智能脉冲技术辅助的经上皮准分子激光角膜切削术(Trans-PRK)矫正中低度近视

摘    要:目的 研究智能脉冲技术(smart pulse technology,SPT)辅助的经上皮准分子激光角膜切削术(transepithelial photorefractive keratectomy,Trans-PRK)矫正中低度近视的效果。方法 回顾性分析2017年6月至11月在我院使用1050 Hz切削频率和SPT辅助的Trans-PRK手术的中低度近视患者39例(75眼)的临床资料,应用Sirius眼前节分析仪测量6 mm瞳孔直径下角膜波前像差。随访6个月,手术前后数据采用重复测量方差分析检验。结果 术后6个月的最佳矫正视力与术前比较,无一例下降超过一行,46.67%(35/75)的患眼无变化,37.33%(28/75) 的患眼提高一行,16.00%(12/75) 的患眼提高两行。术后6个月的裸眼视力,100.00%(75/75) 的患眼达到20/20或更好,88.00%(66/75) 的患眼达到20/16或更好,60.00%(45/75) 的患眼达到20/13或更好,8.00%(6/75) 的患眼达到20/10。术后6个月,6 mm瞳孔直径下角膜球差、高阶像差、彗差分别为(0.25±0.14)μm、(0.45±0.15)μm和(0.23±0.11)μm,均较术前(0.19±0.07)μm、(0.33±0.07)μm和(0.19±0.09)μm增加(均为P<0.05)。结论 SPT辅助的Trans-PRK矫正中低度近视是安全、有效的,术后角膜高阶像差、球差和彗差均有一定程度增加。

Abstract:Objective To investigate the effect of transepithelial photorefractive keratectomy (Trans-PRK) in the treatment of moderate to low myopia with smart pulse technology (SPT).Methods Retrospective analysis of clinical data from 39 patients (75 eyes) with moderate to low myopia in 1050 Hz cutting frequency and SPT-assisted Trans-PRK surgery from June to November 2017 was performed.The corneal wavefront aberration at 6 mm pupil diameter was measured using a Sirius anterior segment analyzer.After 6 months of follow-up,data before and after surgery was tested by repeated measures analysis of variance.Results The best corrected visual acuity at 6 months after surgery was compared with that before surgery,and none of them lost more than one line,46.67%(35/75) of the affected eyes did not change,37.33%(28/75) increased by one line,16.00%(12/75) increased by two lines.As for 6-month postoperative uncorrected visual acuity,100.00% (75/75) of the affected eye reached 20/20 or better,88.00% (66/75) reached 20/16 or better,60.00%(45/75) reached 20/13 or better,and 8.00%(6/75) reached 20/10.At 6 months postoperatively,the corneal spherical aberration,high-order aberration and coma aberration at the 6 mm pupil diameter were (0.25±0.14)μm,(0.45±0.15)μm,and (0.23±0.11)μm,which were increased compared with preoperative data [(0.19±0.07)μm,(0.33±0.07)μm and (0.19±0.09)μm] (all P<0.05).Conclusion SPT-assisted Trans-PRK in the correction of moderate to low myopia is safe and effective,and postoperative corneal high-order aberration,spherical aberration and coma aberration are increased to some extent.
