2 型糖尿病患者血清sdLDL 和脂类水平及相关性分析

摘    要:目的 研究小而密低密度脂蛋白(sdLDL)在2 型糖尿病(T2DM)患者中的水平变化,sdLDL 与T2DM 及 脂类的相关性。方法 将543 例T2DM 患者及836 例健康体检人群纳入研究对象,比较T2DM 组与健康对照组血清 sdLDL 及其他血脂组分水平差异;分析不同年龄组T2DM 血清sdLDL 水平变化;分析sdLDL 与T2DM 的相关性。结 果 T2DM 组sdLDL 水平显著高于健康对照组,总胆固醇(TC)、三酰甘油(TG)和低密度脂蛋白胆固醇(LDL-C) 水平高于健康对照组,高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A1(ApoA1) 水平低于健康对照组,差异均有统计学 意义(Z=4.76, 3.25, 3.42, 4.11, 2.11, 2.05, 均P<0.05)。T2DM 组中,40~60 岁,60 岁以上sdLDL 水平高于20~40 岁, 差异有统计学意义(Z=2.31, 2.62, 均P<0.05),40~60 岁sdLDL水平与60 岁以上比较,差异无统计学意义( Z=1.25, P>0.05)。T2DM 组sdLDL 水平与TC,TG,LDL-C,载脂蛋白B(ApoB) 呈正相关(r=0.268,0.413,0.112,0.100, 均P<0.05)。sdLDL 水平与T2DM 的发生密切相关(OR=2.460,95%CI=1.809~3.346,P<0.05)。结论 T2DM 患者 sdLDL 水平显著升高,sdLDL 与T2DM 的发生密切相关,是T2DM 的危险因素。

Abstract:Objective To study the level changes of small and dense low density lipoprotein (sdLDL) in type 2 diabetes (T2DM) patients and the correlation between sdLDL and T2DM and other lipid components. Methods The study included 543 T2DM patients and 836 healthy people. The differences of serum sdLDL and other lipid components between T2DM group and healthy control group were compared. The changes of serum sdLDL levels in T2DM of different age groups were analyzed. The correlation between sdLDL and T2DM was analyzed. Results  The level of sdLDL in diabetic group was significantly higher than that in healthy group. The levels of TC,TG and LDL-C in T2DM group were higher than those in healthy group, while the levels of HDL-C and ApoA1 in T2DM group were lower than those in healthy group, the difference was statistically significallt (Z=4.76, 3.25, 3.42, 4.11, 2.11, 2.05, all P<0.05). The level of sdLDL in T2DM was positively correlated with TC, TG, LDL-C and ApoB(r=0.268, 0.413, 0.112, 0.100, P<0.05). The level of sdLDL in T2DM aged 40~60 years and over was higher than that in patients aged 20~40 years, the difference was statistically significant (Z=2.31, 2.62, all P<0.05). There was no significant difference between the level of sdLDL in patients aged 40~60 years and those aged over 60 years (Z=1.25, P>0.05). The level of sdLDL was closely related with T2DM (OR=2.460, 95%CI=1.809~3.346, P<0.05). Conclusion The level of sdLDL in T2DM increased significantly, and sdLDL was closely related with T2DM,and it is a risk factor for T2DM.
