected into the polypropylene tube. The model system was centrifuged at 168 g. An innoculum of Streptococcus sanguis in THB was placed in each coronal chamber at 5-day intervals and daily observations were made for bacterial growth in the apical reservoir for 90 days. All positive control teeth showed bacterial penetration within 24 h, while the negative control teeth remained uncon-taminated throughout the test period. Leakage through the experimental teeth was variable ranging from 7 to 86 days. There was no statistical significant difference (P > 0.05) in leakage between the obturated canal when the smear layer was either removed or intact.  相似文献

Canals filled with thermoplasticized gutta-percha had significantly less apical dye penetration than those obturated by lateral condensation (p < 0.001). Lateral condensation achieved significantly better scores for radiographic quality than thermoplasticized gutta-percha from both the buccolingual (p < 0.005) and mesiodistal views (p < 0.001). Low-temperature thermoplasticized gutta-percha root fillings were associated with significantly more apical extrusion of sealer (p < 0.001) and gutta-percha (p < 0.005). Under laboratory conditions the low-temperature thermoplasticized gutta-percha had better sealability but poorer radiographic quality than lateral condensation.  相似文献

Adaptation and placement of alpha-phase gutta-percha delivered with a plastic core-carrier, Thermafil, was compared to the lateral condensation of gutta-percha in a specific tooth model. Fifty-one mandibular molar roots with separate canals, patent canal orifices and curvatures greater than 15 degrees were cleaned and shaped with K-files and 2.5% sodium hypochlorite to a size 30 at the apex, and flared with Hedstrom flies to create a continuously tapering funnel preparation. Canals were randomly obturated with Sealapex root canal sealer and either alpha-phase gutta-percha on a plastic Thermafil carrier, or standard beta-phase gutta-percha with lateral condensation. Roots were radiographed from the proximal and evaluated by three examiners, based on established criteria for overall material adaptation, apical adaptation, and filling material extrusion. Thermafil provided a statistically significant better over-alt canal obturation (P < 0.001), while, in the apical third, both techniques were not significantly different (P > 0.05). When the apical orifice was patent there was a significant propensity for the extrusion of filling materials beyond the apex (P < 0.001) with the Thermafil technique.  相似文献

The aim of this in vitro study was to assess the quality of root canal obturation and degree of apical dye penetration in teeth root-filled with either laterally condensed cold gutta-percha or Thermafil obturators. A total of 144 extracted teeth with single roots were included in the study. All canals were prepared with a standard technique using a sonic handpiece with copious irrigation. Radiographs taken of the teeth to show the maximum degree of canal curvature were then exposed and the angle, radius and position of curvature determined. This information about the canals, together with their working length and diameter at end-point was used to divide the teeth into two experimental and two control groups. A total of 71 teeth were filled with Thermafil obturators, 69 with lateral condensation while four teeth remained unfilled and acted as controls. Following obturation, the teeth were radiographed in two planes and the technical quality of fill assessed on a four-point scale. All access cavities were then sealed and the teeth immersed in dye for 48 h before being split longitudinally. Linear dye penetration via the apical foramen was then assessed. In both straight and curved canals obturation with Thermafil obturators was significantly quicker (P< 0.001) than lateral condensation. Apical extrusion of gutta-percha occurred significantly more often in straight canals obturated with lateral condensation (P< 0.005) but there was no difference in curved canals. There was little difference in the radiographic quality of fill in the apical halves of the root fillings but Thermafil obturators were significantly better in the coronal halves when viewed from the proximal. Apical leakage of dye was significantly worse (P< 0.05) in curved canals filled with lateral condensation with a similar, but not significant, trend in straight canals. Under the conditions of this study, Thermafil obturators proved a satisfactor alternatie to lateral condensation of gutta-perch.  相似文献

The apical seal produced by low-temperature injection of gutta-percha, with or without root canal sealer, in the presence or absence of the smear layer was evaluated quantitatively using an electrochemical technique. The root canals of 44 teeth were prepared with the smear layer intact following a final flush of NaOCl (group 1), while the root canals of a further 44 teeth were prepared and the smear layer removed with a final flush with EDTA followed by NaOCl (group 2). Subgroups were obturated using Ultrafil System alone or accompanied by Calciobiotic Root Canal Sealer. Microleakage data was expressed as a percentage of apical leakage evident in each group. Statistical analysis of the results revealed that the groups obturated with Ultrafil alone showed no significant differences in leakage compared with the groups where sealer was used, either when the smear layer had been left intact or removed. (P>0.05). However, comparison of the combined two groups with smear layer present versus the combined two groups with smear layer removed showed a highly significant difference (P<0.01), with the incidence of leakage reduced in the absence of the smear layer.  相似文献

