      目的 探讨超声心动图参数预测早产儿动脉导管早期自然关闭的价值。方法 对165例早产儿分别于出生后24 h、72 h及7天行超声心动图检查,将7天内动脉导管未自然关闭者归为动脉导管未闭(PDA)组(n=37),7天内自然关闭者为对照组(n=128)。对出生后24 h首次超声检查参数进行统计学分析,对差异有统计学意义的参数行多因素Logistic逐步回归分析。结果 PDA组左心房收缩末期内径/主动脉根部内径、分流束最细处动脉导管宽度、肺动脉压均高于对照组(P均<0.05),最大分流速度、降主动脉与肺动脉压差(ΔPs)、卵圆孔分流速度均小于对照组(P均<0.05)。多因素Logistic逐步回归分析结果显示,24 h首次超声心动图参数中,仅ΔPs与动脉导管早期自然关闭相关(P=0.02)。结论 超声心动图检查参数预测早产儿动脉导管早期自然关闭具有重要意义,ΔPs与动脉导管早期自然关闭相关。

      Objective To investigate the value of echocardiographic parameters for predicting spontaneous closure of ductus arteriosus in preterm infants. Methods One hundred and sixty-five preterm infants were included. Cardiac ultrasonographic scans were performed in all newborns within 24 h after birth and on the third and seventh days of life. On the seventh day examination, preterm infants were divided into PDA group (ductus arteriosus not spontaneously closed, n=37) and control group (ductus arteriosus spontaneously closed, n=128). Statistical analysis was performed on the parameters of the first echocardiography at 24 h after birth, and parameters with statistically significant differences were analyzed using multivariate Logistic stepwise regression analysis. Results The ratio of left atrialdiameter/aortic root inside diameter, width of ductus arteriosus in finest part of the splitter and pulmonary artery pressure in PDA group were both significantly higher than those in control group (all P<0.05), while the mean maximum velocity across ductus arteriosus, pressure difference between the descending aorta and pulmonary artery (ΔPs) and the shunt velocity of patent foramen in PDA group were significantly lower than those in control group (all P<0.05). Among the first 24 h echocardiographic parameters, multivariate Logistic stepwise regression analysis showed only ΔPs related to early spontaneous closure of ductus arteriosus (P=0.02). Conclusion Echocardiographic parameters have important value for predicting spontaneous closure of ductus arteriosus in preterm infants, among which ΔPs has significant correlation with early spontaneous closure of ductus arteriosus.
