| 摘 要： | 目的观察对比经腋窝入路免充气全腔镜甲状腺癌根治术和开放甲状腺癌根治术对cN0期甲状腺微小乳头状癌的治疗效果。方法选取60例cN0期甲状腺微小乳头状癌的临床病理资料，其中经腋窝入路免充气全腔镜甲状腺癌根治术30例（观察组），开放甲状腺癌根治术30例（对照组），对比2种术式的临床疗效。结果2组中央区淋巴结清扫数、中央区淋巴结转移阳性数、总并发症发生率差异无统计学意义（P>0.05）；观察组手术时间、引流液总量、住院时间均较对照组更多，术中出血量少于对照组，术后颈胸部疼痛评分、美容满意度优于对照组，差异均有统计学意义（P < 0.01）。结论相比开放甲状腺癌根治术，经腋窝入路免充气全腔镜甲状腺癌根治术治疗cN0期甲状腺微小乳头状癌在保证手术安全性及并发症总发生率的同时，可以降低术后疼痛，极大提高美容满意度。

| Abstract: | ObjectiveTo compare the curative effects between transaxillary approach non-inflatable total cavity endoscopy and open radical thyroidectomy in the treatment of stage cN0 thyroid papillary microcarcinoma.MethodsThe clinicopathological data of 60 cases with stage cN0 thyroid papillary microcarcinoma were retrospectively analyzed.Thirty cases were treated with transaxillary approach non-inflatable total cavity endoscopy radical thyroidectomy(observation group), and 30 cases were treated with transaxillary approach open radical thyroidectomy(control group).The clinical efficacy was compared between two groups.ResultsThe differences of the number of lymph node dissection and positive number of lymph node metastasis in central region, and incidence rate of total complications were not statistically significant(P>0.05).The operative time, total amount of drainage fluid and length of hospital stay in observation group were more than those in control group, the intraoperative blood loss in observation group was less than that in control group, and the neck and chest pain score and aesthetic satisfaction in observation group were better than those in control group(P < 0.01).ConclusionsCompared with open radical thyroidectomy, the transaxillary approach non-inflatable total cavity endoscopy radical thyroidectomy can guarantee the safety of surgery, reduce postoperative pain and greatly improve aesthetic satisfaction. |