A total of eight teeth were excluded because of technical difficulties, 65 were filled with Thermafil obturators, 63 were filled with laterally condensed gutta-percha whilst 14 teeth remained unfilled and acted as controls. Following obturation, the teeth were radiographed in two planes and the technical quality of fill assessed on a four-point scale. All access cavities were then sealed and the teeth immersed in dye for 48 h before being split longitudinally. Linear dye penetration via the apical foramen was then assessed. Canal obturation with Thermafil obturators (0.7 min) was significantly quicker (P > 0.001) than lateral condensation (6.4 min). Apical extrusion of sealer and gutta-percha occurred significantly more often with Thermafil obturators but there was little difference in the technical quality of the fillings and no significant difference in dye penetration. Under the conditions of this study, Thermafil obturators proved a satisfactory alternative to lateral condensation of gutta-percha.  相似文献

目的:研究根管治疗中制备根尖止点对阻止根管充填材料超充的影响。方法:选取80个单根管上、下颌尖牙,其中40个牙采用逐步后退技术,常规制备根尖止点进行根管预备至根尖孔上1mm,另40个牙亦采用逐步后退技术预备至根尖孔,不进行根尖止点的制备;再将以上各组牙随机平分为两组,分别采用冷牙胶侧方加压和热牙胶垂直加压方法充填根管,观察并记录每组中牙胶超出根尖的牙数,收集每个牙超出的充填材料(牙胶和糊剂)并称量,对实验结果进行统计学分析。结果:制备根尖止点的侧方加压组与不制备根尖止点的侧方加压组之间根充材料超出的量、制备根尖止点的垂直加压组与不制备根尖止点的垂直加压组之间根充材料超出的量均无统计学差异(P>0.05)。结论:通过在初尖锉的基础上扩大3#的方法制备根尖止点并不一定能起到减少根充材料超充的作用。  相似文献

This study evaluated the sectional gutta-percha obturation technique advocated by Lightspeed Technology, Inc. Single relatively straight-canaled teeth were prepared with rotary instruments and divided into three groups. Groups A and B served as controls and were obturated using laterally condensed gutta-percha and either Roth's 801 or Ketac-Endo sealer. Group C was obturated using the Lightspeed technique that included placement of a 5-mm apical section of gutta-percha, followed by backfill with Ketac-Endo sealer and a single gutta-percha cone. All teeth were suspended in India ink for 14 days then cleared. Four additional teeth, which were obturated as in group C then sectioned, revealed a tightly adapted apical section of gutta-percha with a very thin layer of sealer. However, voids were noted in the middle and coronal areas. There was no significant difference in apical microleakage among the three groups. The sectional method was significantly faster than lateral condensation and seems to offer promise as an effective obturation method.  相似文献

Comparison of the obturation of lateral canals by six techniques.   总被引:11,自引:0,他引:11  
The following obturation techniques were compared on their ability to obturate lateral canals in vitro, lateral condensation (LC), continuous wave of condensation (CW), warm vertical condensation (WV), carrier-based thermoplasticized gutta-percha (CB), warm lateral condensation (WL), and vertically condensed high-temperature gutta-percha (HT). A root canal system with lateral canals in the coronal, middle, and apical thirds was prepared in resin blocks. Each block was obturated using each technique (n = 30, 15 each, with and without sealer). The length of gutta-percha and sealer in each of the lateral canals was measured with a measuring microscope and statistically compared. All techniques obturated all three levels of lateral canals with sealer. WV, CB, and CW were able to fill the lateral canals with gutta-percha significantly better when root canal sealer was used. WV, CB, CW, and HT filled the coronal and middle lateral canals significantly better with gutta-percha than LC or WL condensation. CB and CW filled the apical lateral canal significantly better with gutta-percha than HT, WV, WL, or LC.  相似文献

