基于GBD大数据中国膝骨关节炎疾病负担现状与趋势分析

摘    要:目的 分析中国1990—2019年膝骨关节炎的疾病负担以及变化趋势,为我国膝骨关节炎的防控和治疗工作提供参考依据。方法 利用GBD2019年的数据库,采用标化发病率、标化患病率及标化DALYs率对我国膝骨关节炎的疾病负担以及变化趋势进行分析,并与美国和全球膝骨关节炎的疾病负担情况进行对比。结果 1990—2019年,中国膝骨关节炎新发病人数从368.39万人增至842.58万人,患病人数从4 257.08万人增至10 812.01万人,DALYs从136.52万人年增至346.48万人年,经过年龄标准化后的发病率、患病率、DALYs率呈上升趋势,增长率分别为6.65%、6.85%和7.18%(AAPC均为0.2%,t分别为6.9、6.1和6.7,P均<0.05)。年龄组方面,1990—2019年,45岁以上的人群的膝骨关节炎的患病率、DALYs率呈缓慢上升趋势,两项指标均在70岁以上年龄组达最高;而发病率的高峰在50~54岁年龄段。性别组方面,2019年的女性膝骨关节炎标化发病率、标化患病率、标化DALYs率分别为男性的1.64倍、1.72倍、1.70倍。另外,中国膝骨关节炎的标化DALYs率的平均水平高于美国和全球。结论 我国膝骨关节炎的疾病负担较重。应加强中老年人膝骨关节炎的预防,重点关注女性中老年人。

Abstract:Objective To analyze the disease burden of knee osteoarthritis and the trend of change in China from 1990 to 2019, to provide a reference basis for the prevention, control, and treatment of knee osteoarthritis in China. Methods Using the database of GBD 2019, the disease burden of knee osteoarthritis in China and the changing trends were analyzed using the standardized incidence rate, standardized prevalence rate and standardized disability adjusted of life years (DALYs) rate. The disease burden of knee osteoarthritis was compared with those in the United States and in the world. Results From 1990 to 2019, the number of new knee osteoarthritis cases in China increased from 3 683 900 to 8 425 800, the number of prevalent cases increased from 42 570 800 to 108 120 100, and the DALYs increased from 1 365 200 to 3 464 800. After age-standardized incidence, prevalence, and DALYs rates showed an increasing trend with growth rates of 6.65%, 6.85%, and 7.18%, respectively (all AAPC=0.2%, t=6.9, 6.1, and 6.7, respectively; all P<0.05). Regarding age groups, the prevalence of knee osteoarthritis and the rate of DALYs in people aged 45 years or older showed a slow increase from 1990 to 2019, with both indicators reaching their highest in the age group of 70 years or older; while the peak of incidence was in the age group of 50 to 54 years. For gender groups, the standardized incidence, standardized prevalence, and standardized DALYs rates of knee osteoarthritis in 2019 were 1.64 times, 1.72 times, and 1.70 times higher in women than in men, respectively. In addition, the average rate of standardized DALYs for knee osteoarthritis in China was higher than those in the United States and in the world. Conclusion The disease burden of knee osteoarthritis is high in China. Prevention of knee osteoarthritis in middle-aged and older adults should be enhanced, with a focus on female middle-aged and older adults.
