| 目的 评价基因重组抗原ELISA法在梅毒螺旋体抗体检测中的意义。方法 用甲苯胺红不加热血清试验(TRUST)、梅毒螺旋体重组抗原酶联免疫吸附试验(ELISA)和梅毒螺旋体明胶凝集试验(TPPA)对335份梅毒患者和非梅毒者的临床血清标本进行检测。结果 以TPPA阳性者为标准，TRUST有6份为假阴性，4份为假阳性；ELISA假阳性1份，无假阴性。TRUST和ELISA的敏感性、特异性分别为94．8％、98．2％和100%、99．5％。TRUST与TPPA的符合率为97．0%，ELISA与TPPA的符合率为99．7％。ELISA的敏感性、特异性都优于TRUST，与TPPA符合率高。结论 重组抗原ELISA法检测梅毒螺旋体特异性抗体具有敏感性高、特异性好、结果客观、自动化程度高等优点，适用于梅毒初筛和确诊。 |

| Objective To evaluate an enzyme-linked immunosorbent assay (ELISA) technique based on recombinant antigens for detection of antibodies against Treponema pallidum. Methods 335 clinical serum specimens from persons with syphilis or without syphilis were detected by the Toluidine red unheated serum test(TRUST), the T. pallidum particle agglutination (TPPA) assay and the recombinant antigen-based ELISA, respectively. Results Using the TPPA as a standard method, 6 specimens were found false positive and 4 were false negative by the TRUST. However, only one sample was false positive and no sample was false negative by the ELISA. The overall specificity and sensitivity of the TRUST were 94.8% and 98.1%, respectively. The specificity and sensitivity of the ELISA were 100% and 99.5%, respectively. The agreement between the TRUST and TPPA was 97.0%. The agreement between the TRUST and TPPA was 99.7%. The ELISA was more sensitive and specific than the TRUST, with high concordance with the TPPA. Conclusion The high sensitivity and specificity of this recombinant antigen-based ELISA technique was a objective and easily automated method and it could be used as a both screening and confirmatory test for syphilis. |