AIM: The purpose of this study was to evaluate the effects of smear layer and canal instrumentation on leakage in root-filled teeth. METHODOLOGY: Six groups (n = 12) of freshly extracted human canines and premolars with closed apices and single roots were used. Groups A, B, C, and D were instrumented with engine-driven rotary nickel-titanium MCXIM files and Groups E and F were instrumented with conventional stainless steel hand files. Groups A, C, and E were flushed with 3.0 mL of 17.0% REDTA to remove the smear layer prior to obturation. All teeth were flushed with 5.25% NaOCl, then obturated with AH-26 sealer and either the lateral condensation (Groups C-E) or thermomechanical compaction technique (Groups A and B). Copper wire was placed coronally in contact with the gutta-percha in each tooth and, after immersion in 0.9% NaCl solution, a 10 volt dc voltage was connected between each tooth and a stainless steel electrode. The current flow in the circuit was observed for 45 days. One way ANOVA and Duncan's Multiple Range Test were used to compare Groups A-F at time intervals of 10, 20, 30 and 45 days and identify statistically significant differences. RESULTS: Significantly less microleakage occurred when the smear layer was removed and when the canals were obturated with thermoplasticized gutta-percha. Canals instrumented with engine-driven NiTi files exhibited less leakage than hand-instrumented canals irrespective of obturation method. CONCLUSIONS: Smear layer removal is beneficial to root canal sealing. Obturation with thermoplasticized gutta-percha provides a superior seal whilst canal instrumentation with engine-driven NiTi files reduces the extent of microleakage in root canals.  相似文献

The purpose of this study was to evaluate in vitro the apical seal of root canals prepared with a new rotary system, Anatomic Endodontic Technology (AET) and filled with a methacrylate based endodontic sealer and a single gutta-percha cone and a lateral condensation technique, using the methacrylate based endodontic sealer/filler or Grossman's cement. The root canals of 45 freshly extracted human maxillary anterior teeth were prepared and then randomly assigned to three groups of 15 teeth each. After cleaning and shaping the teeth were obturated as follows: in group 1 the canals were filled with a methacrylate based sealer and a single gutta-percha cone; in group 2 the canals were filled using a lateral condensation technique with gutta-percha and the methacrylate based sealer; and in group 3, the canals were filled by means of lateral condensation of gutta-percha and Grossman's cement (control group). The specimens were stored in 100% relative humidity at 37 degrees C for 72 h, after which the coronal portion and the root surface of each tooth was covered with three layers of nail varnish and a final layer of sticky wax. After immersion in 2% methylene blue dye for 7 days, the specimens were imbedded in clear orthodontic resin and sectioned. Dye penetration was evaluated by an independent investigator using a stereo microscope. The results demonstrated that more pronounced leakage occurred in root canals obturated with the lateral condensation technique and Grossman's cement (p < 0.05). The least amount of dye leakage was observed for group 1 and 2 in which the methacrylate based sealer was used, either with a single gutta-percha cone or with lateral condensation of gutta-percha.  相似文献

AIM: To compare the area occupied by gutta-percha, sealer, or void in standardized .06 tapered prepared simulated curved canals and in mesio-buccal canals of extracted maxillary first molars filled with a single .06 gutta-percha point and sealer or lateral condensation of multiple .02 gutta-percha points and sealer. METHODOLOGY: Simulated canals in resin blocks with either a 30 degrees curve and radius of 10.5 mm (n = 20) or a 58 degrees curve and 4.7 mm radius (n = 20) and curved mesio-buccal canals of extracted maxillary first molars (n = 20) were prepared using .06 ProFiles in a variable tip crown-down sequence to an apical size 35 at 0.5 mm from the canal terminus or apical foramen. Ten 30 degrees and 58 degrees curved resin canals and 10 canals in the extracted teeth group were obturated with .02 taper gutta-percha cones and AH 26 sealer using lateral condensation. The time required to obturate was recorded. The remaining canals were obturated with a single .06 taper gutta-percha cone and AH 26 sealer. Excess gutta-percha was removed from the specimens using heat and the warm mass vertically condensed. Horizontal sections were cut at 0.5, 1.5, 2.5, 4.5, 7.5 and 11.5 mm from the canal terminus or apical foramen. Colour photographs were taken using an Olympus 35 mm camera attached to a stereomicroscope set at x40 magnification, and then digitized using a flatbed scanner. The cross-sectional area of the canal contents was analysed using Adobe PhotoShop. The percentage of gutta-percha, sealer or voids to the total root canal area were derived and data analysed using unpaired Student's t-test and the Mann-Whitney U-test. RESULTS: In the 30 degrees curved canals the levels had between 94 and 100% of the area filled with gutta-percha with no significant difference (P > 0.05) between the lateral condensation and single cone techniques. In the 58 degrees curved canals the levels had 92-99% of the area filled with gutta-percha, with the single cone technique having significantly (P < 0.05) more gutta-percha fill at the 2.5 mm level only. In the mesio-buccal canals of the teeth the levels had between 72 and 96% of the area filled with gutta-percha with no significant difference (P > 0.05) between the lateral condensation and single cone technique. The time for obturation was significantly (P < 0.05) greater for lateral condensation compared with the single cone technique in all groups. CONCLUSIONS: The .06 taper single cone technique was comparable with lateral condensation in the amount of gutta-percha occupying a prepared .06 tapered canal. The .06 single cone technique was faster than lateral condensation.  相似文献

