全球多发性骨髓瘤发病和死亡现状及其与人类发展指数的关系

摘    要:目的:探讨多发性骨髓瘤(multiple myeloma,MM)发病率和死亡率与人类发展指数(human development index,HDI)排名、健康投入/国民生产总值(current health expenditure/gross domestic product,e/GDP)的关系。方法:提取全球癌症数据库GLOBCAN2018中2018年MM各国家地区发病率及死亡率,2019人类发展指数报告中2018年HDI排名,以世界卫生统计2018中2015年e/GDP数据近似代替2018年情况,选取新发病人数排列前50位的国家地区,采用简单线性回归评估MM发病死亡率与HDI排名、e/GDP间的关系。结果:2018年全球有159 985和106 105例新发及死亡MM病人,位居所有癌症新发及死亡的第22和18位。MM发病率及死亡率与HDI呈正相关,其R2分别为0.501和0.147(P<0.05),类似的关系在与e/GDP间也存在,其R2分别为0.473和0.152(P<0.05)。死亡发病比与HDI排名及e/GDP间存在负相关,HDI排名及e/GDP越高,死亡发病比越小,其R2分别为0.624和0.512(P<0.05)。结论:各国家地区间MM流行病学特征的异质性明显,MM发病死亡率与社会经济状况的关系提示,需要改善获得医疗保健服务的机会,从而改善MM病人的生存状况。

Abstract:Objective:To explore the relationship between the incidence,mortality of multiple myeloma (MM) and the ranking of human development index (HDI),current health expenditure/growth domestic product (e/GDP).Methods:The incidence and mortality of MM in 2018 were extracted from the global cancer database GLOBOCAN2018,and the HDI in 2018 was ranked in the human development index report 2019.The data of e/GDP in 2015 extracted from World Health Statistics 2018 was approximately substituted for it in 2018.The top 50 countries and regions were selected to evaluate the relationship between incidence,mortality,mortality-incidence ratio (MIR) and HDI,e/GDP by simple linear regression analysis.Results:In 2018,there were 159 985 and 106 105 new onsets and deaths of MM patients worldwide,ranking 22nd and 18th in all cancers.Incidence and mortality were positively correlated with HDI,with R2 values were 0.501 and 0.147 (P<0.05),respectively.Similar relationship also existed between them and e/GDP,with R2 values were 0.473 and 0.152 (P<0.05),respectively.There was a negative correlation between MIR and HDI,e/GDP.The higher HDI ranking and e/GDP was,the smaller MIR was,with R2 values were 0.624 and 0.512 respectively (P<0.05).Conclusion:The heterogeneity of MM epidemiological characteristics among countries and regions is obvious.The relationship between morbidity and mortality of MM and socio-economic status suggests that the access to health care services needs to be improved,so as to improve the survival status of MM patients.
