促甲状腺素抑制治疗甲状腺癌患者的膳食调查分析

摘    要:目的 对促甲状腺素(TSH) 抑制治疗期间的分化型甲状腺癌(DTC)患者进行膳食调查,为对患者进行个性化膳食指导及营养干预研究提供参考依据。方法 从湖南省肿瘤医院甲状腺内科的门诊患者中收集行TSH抑制治疗的DTC患者,根据抑制治疗副作用危险分层标准,收集低、中、高危患者各200例,共600例患者作为研究对象。对所有入选病例进行膳食调查:一年食物摄入频率和3天24小时回顾法膳食调查。结果 TSH抑制治疗的DTC患者整体人群膳食摄入不足且膳食结构不合理;整体人群畜肉类摄入过多,鱼类及水产品、蛋、奶、豆、蔬菜、水果则摄入过少;整体人群脂类、碳水化合物、维生素E摄入量基本达到要求;蛋白质、膳食纤维、钙、铁、锌、硒、维生素A、维生素B1、维生素B2、维生素C、烟酸摄入明显不足;碘摄入基本符合要求;低、中、高危组膳食摄入不足率分别是21.0%、8.0%、11.5%;中危组畜肉类摄入最多,碘摄入有0.5%的患者超标;高危组牛奶摄入最少。结论 TSH抑制治疗的DTC患者膳食结构不合理及多种营养素摄入不足,在临床工作中应加强对整体人群的营养教育及针对性的干预,以降低TSH抑制治疗对心血管系统及影响骨代谢的风险。

Abstract:Objective To survey of dietary of patients with differentiated thyroid cancer during thyrotropin (TSH) inhibition therapy, and to provide a reference for personalized dietary guidance and further nutritional intervention studies. WTHZ]Methods WTBZ]Patients with thyroid cancer who were treated with TSH inhibition were collected from the outpatients of the Department of Thyroid Medicine of Hunan Cancer Hospital. According to the stratification criteria for side-effect risk, 200 patients with low, medium and high risk were collected. A total of 600 patients were included in the study. The dietary survey includes all selected cases of one-year food intake frequency and three-day 24-hour retrospective dietary survey. WTHZ]Results WTBZ]DTC patients treated with TSH inhibition have insufficient dietary intake and unreasonable dietary structure. The overall population who has too much intake of meat, and fish and aquatic products, eggs, milk, beans, vegetables, and fruits are under-resourced. The intake of lipids, carbohydrates and vitamin E basically meets the requirements. The intake of protein, dietary fiber, calcium, iron, zinc, selenium, vitamin A, vitamin B1, vitamin B2, vitamin C and niacin is obviously insufficient. The basic dietary requirements were 21.0%, 8.0%, and 11.5% in the low, medium, and high-risk groups, respectively. In the middle-risk group, the meat intake was the highest, and the iodine intake was 0.5%, and the high-risk group was ingested least. Conclusions The dietary structure of DTC patients with TSH inhibition treatment is unreasonable and the intake of multiple nutrients is insufficient. In the clinical work, nutrition education and targeted intervention should be strengthened for the whole population to reduce the risk of TSH inhibition treatment on the cardiovascular system and affecting bone metabolism.