目的对比评价三种根管充填技术的根尖封闭能力。方法收集2011年10月至2012年5月中国医科大学附属口腔医院口腔颌面外科门诊因正畸或牙周病拔除的新鲜离体单根管牙34颗,根管清理后用机用ProTaDer镍钛锉经Crown—Down技术预备,然后随机分为A、B、C三组。A组(11颗)采用冷牙胶侧方加压充填技术充填根管,B组(10颗)采用超声侧方加压充填技术充填根管,c组(13颗)采用连续波热牙胶垂直充填技术充填根管。所有实验牙均使用AHPlus根充糊剂。根管充填后的各组实验牙采用染料渗透法检测根尖微渗漏的发生情况,体式显微镜下测量自根尖孔向冠方的染料渗透长度,采用SPSS11.5统计软件包对测量数据进行分析。结果A组与B、c组的根尖微渗漏数值间差异有统计学意义(P〈0.05),B组与c组的根尖微渗漏数值间差异无统计学意义(P〉0.05)。结论超声侧方加压充填技术和连续波热牙胶垂直充填技术的根管封闭能力优于冷牙胶侧方加压技术,而前两种充填技术的根管封闭能力无明显差别。  相似文献

The purpose of this study was to assess the seal obtained in straight and curved root canals filled with either laterally compacted gutta-percha or Thermafil obturators. Each technique was used in combination with three different sealers (RSA RoekoSeal, AH Plus, AH 26). Thermafil obturators were also used without sealer, resulting in a total of 14 test groups of 16 teeth each. Sixty teeth served as positive or negative controls. Included in the study were 142 extracted teeth with straight and 142 with curved root canals. All canals were enlarged up to size 40. After obturation, all roots were placed in India ink for 48 h and rendered transparent to measure the maximum linear dye penetration. Canals filled with Thermafil obturators had significantly more extrusion of filling material than canals filled by lateral compaction (p < 0.01). Thermafil without sealer showed significantly greater dye penetration compared with all other groups both in straight and in curved canals (p < 0.05). As long as a sealer was used, the seal obturated with Thermafil was equivalent in terms of dye penetration to lateral compaction. There were no statistical differences in the mean apical dye penetration among the three sealers. The differences between the dye penetration in straight and in curved canals were insignificant for all groups (p > 0.05). Under the conditions of this study, Thermafil obturators achieved seals comparable to lateral compaction, as long as a sealer was used.  相似文献

The purpose of this study was to evaluate the potential of using mineral trioxide aggregate as a root canal filling material by comparing its apical sealing ability with that of laterally condensed gutta-percha with sealer and high-temperature thermoplasticized gutta-percha with sealer in extracted bovine teeth. Sixty bovine incisors with single canals were prepared in a standard manner using LightSpeed instruments, randomly divided into three groups of 20 teeth, and obturated. The sealing ability of each technique was assessed by immersion in 1% methylene blue dye for 3 days. The teeth were cleared, and the linear extent of dye penetration was measured with a stereomicroscope. Data were analyzed by the Kruskal-Wallis one-way ANOVA followed by Dunn's test. Canals filled with laterally condensed gutta-percha or thermoplasticized gutta-percha showed significantly less apical dye penetration than canals obturated with mineral trioxide aggregate (p < 0.001). There was no significant difference in leakage between the laterally condensed group and the thermoplasticized group. The results suggest that gutta-percha obturation may provide an apical seal that is superior to MTA.  相似文献

Objective:

This study evaluated whether smear layer removal has any influence on the filling of the root canal system, by examining the obturation of lateral canals, secondary canals and apical deltas.

Material and Methods:

Eighty maxillary and mandibular canines were randomly divided into two groups, according to their irrigation regimen. Both groups were irrigated with 1% NaOCl during canal shaping, but only the teeth in Group II received a final irrigation with 17% EDTA for smear layer removal. The root canals were obturated with lateral condensation of gutta-percha and the specimens were cleared, allowing for observation under the microscope.

Results:

In Groups I and II, 42.5% and 37.5% of the teeth, respectively, presented at least one filled canal ramification. Although a larger number of obturated ramifications was found in Group I, there were no statistically significant differences between the two groups (p = 0.4957).

Conclusion:

Smear layer removal under the conditions tested in this study did not affect the obturation of root canal ramifications when lateral condensation of gutta-percha was the technique used for root canal filling.  相似文献
