摘    要:目的 分析体重指数、腰围与高龄老年人死亡风险的关联,为完善高龄老人的体重管理提供科学依据。方法 采用中国老年人健康长寿影响因素调查(Chinese Longitudinal Healthy Longevity Survey, CLHLS)2011—2018队列数据库4 508例样本,暴露变量为体重指数(body mass index, BMI)和腰围(waist circumference, WC),结局为死亡。采用Cox比例风险回归模型分析体重指数、腰围和高龄老年人死亡风险的关联。结果 中位随访时间为3.08年,死亡3 416人。按照BMI分类,体重过轻对高龄老年人死亡风险最大(HR=1.31,95%CI:1.19~1.43),超重是老年人死亡风险的保护因素(HR=0.85,95%CI:0.74~0.98)。按照WC分类,中心型肥胖是老年人死亡风险保护因素(HR=0.84,95%CI:0.76~0.93)。BMI和WC构建的综合体重表型结果显示,BMI超重合并腰围肥胖是老年人死亡风险的保护因素(HR=0.82,95%CI:0.69~0.97),而BMI过轻合并腰围肥胖是老年人死亡风险...

Abstract:Objective To analyze the relationship of body mass index (BMI) and waist circumference (WC) with the risk of death in the elderly, and to provide scientific basis for improving the weight management of the elderly with advanced age. Methods A total of 4 508 samples from the Chinese Longitudinal Healthy Longevity Survey (CLHLS) 2011 to 2018 were retrieved. The exposure variables were BMI and WC and the outcome was death. Cox proportional hazard regression model was used to analyze the relationship of BMI and WC with the risk of death in the elderly. Results The median follow-up period was 3.08 years, and 3 416 deaths were reported. According to BMI classification, underweight was associated with the greatest risk of death for the elderly (HR=1.31, 95%CI: 1.19-1.43), and overweight was a protective factor for death risk in the elderly (HR=0.85, 95%CI: 0.74-0.98). According to WC classification, central obesity was a protective factor for death risk in the elderly (HR=0.84, 95%CI: 0.76-0.93). The results of comprehensive weight phenotype constructed by BMI and WC showed that overweight in BMI with obesity in waistline was a protective factor for death risk in the elderly (HR=0.82, 95%CI: 0.69-0.97), while low BMI combined with obesity in waistline was a risk factor for death in the elderly (HR=1.45, 95%CI: 1.12-1.88). Conclusion Among the elderly with advanced age in China, underweight is associated with increased risk of death, while overweight and obesity are associated with reduced risk of death. Interventions to reduce malnutrition should prioritize the elderly ith advanced age.
